INTRODUCTION
Modifiable risk factors refers to the factor which includes diabetes, high blood pressure,
smoking, high blood cholesterol and overweight. There are many risk factors which can be
changed the behaviour of any individual. Due to this risk factor, there are different cause of
change in behaviour of the patient with chronic disease or disability. In this report, there is
discussion about different modifiable risk factors behaviour along with their contribution to the
behaviour. There is also discussion about impact of this behaviour on the chronic illness or
disability. There is also discussion about identification of health behaviour theory which can
explain appropriately about the behaviour which can used as the intervention for modifying
behaviour. There is also a discussion about innovative and practical strategies which can be
implemented within home by the patient for the behaviour modification. There is also a
discussion about the reason for the relevant strategy which can be used (Pfeifer, (2018)).
MAIN BODY
Summarizing case study along with definition of chosen modifiable risk factor behaviour
Parker is a student who is 19 years old and studying marketing. Parker has medical
history of anxiety, depression, asthma and attention deflect Hyperactivity disorder. Mother of
Parker was the one who managed and coordinated his schedules, doctor appointments,
medications and diet. When she passed away, Parker got along and lives alone and due to which
he not able to maintain his regular life and not able to control the prescribed medications and
sometimes forget to attend appointments. Due to having anxiety and poor attendance he lost his
job at McDonald's. Parker suffered form ADHA and not sure about repeated prescriptions.
Parker has been referred to the GP who has advised him to take help of occupational therapist,
physiotherapist, dietitian to take appointments within next 6 weeks. Modifiable risk factors
behaviour are some of the respective can influence the behaviour of Parker which is regarding
chronic illness or disability. Parker is suffering from ADHA that is attention deflect a
hyperactivity disorder where a person is not able to remember and always getting confused.
There is hyperactivity within patient and can’t sit still. Parker is not able to stay calm or quiet
and is also not able to concentrate on any task. Due to excessive physical movement and
excessive talking, he is not able to wait for his turn (Minakawa, (2019)). He have also the
symptom of acting without thinking which creates difficulty for Parker. Due to interrupting in

the others conversation creates a difficulty for Parker and shows the lower standard of life. There
can be the risk factor which is brain disorder. This generally affects the attention, sit stills and
and control behaviour regarding controlling body. This is the disorder which generally occurs in
children or teens. This can also continue within adulthood which is generally diagnosed as
mental disorder in children. There is no any specific risk factor is claimed regarding ADHD.
According to researcher, it has been analyzed that Genetics plays an important role in this type of
disorder or there may be a chance of getting brain injury which can lead to create the situation of
attention deficit hyperactivity disorder. There is a chance of genetic disorder can create a
difficult for the Parker to live a standard life. Due to different health related issues like
depression, anxiety, ADHD which creates high difficulty to live a standard of life. It is really
difficult to behave normal due to presence of such type of chronic disease a disability. This lead
to change in behaviour due to which Parker is not able to behave like normal people and is not
able to show himself normal during meeting with any person, healthcare professionals or
therapists (Vikulova, (2019)).

disability
There are some of the factor like anxiety, depression and ADHD which creates difficulty
for Parker to contribute attention towards the behaviour. This is the one which is due to having
different health related issues and due to not able to manage lifestyle in effective way, this create
a behaviour change. There is a small change in behaviour due to the disorder of ADHD which is
hyperactivity with Parker. This is one of the reason which creates the difference between change
in behaviour of the Parker. This is the one which is related with behaviour change. This
contributes an important role of ADHD which have the symptoms which creates hyperactivity
and impulsiveness within patient. This is the one due to which patient is not able to sit constantly
for longer period of time and is not able to get calm. Due to which, this create difficulty for
Parker and leads to change in behaviour (Wagenaar, (2017)). This behaviour occurs due to not
having able to wait for his chance during conversation and interrupt between the conversation of
others. Such type of activity creates a change in behaviour of Parker which leads to to impact the
chronic illness or disability. Parker need to change his behaviour in sufficient way that behaviour
should be normal and compared to other people. Maintaining the behaviour can help Parker to
positive impact the chronic illness or disability. But due to Hyperactivity, Parker is not able to

