INTRODUCTION
The disease or illness refers to situation of an individual in which they are not able to functions
properly and feel pain or uneasy. It consists number of health issues which can consist like high
blood pressure, obesity, injury, diabetes and many more. Basically, it includes specific cause
behind every disease or illness like overweight, substances abuse, injury, inappropriate
environment, lack of physical activity and so on (Adler, Glymour and Fielding, 2016). In this
context of this project, it is based on case study of patient named Eileen Poole who gets admitted
in emergency department because of pain in wrist developed by a fall. The given report will
going to focus on brief overview of selected case of patient including their problems and medical
history. It will also include social determinants with relevant issues to be addressed.
MAIN BODY
The basic information related to selected case scenario of patient
In context of given case, the patient is 74 years lady named Eileen Poole complaint about pain
along with decrease amount of wrist movement as well as deformity swelling. It includes the
condition of facing fall of by patient which leads to extreme level of wrist pain and interrupted
movement of the same. However, it has been analysed that Eileen has lost her husband 6 months
later due to which she lost her appetite that is responsible for developing internal weakness in
body. She was very active in garden and managing home earlier when her husband was alive.
The medical history of Mrs Eileen Poole involves various health issues of illnesses like
hypertension, osteoarthritis, hypercholesteremia and type 2 diabetes. Meanwhile, the selected
patient also diagnosed with psychological problem of condition having mild depression and she
had bilateral knee replacement before 5 years recover in rehabilitation with physiotherapy. The
patient use several medications such as Atorvastatin (Lipitor) 40mg mane, Candesartan (Avapro)
32mg mane, Panadol osteo 1330gm TDS, Glucovance 250mg/125mg BD, Sertraline 50mg nocte
and Aspirin 100mg mane.

The health related social determinants include several condition when an individual born, live,
work, age and grow which plays an important role for improving health condition (Nutbeam,
McGill and Premkumar, 2018). It includes the fact that such situations are formulated with the

help of resources & power at global level, local & national context and distributing money
(Sharma, Pinto and Kumagai, 2018). However, it has been analysed that social determinates are
helpful to increase recovery rate of an individual along with supporting a person for avoiding
related complexities (Kidd, 2020). According to case scenario related to Eileen Poole, several
social components involves healthcare & health, formulated atmosphere & neighbourhood,
community & social context, economic stability and education. There are few of other social
elements having responsibility in regards to health such as marital status, place of residence,
crowding, transportation systems, discrimination and many more (Milner and et. al., 2016).
Meanwhile, Eileen Poole wants care of close ones and family nut she stays alone after death of
her husband. Her son and daughter are also stayed separated from Eileen which is a factor not
appropriate for her wellbeing. Moreover, there are several social determinants including stress,
early life, social exclusion, work, unemployment, social support, addiction, food, transport and
social involvement (Watt and et. al., 2016). In case of Eileen, she has very limited social
engagement and condition of unemployment factor of loneliness that is not favourable for the
patient.
Explain the way of how problems can raise from the case studies that are related to several
designated social factors of health
In context of Eileen Poole, it has been analysed that her disturbed lifestyle is responsible for her
weak and unhealthy condition which is required to be focussed in future to maintain their
wellbeing. It is observed that she live alone after death of her husband due to which she lost her
appetite and remain alone all the time which also has potential to make her unhealthy. Basically,
it is observed that social isolation is responsible for developing loss of appetite to acquire weak
condition. It consist the fact that a person needs family support at the age of 74 which is not
gained by selected patient due to which she acquire unhealthy condition. However, it is observed
that several issues arise by social determinants in given case which is explained here.
Lack of social engagement – This can be described as a condition when a person remain alone
all the time and never participated in social gathering and relevant activity in community or
family. It includes the circumstances of people remain alone in the house and never come out to
meet with neighbours and friends that is known as isolated condition (da Fonseca and Avenetti,
2017). In context of Mrs Eileen Poole, she is facing isolation because of lack of social
engagement as she lives alone without her children after death of her husband. She does not go

out much and remain in house alone as her son and daughter lives somewhere else. She needs
family support and regular gathering of friends and neighbours which is helpful to make her busy
in daily living along with avoiding to think extra about her alone condition in order to remain
wellbeing.
Neighbourhood and built environment – The neighbourhood plays an important role in for
betterment of health by social support of neighbours to an individual (Di Marino and et. al.,
2018). A lonely condition is worst for an individual because it impact negatively in their physical
as well as mental health state (Jeon and Heo, 2019). According to the given case of Mrs Eileen
Poole, it is an issue that she does not have supportive neighbourhood and environment of pave
whereas she lives due to which her loneliness become worsen for her. Moreover, lack of
supportive neighbourhood is responsible for making weak condition of patient worst (Albertini,
2016).
Stress – This can be defined as stressed condition which occurs due to death of Eileen’s husband
which get worsen because she lives along without her son and daughter and their family. It
includes the issue that she thinks all the time about her husband and never eats properly which
develop problem of internal weakness that develop health problem leading to a fall and wrist
pain (Dallos and et. al., 2017). Meanwhile, it has been evaluated that stressed condition of Mrs
Eileen Poole develops inappropriate psychological and physical health condition that leads to a
fall and wrist pain.
The possible recommendations in order to address the determined problems for selected patient
for enhancing outcomes related to patient.
In context of given patient, it has been analysed that medical treatment is applicable to recover
with wrist pain and movement along with making her healthy. It includes the fact that patient
needs extra care and support along with social involvement after becoming wellbeing which
avoid problem of loss of appetite that improve entire health condition. However, the social
support and family care is must for this old lady to avoid further severe health problems to stay
healthy and comfortable (Choi and Ryu, 2017). It is necessary to make an individual feel
engaged with family member and social activities for removing risk of occurring problems
including falls and relevant injuries. Some of recommendations for Eileen Poole are given
below.

Considering the factor of lack of social engagement, it has been recommended that son or
daughter of patient should permanently stay with Mrs Eileen Poole for taking care of her all the
time. It is favourable for patient to have proper meals and remains busy with family members
that are suitable for maintaining her psychological as well as physical health (Temesgen, Chien
and Bressington, 2019). However, it is necessary to encourage her to have morning and evening
walk in Community Park with ladies of her age group which make her healthy. Moreover, the
children of patient should make her feel cared and supported which improve her emotion, moral,
spiritual and psychological state properly.
By analysing the factor of neighbourhood and built environment, it has been suggested that
patient should encourage participating in community gatherings and festivals. It includes the
criterion of organising social program or event by daughter or son of Mrs Eileen Poole for
formulating healthy relation of neighbourhood along with establishing environment of mutual
understanding in community. It will facilitate to gain support of neighbourhood and build health
environment that is favourable for patient to improve better health condition and quality of life
(Ohama and Fukui, 2020).
On the other hand, it has been recommended that family support should be provided to Mrs
Eileen Poole by which can be done with her children and relatives. She get satisfied and stay
happy when her son and daughter stay with her because alone situation is responsible for
facilitating complicated stressed condition of patient that is required to be improved in effective
manner. However, it has been analysed that patient needs family support and gathering which
helps to manage her psychological state with extreme sadness of losing her husband for
improving comfort level to spend life properly.
CONCLUSION
From the above assignment, the summary of this project includes that specific health problem is
responsible for developing issue to spend regular life routine. It includes the fact that death of
loved one impact potentially health of individual from which overcome is very important.
However, the close friends, relatives and family members should provide support to overcome
with trauma of someone’s death and they will avoid acquiring severe health problem to continue
their life properly. A person should remain involved with neighbours and family to stay
comfortable and healthy ahead.
