How well did you achieve the practice criteria for this
unit?
With the help of the practice criteria of this specific unit, I
can say that I have clear and in-depth understanding
regarding the law which is a set of binding rules and meant
to enforce justice. With this, I am also able to understand
the basic principles of policy by which a government is
guided. Moreover, I have gained detailed understanding
about the ethical practices as well which is kind of moral
principle that govern person's behaviour in health and
social care profession.
What challenges did you face? In order to evaluate the challenges that is being faced by
me, I have realized that there are certain jargon that are
being utilized in the health care profession. As I was not
very much well-aware about each and every terminology,
hence I faced certain issues in order to grasp knowledge
regarding some specific content. Thus, I faced a lot of
challenges in order to manage my time as I have to learn
certain concepts in later terms while referring them at
home through internet.
Are there any specific examples from your placement
that evidence your learning/development, e.g.
adhering to policies, procedures and practice?
In order to adhere towards ethical practices, I had once
adopt certain ethical approach as I stayed late in order to
cover one of my friend cum colleague's shift in another
department for free of cost and also provide my services in
context of give patient treatments to fulfil my moral duty.
Another example which I have remember from my
experiences is that one there was a patient came to me
who requires critical treatment, while at that point of time
before starting treatment, the consent form had been
signed by their parents from my side in order to get their
appropriate consent towards the treatment.
What changes or improvements do you think could be
implemented into practice as a result of completing
this element of the unit?
The elements that are related to practice, function and
effective leadership along with appropriate culture that
promotes changes. In order to complete the elements of

the unit changes such as shared vision, creating a culture
that promotes readiness of change and communicate
changes into most effectual manner will be highly
entertained.
How can you action plan to develop these skills
and/or qualities further/ help to improve provision in
the healthcare sector?
In order to build or develop team working abilities, I will
clearly identify my roles with positive mindset and then
exercise things in collaborative manner. Moreover, I will
also develop good communication abilities by becoming a
good listener. Both the skills will extremely provide
assistance to me to improve my career into health and
social care sector. I will try my level best to generate such
abilities within 6 months of time duration.

How well did you achieve the practice criteria for this unit? Principles of support is highly valuable in productive
health and social care settings. The professional
principles and values are efficient enough to provide
support in order to make sure about each individual
that the people are being cared in most proper
manner. With the help of this unit, now I am able to
gather appropriate information, communicating them
with empathy, embed modern technology and being
able to develop appropriate skills and knowledge that
will help me in generating my professional skills and
values into health and social care system.
What challenges did you face? While getting knowledge regarding the unit i.e.
professional principles and values into health and
social care practices, I have learnt that values such as
respect for persons, compassion, commitment to
integrity and ethical practices, etc. are some most
significant type of requirements. However, I had faced
some challenges such as there are certain things
through which earlier I was not very much well aware
and hence in between the learning sessions,
sometimes I felt low motivated and face difficulty in
order to concentrate over the topics that are going to
teach by my tutor.
Are there any specific examples from your placement that
evidence your learning/development, e.g. Interpersonal
skills, dealing with specific issues requiring ethical
consideration?
I had faced once difficulty in order to convince one of
my patient to take specific medication for their own
betterment as they have anxiety related issues and
challenges. But that patient was very much rigid and
do not want to take medicines. Thus, I have forward
that patient to senior doctor for further consultation
as that patient was in high danger and requires
immediate approach of any doctor. As I was unable to
convince him, I refer the case to some other doctor
which shows fulfilment of my moral duty by providing
them efficient services through providing elicit and
understanding patient's concern in most considerable
ways.
What skills and/or qualities do you consider you need to
develop as a result of completing this element of the unit?
Skills related to commitment to professional
competence, patient confidentiality, interpersonal and
communication abilities, professionalism in context of
humanism and cultural competence, generate self-
confidence, etc. needs to be developed in order to get

competence in this specific unit.
How can you action plan to develop these skills and/or
qualities further/ help to improve provision in the
healthcare sector?
From the detailed evaluation, I have realized that
there are certain skills that needs to be integrated or
enhanced within myself such as professionalism for
which I will take necessary actions such as observing
seniors, attending various conferences, participating in
workshops, reading around specific relevant and
related subjects and other similar kind of activities that
promote my abilities effectually. Moreover, for
cultural competence, I will get training over cultural
awareness topic.

