Document section: Community Assessment and Needs Analysis.
Student responsible: ___________________________________
Word count: 400 words
This section will be the collaboration of all research and community profile information collated from
research conducted during NURBN1004. As a group, consider the following questions whilst
developing your health promotion program. Think about the ‘Who, Why, Where, How, What &
When’ whilst making your decisions. Remember to also utilise the information from LGA health &
Wellbeing Plans and Priority areas of Local, State or Federal governments.
Question 1:
What evidence do you have from your
community to specify the health issue and
focus population for this program?
(What data tells you there is a problem here?
Who is at risk?)
The obesity is highlighted as the health issue for this
community. The community have three major
problem such as High unemployment rate, High cases
of overweight and obesity. As per the field report2.8
% of population were overweight in the community
and 3.2 % were classed as obese which further
highlighted obesity as the major health issues. The
Cranbourne community have poor dietary routine,
and physical inactivity which further lead to the
obesity. The obesity further facilitate the other
associated diseases such as diabetes type 2 and heart
disease. In the population , individuals aged of 65
years above 65 years are at higher risk of obesity and
should be the focus group of the program (Hocking,
Dear & Cowley,2017)..
Question 2:
What social, environmental and behavioural
determinants of health are contributing to this
health issue? How?
(What is CAUSING this issue? Why?)
The field report on the community suggested that
dietary and lifestyle factors are the major social
determinants of the health. The social determinants
such as physical inactivity, poor diet quality, high rate
of unemployment, are major contributing factor to
the chronic disease such as obesity. Within the
community only few people have high weekly gross
income. As per the field report, the unemployment
rate in the Cranbourne is high compared to the entire
Victoria. As per the field report, in 2016, 3.8 % of the
individuals have high income and 39.7% individuals
are with low income. Socio-Economic Indexes for
Areas (SEIFA) of 2016 ranked 940 in terms of low
income. Consequently, the community have high
smoking rate and involve in the drug related crime.
Question 3:
Are there any existing programs/policies etc.,
already addressing this issue in this
community? If so, what are they and are they
being effective?
(What is already being done? What gaps are
there?)
When the major health issue, obesity was observed
in the community, for promoting the wellbeing of the
population, government had taken the initiatives. The
Australian government with assistance of Australian
Health Policy proposed a nationwide actionable plan
to guide the prevention of chronic disease in major
four direction. However, the major gap in this case
that no focus was given to this community where rate
Document Page
of is extremely high (Brownie, Muggleston & Oliver,
2015). This gap can be resolved by Promotion and
implementation of interventions programmes
designed by state and local government.
Question 4:
What resources, assets or strengths does your
community have?
(What can you use from WITHIN YOUR
COMMUNITY to help support your program?)
One of biggest strengths of the community is that
Physical activities undertaken by the members of this
community are beneficial by helping in weight control.
Cranbourne Multicultural Community Centre opened
for the public and they have indoor skating facility as
well as Indoor Sports Centre for basketball, football
and netball) which would help to reduce the obesity.

Document section: Planning.
Student responsible: _________________________________
Word count: 400 words
This section will demonstrate the considerations you have needed to make when PLANNING your
program and what you want to achieve. It will include the processes you will need to put in place to
achieve your goals, the objectives you wish to address in order to get there and any challenges you
might need to be overcome? What cultural or political considerations might need to be made? Why
you might make the decisions you have in terms of how you wish your program to appear or what
you would like it to do for your target audience?
Question 1:
What is the SMART goal of your program?
This is the broad overall aim (long term changes)
and who is your target population? – relates to
outcome evaluation
(What do you want to achieve? Who do you
want to achieve it?)
The obesity is highlighted as the major
health issue of the community. In the
current context,
To create social changes and individual
behavioural to foster the achievement
and maintenance of the healthy weight
of 80 to 86 kg pound within next 8
months.
Target population is males of the
population aged in between 65 to 85
years.
The prime reason behind it is that
majority of individuals in the affected
group are person aged 65 years and the
life expectancy of the individuals in the
community is 85 years, especially male
(Hocking, Dear & Cowley,2017).
Document Page
Question 2:
What are the objectives that relate to your
goal? How will you realise these? – relates to
impact evaluation
(What changes do you need to make to achieve
the goal?)
