INTRODUCTION
Heath and social care are provisions, given to needed person like old young and
children people to make them able to fight with their health disabilities and diseases. Present
report will talk about Jonathan Tertullien, who is facing elderly problems like multi disease,
memory loss, physical disability and social boycott. Report will also articulate his unmet need
which put him in such condition. Report will also give suggestion about social and health care
should be given to him along with rationality behind these services. Lastly, his non-
achievable need also cover in report.
1. Unmet need in case study
As per the given case study there are so many unmet need are present which create
problem for Jonathan Tertullien in his weak days. He does not have support of family in to
render him physical and physiological support. Also, he does not have his own house which
put burden of rent. The only social support is received by nearby pub also close down (Challis
and Clarkson, P., 2017). It seems that Jonathan Tertullien not blessed with caring neighbour
after one who is now hospitalised.
There is lack of health infrastructure which make him depend to other for his basic
needs. Jonathan already have some serious health issue which demand proper care and
observation, here also he is not getting social and personal care. There is no healthy
environment present at his home because of his incapability (Rose, 2018). An old age person
require Nutritious fresh food, Mobility arrangements, healthy life style and periodic medical
check up etc.
2. Type of heath care and social care service
Nursing care-
Jonathan Tertullien needs a fulltime nurse for his medical condition because he is not
able to perform any task by himself. Nurse will take care his personal, medical, food and basic
assistance care. She will look upon medical checkup, nutrition requirement and therapy
necessary for stroke and vascular dementia (Åhlin and et.al., 2015). She will also give him
medicine on time and Companionship too. Nursing care comes under prime health care
service.

Generally, it is also called “ old age home”. It will help Jonathan Tertullien because he
is all alone at home and isolate from social contact. He is already paying the rent of house,
which can be transfer to fee of old age. These elderly home not only render staying facility to
people even they arrange some complimentary service for them. For instance NHS also design
socila health care program in which household gadgets are modified to easy elderly life.
3. How these services can help Jonathan Tertullien
Nursing care suitable for Jonathan Tertullien because he need observation and
personal care, which is not possible in any other health care service. Disease like vascular
dementia, heart condition and weak memory can be only cure by regular medication,
exercise, and therapy. These things can be easily done by nurse. As he is lacking in
psychological needs, that can be solve when they become friend (Almazan-Isla and et.al.,
2017).
Jonathan Tertullien is not living in his own home, he can move easily from this home
to old age home. And, this very necessary as well because old age home not only assist old
people to live their live normally, but they also give a holistic care to them. Nursing care and
old age home care both given in one time. This combination will give him best care. He does
not have friend and family with him, and in social gap can be minimize as he will receive
companionship of other people living there.
4. Non achievable needs of Jonathan Tertullien
Jonathan Tertullien can receive health and social care but nobody can fulfill need of
family in life. Old age people want to be taken care by his family members, with whom they
want to spend their last time. Apart from it, he does not have friends too, with whom he shares
his moments. His love and belonging need can be remain unmatched. Also, he can not
perform outdoor activities as earlier because medical assistance infrastructure also required
support of other to operate (Boulware and et.al., 2016).
He is suffering from multiple disease at same time, which can lower his motivation to
live life with interest. His depression and loneliness cannot be cure and he will not cop-up
with his miserable health condition as medicine only works when patient is happy. Old- adults
often face lack of social care due to death of spouse, insensitive society and retirement. Same
happened with this person, which is not achieved by any health and social care.

CONCLUSION
From the above case study it can be conclude that old people faces major health issue
became lack of personal and social care. And, this cannot be cure until society remain
unaffected by these issue. It is required to induce human sensitivity among people so that
these people will not face such boycott. Also, a person should invest in old age program at
young age.
