Rationale
Universal Health requires a RN navigation form for the oncology services that will be used in
both operational areas named Oncology North and Oncology South. The purpose of this
development was to implement an EHR or Electronic Health Record system integrated with
both Oncology North and Oncology South and that can capable to store electronic data about
patients or applicant. To develop a universal form MS Word software has been used where
the data can be directly inserted for each applicant or patient to form an EHR database system
(Oh et al. 2015). The system is also incorporated with export option where the EHR database
of Oncology RN Navigation can be exported to an excel sheet or word. To avoid complexity
drop down option has been used. Furthermore, the clinical assessment section has been
minimized so that the detailed data can be inserted in a single database cell. Along with that,
from Oncology North intake form the financial details and medical history data structure
format have been adopted. In order to increase the simplicity the form has been segregated
into 5 major sections namely applicant details, medical history, personal details, financial
details and treatment details (McMurray & Cooper, 2017). In order to ensure the provision
for inserting the program are specifications the two optioned drop down many has been used
where the user has to select either Oncology North or Oncology South. The form has been
developed with extreme simplicity so that the user can used this form without any special
training.
