Q1. Develop a set of own responsibilities and performance objectives at work
[P1.1, M1]
A community support worker has to take care of various responsibilities for attaining
performance objectives that will enable the maintenance of work standards. A
community support worker has to develop and maintain a healthy and positive
working relationship with the care seekers so that a pleasant and homely
environment is developed for the service seekers. Community support worker has
the responsibility to participate and get involved in the various risk programmes and
assessments, support plans, make written records according to the organisational
guidelines to ensure the development of the service users (Thedtgroup.org, 2019).
Apart from this, they should participate in different activities to accompany the
service users to make it beneficial for them to the best possible extent.
The role of a community support worker also requires being interactive and having
good communication skills so that the service users can communicate their problems
or doubts easily with me so that they can be provided with a solution that will allow
them to improve their medical condition. A community support worker should have
time management and IT skills so that adequate time is given to each service
seeker. IT skills will allow the community support worker to maintain a record of vital
information of different service seekers that can be used for future reference so that
the service seeker receives treatment of better quality. Along with patience, a
community support worker should have resilience so that the even difficult situations
can be handled by the support worker effectively and does not result in loss of calm.
Patience and resilience will ensure the correct decision-making process even in
times of difficulty or complexity.
Q2. Evaluate your effectiveness against your defined performance objectives
in Q1. [P1.2, D1]
As a community support worker working in The Disabilities Trust, I need to possess
certain skills that will allow me to fulfil the above-discussed responsibilities and lead
to the attainment of professional objectives. I have good communication skills that
allow me to develop relationships with different service seekers, but I am unable to
maintain relationships with the service users, which makes it difficult for me to help

them by analysing their behavioural change and other aspects (Mwai et al., 2013). I
can develop a positive and pleasant environment for the patients to help them cure
faster and in a better manner. I encourage the service seekers to participate in
various activities to make sure that they are happy and I often accompany them in
the activities to maintain a positive competitive spirit in them. It allows me to provide
them with mental peace and happiness that will help in the faster recovery of the
service seekers from the medical conditions.
I have patience and resilience that allows me to take correct decisions in difficult
situations. Even in chaotic situations, I can maintain my calm and taken decisions
that will be appropriate for everybody. However, I need to improve my time
management and IT skills so that I can allot appropriate time to every service seeker
and provide them with best and quality service to help them in improving their
condition. It will further enable me to maintain records and vital information about the
case of different service users so that the information can be used for further medical
treatment (Deci and Ryan, 2012). It can also be used for reference purpose for other
service users who are undergoing similar conditions.
Q3. Using your weakness identified in your performance, make a
recommendation for improvement [P1.3]
Community support workers should adopt various measures that will allow them to
improve their skills and knowledge, as it will allow them to take care of the service
users in a better manner and lead to growth and development in their professional
careers. I need to improve my skills by adopting measures that can be applied in my
practical work experience and provides me with a way to evaluate and analyse the
results obtained from the measures. As discussed above, some of my skills are
weak that should be enhanced for better performance that will allow me to serve the
users better. I need to improve my communication skills by learning about the
different models and frameworks of communication. Appropriate theoretical
knowledge of the different ways in which communication skills can be improved will
allow me to implement it in practical life and understand the measures that can be
most useful for particular situations.
Even in the case of time management skills and IT skills, I can read different articles

for improving my IT skills and manage my time effectively. I can participate in
workshops to improve my IT skills, but I will have to ensure that the workshop is not
during my working hours as it will make it difficult for me to attend it (Gopalan et al.,
2012). For managing my time, I will start using a planner and sticky notes to divide
my time for different activities and service users so that I can manage my time.
Sticky notes will allow me to remember tasks and activities leading to the completion
of every task. The planner will also enable me to prioritise my work and manage my
workload so that there is no mistake or missing deadlines.
Q4. Review how you would use motivational techniques to improve the quality
of performance [P1.4]
Motivation is essential for every employee as it will ensure that they always involved
in their work and do not get bored with it. Different measures can be adopted for
motivating employees and the measures adopted by the management of an
organisation depends on the work and the type of employees. I motivate myself by
maintaining healthy completion with my colleagues so that I can improve my
organisational performance. My passion for taking care of people who are unable to
take care of themselves ensures that I stay motivated towards my work. I want to
improve my performance to be the best, and for this, I make sure that I learn new
things and incorporate them into my work practice in order to improve my
performance (Elwyn et al., 2014). These measures ensure that I stay motivated
towards my work and my quest for new information allows me to keep an open
learning attitude and gather varied information from different sources.
Further measures are also adapted to ensuring high motivation in my professional
career. I set personal objectives that will help in my professional life and allow me to
improve my organisational professional. When I achieve the personal objectives, I
receive rewards that were determined along with the objective. The rewards can be
personal or professional but valuable enough to push me so that I increase my
efforts and hard work for the attainment of personal objectives (Jaskiewicz and
Tulenko, 2012). I also use reflective practices to analyse my actions and
experiences to identify the instances where I performed well as well as the instances
where I can adopt alternate measures for better performance. It allows me to

