INTRODUCTION
Learning disability is the difficulty which is associated with many other elements and
creates problems to understand effective results and outcomes (Bert, Giacometti and Siliquini,
2014). Present study is based on health and social care services for individual who are suffer
from learning disability. Further, in this nurse supporting them for reducing their problems so
that they can understand outcomes of the health care program. In this aspect, report assesses
barriers regarding health promotion and facilitation. In addition to this, it includes learning
disability nurse who support to each individual for access different services related to health
promotion.

facilities to enhance their understanding in personal life. In this way, people enable to participate
in society and community functions. In addition to this, these facilities assist to grow effective
operations in care home to enhance participation in achievement of different services. However,
there are many barriers comes in respect to solve issues and outcomes of health and social care in
term of mental health illness (Kokko, Green and Kannas, 2014). In this way, appropriate services
are requires to enhance capacities and understanding of every individual. Following are such
barriers which include in order accessing services:
Financial resources
In order to providing and using different services, there are many issues occur at
workplace which needed to reduce mental health problems. In this aspect, financial resources are
main element which enhances performances of care users (Jakovljevic and Tomic, 2016). Care
home and providers are charge very high to each individual in term of different elements and
resources. Every individual unable to access these equipments for reduce their mental issues.