maintain his behaviour and due to which negative behaviour creates a negative impact on the
chronic illness and disability. This creates worsening of disease and unable to live standard life.
Parker has also a behaviour that he do not care about the prescription which helps provide proper
medication. Parker is not able to focus and paying attention on the prescription which can lead to
create had difficulty. Due to such type of behaviour lead to create high negative impact on
chronic illness or disability. Parker is not paying attention and fail to attend the appointment such
type of behaviour creates a high difficulty for the improvement in Parker’s health (Almustafa,
(2018)). This cause worsening of illness and can lead to create a hard difficulty for living
standard life due to factors like anxiety and depression and attention deficit hyperactivity
disorder. This creates fail to paying attention and focus on any thing. This allows Parker to
pretend like unaware of is medical treatment and due to which is not able to regular attend is
appointment with physiotherapist, dietitian or occupational therapist. This behaviour creates
difficulty to meet with therapist and his occupational therapist and dietician. They are the one
who can help Parker by providing different therapy which can be highly helpful to get better with
his health. Due to change in behaviour or of these reasons or factors creates a difficulty for
Parker to join session of therapy. Such like that, this behaviour is highly able to impact the
chronic illness or disability which can lead to create a worsening of his illness (Jørgensen,
(2021)).

highly appropriate for explaining the behaviour of Parker. This can also be used as a basis of

and improvement in illness. Social cognitive theory refers to a cognitive formulation of social
learning theory. This theory explains human behaviour in different terms like dynamic, three-
way, reciprocal model. There are different environmental influences and personal factors along
with interactional behaviour. This can lead to create a impact on the behaviour. This is an
emotional model which can create to change in behaviour and can be applied during counselling.
This is the intervention which can help to prevent the disease and helpful in managing

therapy to Parker for improved health. This can be highly helpful to provide a basis for the

because through providing motivation can help Parker to change the behaviour in positive aspect
that can help to managed effectively about the behaviour. This can also helpful to arrange
properly and leads to improvement in illness. Motivation plays an important role in providing
care and also to motivate for changing in the behaviour. This can help in changing in the

to play provider who can provide care to the Parker to implement the strategy by motivating him
to changing in the behaviour. This can also help to improvement in health of Parker. Registered
nurse have the responsibility to take care of Parker and provide the medication on proper time for
health improvement (Upshur, (2018)).

There is a need to provide person-centre care as appropriate service to Parker. This is
highly able to assist in appropriately changing the behaviour. Present-centre care is the service
which is highly able to provide care to the patient all the time. There is a nursing staff who can
provide care to the patient within home in all over the day. There is Nurisng professional who
take care of daily activity of patient and provider medication on the time. Nursing professional is
also helpful in providing help to perform daily activity and take care of safety and care (Gottert,
(2021)). Health Care professional is also able to provide guideline to the patient to perform every
activity which should be done properly. Nursing staff is also able to provide safety and security
to the patient and to focus on their activity which should not how many person or to the patient
with the help of person-centred care. There is delivery of high level of care service to the patient
which can highly assistant the patient to perform daily activity effectively for health
improvement. This service should be funded by specific healthcare provider where registered
nurse have the role to play and service provider of person-centred care. With this strategy
registered nurse can provide effective care and helpful in changing in behaviour of the Parker.
This can also help in health improvement of patient in effective manner. Registered nurse play an
important role in delivery of care as a person-centred care to other (Wilson, (2017)). Through
this, it can be highly applicable for providing effective care service which will be recommended
by therapy and health care professionals are having role to focus on daily activity and providing
care and safety from any type of activity. This can harm the patient or any one. Due to patient,