Q5. Identify effective time management strategies you have used at work to aid
your performance. [P2.3]
A community support worker has to perform various tasks and activities, at times
with their services users and at times single-handedly, which makes it essential for
them to manage their time accordingly so that the tasks are conducted successfully
and on time. Different time management strategies can be adopted to manage the
various tasks and prioritise tasks for better management of the workload (Cherry and
Jacob, 2016). A planner or schedule can be used for keeping a record of the
different tasks and allot each task a particular time slot. I will have to ensure that the
task is completed within the predetermined time slot to make the time management
strategy successful. It will enable me to set tasks according to their extent of priority
so that they can be completed before deadlines and none of the tasks is missed.
Community support worker can also confirm from the service users about their visits
so that they are no sudden cancellation of appointments and the time is not wasted.
Sticky notes can come handy in the management of time and different tasks as it will
facilitate remembering of different tasks and activities. It can also be used to keep in
mind minute details that will be relevant in conducting tasks or taking care of patients
(Bottery, 2013).
Q6 Explain the roles people play in a team and how they can work together to
achieve shared goals. [P3.1]
In a health and social care organisation, teams usually comprise of members
belonging to multidisciplinary fields and expertise. The role of every member is to
execute a specific role or service to different users. Most importantly, each member
at the health and social care setting, each member has different responsibilities as
per the shared goal and their personal skill set and talents. In general, role of team
members is to follow the set guidelines, participate in the decision-making process,
brainstorm ideas and execute actions appropriately is the fundamental aspect that is
carried out. Health and social care is a challenging profession, as it involves different
tasks and responsibilities to be implemented, therefore, the role of different teams is

to work on specific areas and departments. At The Disabilities Trust, the primary role
of team members is to provide the individuals with disability with respite care
(Thedtgroup.org, 2019). In general, the role of team members is to provide
temporary or emergency temporary care to individuals and their families.
The primary objective of the individuals in the team is to develop innovative ideas
and processes in order to help the individuals with the development or intellectual
disabilities. Their extensive services include the actions to provide additional support
to their family members. In order to accomplish, the role of members at The
Disabilities Trust is to develop an environment, which supports individuals to live
independently and contribute to the community as well. Their role also includes
providing supervision to protect a person’s safety in the absence of family member
and help them meet their basic needs (Reeves et al., 2017).
All these actions and objectives can be fulfilled, if there is a strong network of
communication and clearly defined roles and responsibilities. Members can develop
a proper understanding among themselves so as to determine the actions and
services that each one of them will undertake and follow, thereby, establishing a
strong sense of support and interdependency to carry out actions as a team.
Q7 Analyse team dynamics at your health & social care work place [P3.2, M2]
At The Disabilities Trust, there are different teams that are provided with different
tasks and responsibilities so as to cover the entire services and plans that the
organisation intends to accomplish. Working, as a team member at The Disabilities
Trust is simple task, as the group members are provided with proper understanding
and knowledge as to what needs to be executed, what is the plan and different
aspects about aim. Important aspect of health and social care is that every individual
with disabled has different needs and requirements, therefore, often leading to
complications and undesirable situations. However, at The Disabilities Trust, team
members are provided with detailed training sessions, where different situations and
their respective solutions are explained in detail. Along with this, the senior
professionals are always available for team members to provide supervision and
guidance, thereby, establishing a healthy environment for work. These aspects have
high impact on the team dynamics and their performance. Team dynamics mostly
refers to the behaviours and systems that exist within a team (Freund et al., 2015).

At The Disabilities Trust, it is common or usual to witness team members to be
highly optimistic and positive towards their roles and responsibilities. The work
culture at the charitable organisation is highly motivating and encouraging, which
leads the members to be more determined and dedicated towards their work. The
best part is that the team members have proper line of communication established
among all the networks and support services, leading to prompt action and better
handling of emergencies (Stevens et al., 2016).
However, at times, it has been observed that teams experience few issues and
conflicts. Although, there is an open communication that exists among team
members, however, it has been observed that members do not share complete
information during the change of their shifts, leading to issues with disabled
individuals and their families. Recently, issues related to the transparency at work
were witnessed that resulted in team conflicts and misunderstandings.
Q8 Based on your analysis of teams in Q6 & Q7 suggests alternative ways to
complete a task as a team and achieve the goals in Health and Social Care
sector. [P3.3]
The primary goal of Health and Social care organisation is to deliver the best quality
support and care to all individuals as per their necessity and requirements. As per
the above analysis and assessment, it has been observed that there exists a strong
team dynamics within the organisation that has helped the charitable organisation to
extend their services across the country and multiple individuals (Kilpatrick et al.,
2016). However, few issues and problems related to the transparency and
interpersonal communication was observed at The Disabilities Trust. In order to
resolve these aspects, few recommendations and suggestions have been provided
with the objective that it will help the management to build robust and proactive
teams:
In order to resolve the issue of sharing of information among team members after the
change of shifts can only be resolved with the development of a strong leader, so as
to ensure that all the details and information are shared among these two individuals
of teams and thereby followed accordingly. The role of leader will be to instruct the

In order to increase the transparency at work and enable the team members to have