The objective is to provide adequate
health education to the target
population along with the other
members of the community to maintain
healthy weight
To assist them in the changing the life
style and diet for maintaining healthy
weight (Hocking, Dear & Cowley,2017).
To provide health and sports facilities
to the population of the community.
To provide the opportunity of
employment and education which is
highlighted as the social determinants
contributed to the development of
obesity.
After 8 months the achievement of
healthy weight would be the evaluation for
the healthy weight.
Question 3:
What are the strategies, or activities related to
the objectives? What things will take place on
the ground? - relates to Process evaluation
(What actions will you take to make the
changes?)
Education can be given to the
target population in the
community hall where
educators as well as health
professionals would provide the
adequate education with
PowerPoint and photos (Eng,
Moy & Bulgiba, 2016).
It would done by informed
permission from them.
Local and statement governing
bodies can be involved in the
program to amend the policies
related to the fast food and
banning of smoking (Lee et al.,
2016).
Governing bodies can be
involved for providing the
opportunities of employment
who are unemployed in the
community (Gruver et al.,
2016).
The individuals in the
population can be encouraged
and engaged in different social
activities to maintain the
healthy weight Rezapour
Mostafavi & Khalkhali, 2016)..
The population can be
encouraged to walk every day
to cover short distances rather
Document Page
than using public transportation
(Rezapour Mostafavi &
Khalkhali, 2016).
The population can be
encouraged to involve
themselves in physical activities
and sports activities (Rezapour
Mostafavi & Khalkhali, 2016)..
The population can be
encouraged to eat healthy
drinks and vegetables as snacks
to maintain healthy weight
The primary health care
facilities can be given to the
patients who are obese (Eng,
Moy & Bulgiba, 2016).
Question 4:
What is your timeline? What challenges might
occur in meeting your targets?
(How long do you need to make things happen?)
The timeline of this goal would be 8 months.
The challenges might be the
ethical dilemma since many
individuals who are obese can
be forced to participate in the
program which will breach the
autonomy (Lee et al., 2016).
Many individual may not be interested
in the health program.
To implement these strategies, a
shortage of fund can be observed.
Miscommunication can occur if the
individuals belong to diverse cultures
(Hofmann, 2016).

Document section: Implementation
Student responsible: __________________________________
Word count: 400 words
This section will demonstrate the considerations you will need to make to implement your program
into your community. It will look at the framework you have based your processes on, the ‘Who,
Where, When and Why’ factors that you need to address when presenting or delivering your
program. You will need to consider the decisions being made as to how you wish to make the best
impact through your program design.
Document Page
Question 1:
What health promotion model or models is the
program going to utilise and why?
(Eg: Health Belief Model, Stages of Change,
Ottawa Charter, etc?)
Ottawa charter would be most suitable health
promotional model for maintaining healthy
weight and prevent obesity. The rationale
behind using this model is that Ottawa charter
consider as a good health promotional model it
is the product of social, economic and personal
environment (Thompson, Watson & Tilford,
2018). To improve the wellbeing, these
dynamics are required to consider. This model
covers five major areas of action to improve the
population health and wellbeing of the
individuals. These action areas include
Building healthy public policy
Creating supportive environments.
Establishment community action.
Developing individual skills (Stock,
Milczarski & Saboga-Nunes, 2016).
Re-orienting health care services
towards prevention of obesity and
promotion of health.
Question 2:
What is the setting/supportive environment
where your program will take place? Is this the
most appropriate given your target population?
Why?
(What venue/location will you use? How will it
be accessed? What facilities are required, etc?)
The community hall would appropriate
setting for obese patients (Hofmann,
2016). The environment would be
appropriate for health promotion
because in the community hall all
members of health promotions can be
gathered together for educating each
individual. Moreover, presence of every
member of community in the
community hall will make population
more comfortable and encourage to
participate in the program.
The venue would accessed by
communicating with community hall
members and local government (Alami
et al., 2016).
The health care facilities, nutritional
facilities and exercise facilities are
required for the health promotion.
Question 3:
What individual health education (or personal
skills) need to be considered? What knowledge
deficits do you need to address?
(What education is needed or gap in knowledge
recognised? For Whom? Why?)
