Abstract
This assignment is about the description of milestone comes in a path of an individual while
launching the health awareness program to get rid of chronic illness. This assignment will
emphasize on financial and evaluation plans. This program requires human and financial
resource in which an individual has to arrange funds from sources like donations and crowd-
funding to recruit medical professionals. A benchmarking tool used to analyze the effectiveness
of a program and patient’s data is measured to ensure the proper working of this plan.

Chronic illness health awareness program develops by an individual in the pike county to
aware people to make them more serious about their health. For executing this program, it is
essential to identify the resources that will utilize in the program is mention as below:
Human resources- For launching health awareness program, the staffing requirements identifies
by program manager to meet the needs of recruiting professional medical and Para-medical staff.

this event requires heavy sum. The social workers will also help in this campaign in enhancing
people’s spirit in taking initiative for their own and someone else’s health and keep their nation
illness free.
Suggestions for ensuring the financial sustainability of the program
It is advisable to the organization to search for different sources of arranging funds to be
financially stable for launching this program on the floor. Sources of funds which an entity will
need for meeting the project goals and the objectives will consist of crowd funding, a subsidy
from the government, and donations from the Kentucky health department (Hissa and et al.,
2019, pp. 23-39). The program will successfully implement by meeting the funding needs of this
project. It is recommended to use the dashboard system to keep track of all the activities of this
event to track its progress that all are completed on time or not.

Checking patient’s health history- In checking the effectiveness of a program, the previous

Technology in treatments- Measuring the data of the technology utilize in treating the diseases

Patients who will attend the event- The information of all the patients who attended this self-
management program to fight against the chronic illness.
Tools to measure the effectiveness of the program in reducing the health disparities
Patient safety- The safety of the patient is ensured by giving entry and exit to a patient in the

card is given to a patient to recognize a person’s health history easily by scanning their health

This program will open for all classes of the society in taking benefit of all the health
services offers during the event. Patients are categorized into different color codes to allow a
doctor in recognizing one patient from another. This will, in turn, avoid isolating a diseased
