INTRODUCTION
Managing change is the essential term or part of the organisation management which
involve the various steps, activities, approaches and theoretical support for making the important
changes with in organisation in order to make it effective and competitive, by meeting the
changing requirements of the consumers and situation (Anderson, Nasr, and Rayburn, 2018).
Change management is the combine function of different approaches and strategies which work
together to help, support then organisation team and individual in order to make company
changes. Govan health centre is the community health care centre of Glasgow, Scotland UK.
This report is about organisation improvement strategies development by the analysis of issues
needed to be change and then implementation of changing approaches along with evaluation of
its effects in organisation development and growth.
MAIN BODY
Background
Two vulnerable patients were joined the Govan SHIP under which their mental
condition's were improved in comparison to their previous condition. Such as the 22 year old
patient was having the personality disorder and anxiety illness. This person was presented to
A&E 590 times. Another patient was 66 year old person with the mental condition, know as the
dementia and there was the risk of his safety and care due to the difficult situation of his
daughter addiction.
Analysis of the situation with in the organisation where changes is essential and what has to be
changed.
Patient-level
Need for a reduction in a & e visits- This is the the important area of the improvement
in context to the patient level. So this can be done by changing some procedures of patient health
management, with in residential care services, home care services and at the hospital. Changes
is require to be done in the form of effective and improved care facility's and services which will
improve the patient condition and reduce the chances of the accident.
Improved patient safety in their social environment- There is need to develop and
implement more safety interventions in context to the patient safety in their social environment.

This can be done by the development of more regulations, awareness, people and patient
knowledge and personal nurse facility for care and safety at every stage.
Improved quality of care and well-being of vulnerable patients- Mental health care
organisation and residential care organisation have to improve their quality of care and well-
being of vulnerable patient with effective sessions and health, hygiene management with more
facility's and evidence based practices.
Organisational-level
With in the developing nation, people requirement and different health care issues, health
care organisation need to change their organisation process and techniques with the aid of new
technological implementation and unique patient management strategies. In order to meet
competitive edges and provide the best care to patients. In order to analyse the situation of the
change organisation has to conduct the internal and external factor analysis by the aid of
PESTEL analysis and SWOT analysis (Bailey, 2019).
Internal and external factors are the essential element which highly influence the business
strategies of organisation, it involve the consumers needs, finical condition, market and
organisation techniques, law of the nation and so on. Govan SHIP using this change managing
function in order to change in the person care process with in the application of GP practices.
Brief explanation of the organisation situation analysis with the aid of effective tool and model is
given below.
PESTEL analysis
This is one of the most effective external factor analysis tool which help the organisation
to analyse their organisation needs for the improvement and changes. This include the analysis
of the political, economical, social, technological, environmental and legal factor in order to
understand the are of the organisation which should be change.
Political- It involve the analysis of political factor's of the market and nation related to
the organisation, such as rules and regulation for the company process and management. In case
of the Govan SHIP political factor is law of people effective care and safety, which influence the
organisation to change their process of care such as political factor's create forceful situation

Economical- This involve the finical condition's of the patient in context the health care
organisation and nation economy, according to which health care organisation have to develop
their process plan. Financial problem of the patient is the situation, which influence the
organisation to make some care process which can be provided to the patient at lower cost to in
free of cost. Health care sector economy is support by the government so they have to focus on

political rules for the environment management organisation have to avoid unnecessary waste of

Legal- This factor is involving changing rules and regulations of the government in
context to the organisation management functions and operations. Such as legal act mental