The participants of the health promotion
would require active verbal and nonverbal
communication and interpersonal skills. These
skills are demanding skills for the volunteers
who are going to participate in the health
education for providing education to the
population (Eng, Moy & Bulgiba, 2016).
An adequate knowledge of obesity and
maintaining healthy weight would be
provided ,which is required to reduce the
Document Page
obesity (Eng, Moy & Bulgiba, 2016).
. The gap in the knowledge is that majority of
the obese individuals in the population are
unaware of the chronic diseases which were
developed because of the obesity. The
population are also unaware of the benefit of
healthy eating and physical activity.
Question 4:
What other stakeholders need to be involved in
your program? And why?
(Who else could be involved? How might they
contribute?)
The involved stakeholder would be
Local government, state level
government and policy makers to
implement policies and provide
opportunity of the health care facilities,
employment and sport activities.The
health care professionals who would
provide the support to the participants
to provide health care services Eng,
Moy & Bulgiba, 2016).

The participants who would participate
in the health promotion program for
prevention of the obesity and
maintaining healthy (Edwards et al.,
2016).
The community member would be
involved to support the individuals who
are obese and encourage them to
maintain healthy weight.

Document section: Evaluation
Student responsible: _______________________________
Word count: 400 words
This section will demonstrate your thoughts around the appropriate way to evaluate the
effectiveness of your program. Evaluation of your program must be considered throughout the
development, not just at the end to evaluate if it has been successful. Think about how you might
continuously reflect on the processes being under taken in order to gather information and make
your decisions. Consider the challenges you have to face to measure the impact of your program.
Think about the strategies you have chosen in order to reach your goal, are they appropriate? How
do you know? What changes may have been necessary along the way?
Document Page
Question 1:
Outcome Evaluation
How will you know that you have reached your
SMART goal?
(What tells you your intended outcome has been
achieved?)
The main parameter in this case are the
individual behavioural and social changes
prevention is to create through foster the
achievement and maintenance of the healthy
weight of 196 to 200 pound within next 8
months. The outcome evolution can be done in
this case. The outcome evaluation can be done
by analysing the measured result of the
program for determining how well the
program performed (Edwards et al., 2016). In
this case the results from medical examination
of individuals should analysed to evaluate the
outcome.
Question 2:
Impact Evaluation
How will you know when you have reached your
objectives?
(What is telling you that changes have
occurred?)
To evaluate the objective, Population based
survey can be conducted to evaluate the
whether the objective was achieved or not. The
positive result of the survey along with the
average weight of the individuals can provide
the idea whether the goal was achieved on not
(Berezin & Lamont, 2016). The participants of
the survey would be the individuals who were
overweight or obese. Their beliefs, values and
attitude would reflect the changes that have
occurred which further help to gain the
emotional data. Besides survey , the field
reports , and government websites can be used
to understand the impact of the evaluation.
Question 3:
Process Evaluation
How will you assess that the strategies or
activities used have been effective?
(How will you know if what you are DOING in
your program is effective?)
The process evaluation would be done through
statistical survey of the obese individuals,
collecting data of obesity and health conditions
interviewing about their food habits (Berezin &
Lamont, 2016). The stake holders can
interviewed and along with participants to gain
the understanding of the patient satisfaction
(Hocking, Dear & Cowley,2017)..
. The feedback from the participants who are
attending the program would be effective to
gain the patient satisfaction. If high patient
satisfaction as well as desirable weight were
achieved for population then it can used as
parameter for the success of the promotional
plan (Berezin & Lamont, 2016).
Question 4:
Tools
What formal/informal tools will you use to
evaluate your program in terms of success or
the effect on your target population?
(What resources will provide data or
information regarding your program’s success?)
The audit would an effective tool to evaluate
the program in terms of success or the effect on
your target population. Audit helps with
establishing good-quality improvement systems.
This process would help to identify the major
cause or problem in the process (Christensen et
al., 2016). After identification, it would also help
to design the strategy to mitigate the problem.
Document Page
The audit data also provide the data regarding
whom to involve in the process of mitigation.
Therefore, it is an effective method of the
evaluating the health promotion program and
reassessing the strategies to achieve desired
outcome.