integrated health and social care, I am able to
understand the process related to protecting
clients, patients as well as colleagues from
any kind of harm in health and social care
settings. Moreover, Now I am appropriately
able to get benefits by following a person
centred approach with users along with ways
to handle ethical dilemmas and conflicts that
may arise while providing support, care and
protection to users.
What challenges did you face? From the detailed evaluation, I have realized
that in theoretical manner, the things are
different but at the time of practically
implementing the things I faced difficulty in
implementing the policies, legislations,
regulations and code of practice that are
relevant to health and social care along with
in order to identify their impacts as well over
the company policy and practice.
Are there any specific examples from your placement that evidence
your learning/development, e.g. duty of care, demonstrating safe
practice, identifying and reporting unsafe practice, providing
leadership in a team, empowering an individual to communicate
their changing care needs?
An example which I have remember is that
there was a patient who was very much aged
that he was not even able to walk or sit
properly came to me for medication. While
providing them appropriate services, I felt
that it was my duty of care to provide them
complete safety and high quality services
along with giving them respectful treatment
into most effectual manner. I informed him
each and every time about their medication
along with the reason as well as also ask for
his consent while doing check-ups shows my
duty of care towards the patient.
What skills and/or qualities have you developed as a result of
completing this element of the unit?
Skills that are highly required by any
professional while dealing with their patients
are empathy, good communication abilities,
critical thinking skills, active listening power,
self care, patience, etc.
How can you action plan to develop these skills and/or qualities
further/ help to improve provision in the healthcare sector?
I will prepare a list of all my current abilities
along with mention in them about my
weaknesses and requires further
improvements. Moreover, I will ask my
seniors, tutors and peers to give me feedback
regarding my drawbacks. Furthermore, I will
take online practice sessions that enables me
to enhance by certain amount of abilities

How well did you achieve the practice criteria for this
unit?
With the help of this practice, I am able to take systematic
approach in order to examine diversified range of evidences to
answer key questions related to relevant sector. Through this, I
am now able to understand the purpose and process of research
into health and social care specifically in relation to promoting

integrated approaches to care.
What challenges did you face? Evidence based practice helps the health care professionals to
improve health care quality and also assist future nurses to
minimize the gaps that create challenges. But at the time of
getting information regarding the same, I faced certain challenges
such as lack of time, resources, difficulty in assessing research and
certain lack of perception of the benefits of applying evidence
based practices into clinical or practical ways.
Are there any specific examples from your
placement that evidence your learning/development,
e.g. raising awareness of appropriate approaches to
take in certain situations e.g. sensory impairment,
English as a second language; ensuring everyone is
involved in certain activities or that provision is made
to facilitate any additional needs or preferences?
I had helped one of my patient who was having a difficulty of
deaf-blindness which is impaired difficulty of both hearing and
sight loss and provides several serious impacts over the person in
their day to day life. Then at the time of assist them, I make sure
that the individual want to communicate with me or highly
attentive before trying to communicate with him.
What skills and/or qualities have you developed as a
result of completing this element of the unit?
There are certain significant skills that are highly required in order
to develop evidence based practices such as critical thinking
ability, relational skills, integrative capacity and scientific
mindedness. Moreover, I require abilities to reflect over practices
and critical appraisal skills are also highly required to get in-depth
and detailed evaluation of the things grabbed by me.
How can you action plan to develop these skills
and/or qualities further/ help to improve provision in
the healthcare sector?
In order to enhance my scientific mindedness up to considerable
level, I will observe, sort, compare, predict, experiment, evaluate
and apply essential things in order to generate scientific thinking
within myself. Furthermore, in order to enhance my critical
thinking abilities, I will prepare a list that will focus towards my
core requirements, deal with biases, consider all kind of
consequences, do research appropriately and try my level best to
do not over complicate things will help to generate my critical
thinking ability along with enable me to serve patients into most
effective manner.
