This report presents a comprehensive system analysis and design for the My Health Record project. It begins with a stakeholder analysis, identifying and describing various stakeholders such as hospital owners, board of directors, medical directors, system administrators, patients, government bodies, doctors, nurses, and staff. The report then includes a set of questionnaires designed to gather feedback from patients regarding the new health record system. A use case diagram illustrates different use cases, including deleting users, updating software, monitoring data processing, allowing registration, registering users, file transfer, patient health records, and the login process. Each use case is further described, detailing the actors, descriptions, and pre- and post-conditions. A fully developed use case, focusing on chat functionality, outlines the scenario, event triggers, and expected outcomes. The report concludes with a bibliography of cited sources.