one individual. As huge number of the mentally ill adults patient create the inappropriate and
unspecific situation with in the organisation (Carini, and et. al., 2019). So for solving this
situation organisation focus on the health care practices improvement along with changes in
facilities with better options which can aid to improve that situation.
Evaluation of the different theory's and concepts of process change which help to get the support
of those who get affected by change.
Change management is important for every type of organisation, and in context to the
health care organisation it is similarly important. There are the different models and theory's of
the change management given by the various specialist and psychologist. This model help the
organisation manager to make the changes with in the organisation and instruct their team to
adjust with in the changed process (Levin, and Crighton, 2019). This is essentials in order to
sustain in the environment with successful plan. In health care it can be related to the facilities,
techniques, process, technology's etc.
Lewin's Change Management- Kurt Lewin born in 1890, was the Psychologist who
was developed this model of change, which is divided into three steps, that is unfreeze- change-
freeze.
Unfreezing- This is the firs step of change making, in which every one have to prepare to
accept the new changes. It start from the braking of present process and techniques before the
selection and implementation of new process. Their is requirement of the strong support and
strong efforts to change. In context to the Govan SHIP project they have to first remove some
inappropriate process and make the people understand new process. Such as this project unfree-
zed the long term process of attendances at A&E.
Change- This is the second step where people have to analyse new way of process by
resolving their uncertainty. Once the new process and techniques get selected then it should be
implemented with in the organisation system. Govan SHIP have selected the new process of the
patient care under the multidisciplinary approaches and Gp practices, with the support of social
care workers and some health care professionals. Where workers and professionals have to
understand the new process and practices.

step is to refreeze it with, organisation chart, rules, regulation and job role differentiation. Then
Govan SHIP have make this permanent, freeze process, by distributing the different GP practices
and role to the workers and professionals along with code of conduct. Such as they have selected
to give the holistic care process at home with home cleaning and GP appointment.
Strength

Govan SHIP- This project of Govan SHIP (Social and Health Integrated Partnership)
was launched in April 2015 in order to change and improve the patient care by the aid of general
practices and Health & Social Care Partnerships collaboration opportunity (Nahin, and et. al.,

the various general care practices, which has to be provided with in the four additional weekly

activities such as information, children and families involvement along with their care, frail and
elderly, unscheduled care.
According to those to patient stories this project have used this three approaches in order
to reduce the numbers of cases at A&E, to reduce the demand of acute and residential care and to
improve the chronic disease management. By the aid of different practices and process of these
three approaches and along with strategic plan for its implementation given below.
Strategic plan for implementation of approaches
In this case of the study Govan SHIP plan is to develop the multidisciplinary team from
the professionals of different hospital and community, such as involving social care
professionals and patient management professionals for the crisis situation (Prytherch, and et.
al., 2019). Patient for this project objective achievement is selected from the vulnerable category,
for example patient who is suffering from the mental disorders and addiction problems and along
with the complex needs of the care.

that specific person to share their view which help them to reduce stress and feel free. This also
used in appropriate way to reduce the more complication which is arise further. Moreover, this
approach help him to make collaborative behaviour with other and they share their thought and
view on a particular topic to other can minimise the complex situation which is created when
person make it self isolated. This implementation become sustainable when proper medication is
also provided in minimum dose like using of anti-anxiety medication such as diazepam,
alprazolam and may more. This medication also help to reduce stress and support strategies

the social work team and addiction team start to taking care of carer. In this case, the psychiatric

they try to make and fact which help them to stop doing any kind of addiction which may harm
the life and does not create situation which is life threatening. This strategies only work when
engagement is with carer to manage the changes which is required to treat and make away from
substance that carer used to take for their addiction. The plan which is implemented in such a
way which help carer to overcome from this all.

From the above study it has been analysed that for the success and growth of any type of

requirements and implement the different approaches to make changes. So this management
function involve different steps such as analysis of the situation and factors which has to be
change, this can be done by the use of internal and external environment factors analysis models
such as PESTEL analysis and SWOT analysis. Then in order to get the support of different
change influencing factors organisation should use model and concept of the change
management such as Lewin's Change Management, kotter's theor, ADKAR model and so on.
Then after the involvement of change management model next is to select and justify approaches
along with the development of strategic approach for its implementation. In context to the Govan
SHIP their changing approach is including the Multidisciplinary approach and GP practises. This
approaches and strategic plane is implemented by the aid of change management theory's and
evaluation of changes sustainability is done by the help of data analysis techniques.
