Assignment 4: Group Project
Overview of a program
Chronic illness management becomes a crucial task for various people. It is essential to
undertake relevant and suitable steps so that chronic and deadly diseases manage effectively. The
current topic is focused on the maintenance of a fitness routine. It is essential to work on fitness
routine by taking the appropriate dietary plan. Thus, a proper fitness routine is essential which
include exercise, appropriate diet plan and other section which makes a person fit. Therefore, a
fitness routine is helpful to minimize chronic illness causes.
Rationale of program
According to Woodward (2018), physical activity is helpful to fight with intra-abdominal fat so
that chances of heart diseases, cancer and diabetes can reduce. Exercise is a part of the fitness
routine which assists in minimizing the fat and reduces weight, which is considered a hidden
factor for much chronic illness. One of the studies has suggested that exercise loses 5.5% fat
from women body with regular use and 0.75% fat from men body. This program is essential for
managing an active and healthy lifestyle. Fitness routine also focuses on the current diet like
intake of fresh vegetables and fruits. Proper control is required on the diet which considers
power-on consumption of burger, and other food material. Thus, this program is essential from
the perspective of sufficient health to minimise major chronic diseases factors.
On the basis of Powers, et. al. (2017), diet and lifestyle change is associated with a decline in
chronic diseases. It is required to facilitate a healthy lifestyle so that the individual becomes
robust to deal with various chronic diseases. Firstly, a fitness routine includes avoidance of
tobacco. It is essential for a person suffering from chronic illness to avoid the use of smoking

intake of alcohol, cigarette etc. Along with this, a person is required to do exercise regularly so
that their weight gets in control.
Moreover, doing some physical activities on a regular basis e.g. use stairs to reach the office,
play games and another medium. Thus, such methods are helpful to control chronic diseases and
illness accurately. Moreover, take vegetables and fruits at breakfast and eat light at night. This
balanced diet is essential and beneficial to minimize chronic illness and diseases. Thus, such a
program is essential and completely rationale with the work so that things get done effectively.
Two major goals of the program
A fitness routine management is essential and indicates some goals which strata as follow:
Avoid and minimize the significant chronic diseases like heart issue, cardiovascular etc.
Another goal is to spread the healthy lifestyle aspects to the new generation people to
improve their immune system.
These are the significant goals of fitness program so that chronic illness manages effectively. It is
essential to implement such a plan and program on a regular basis. It will be going to make the
young generation active and enable them to deal with chronic illness. Moreover, a healthy
lifestyle will be provided which control early death chances.
Planned activities which help the individual to meet the goals of the program
Planned activities are required to become healthy and deal with chronic diseases properly.
According to the Hallgren, et. al. (2017), exercise is an essential factor with a balanced diet so

that fitness routine can be followed. A fitness routine includes different planned activities
through which a person can handle distinct chronic illness properly:
Drink an appropriate amount of water
Consume fruits and vegetables in breakfast
Do cardio and resistance exercise on a regular basis like running, walking
Take a heavy diet in lunch
Do physical activity more to digest lunch
Avoid the use of alcohol, cigarette etc.
Take light dinner
Take appropriate sleep at night as it also helps to maintain regularity in routine
All these are planned activities which enable to deal with chronic illness and diseases properly.
All these activities are helpful to facilitate the program of fitness routine properly. Moreover,
these activities are helpful to accomplish the goals of the program. Thus, to maintain an
appropriate weight, it is required to undertake planned activities. Consistency is essential for
fitness routine like a proper dietary plan, exercise to remain fit and healthy. This also brings a
change in the lifestyle of a person so that they remain healthy for entire life.
Problems related to program and use of essential steps
Barriers are considered as obstacles which arise while initiating a plan regularly and adequately.
It is necessary to undertake and determined significant barriers related to program effectively and

undertake relevant steps to minimise them. The important barrier which is facing by group and
experience causing the delay in participation includes:
Late night sleep
Inappropriate consumption of diet which imposes health issues
These two are the significant barriers which indulge with people of a team. Thus, the fitness
routine plan is essential for group members. This plan is helpful to improvise their health issues
and facilitate them to deal with different health issues. Thus, program execution is important to
minimise the significant issues and problems related to team members.
Include requested information
Following are some information required for execution of a project. This includes information
about different participants in the study. Along with this, it is focused on the time frame of a
program. There are three families evaluated for the study with the help of four group members.
Moreover, all members incorporate activities for one month in a regular manner. Human and
finance are two significant sources which depict as an essential one for a project. Human and
basic evaluation criteria are required for the project program. Thus, an action plan is prepared for
the required information about the fitness routine program:

appropriately determined.
The time frame of the program This program is executed for one month.
Resources needed to provide program Physical interaction is required to execute the
plan. Human resources and finance are two
significant resources considered as an essential
frame for fitness routine program.
Resources needed to participate in the
program
The significant resources required for the
program include human intervention.
Identification of evaluation method required data and plan for data collection
Data which is identified as essential for evaluation include a secondary aspect. A secondary
source of information is necessary for the assessment so that current health status and chronic
diseases determine in Canada. Along with this, a plan is essential to collate information from
different sources defines as follow:
Group member Work
First member Determine a program required to manage
chronic diseases

Second member Define activities necessary to control chronic
diseases
Third member Work on requested information and reach
people for program development.
Fourth member Work on the data evaluation process.
Different evaluation techniques have anticipated for the research which facilitates the study to
make it relevant. Research gets facilitated and evaluated with three major approaches include
goal based, process-based and outcome-based (Pearce, 2016). It is required to determine the best
method to assess the proposed study focus on fitness routine program.
The program is initiated to control chronic illness management in young adults and ageing
people. It is required to focus and use the relevant technique of evaluation so that study becomes
useful and applicable with proposed targets.
This study includes an outcome-based evaluation technique. The primary reason for choosing an
outcome-based evaluation technique is to examine the broader impact, and functional effects
serve through the result of the program. This program helps better results to minimise chronic
illness and protect people in the long term. Along with this, this program is evaluated with a
subjective approach as well which includes qualitative aspects. Three families are observed for
one month, and it gets to determine that their health status become appropriate. Thus, such
