Lin could have asked all the required details that were necessary to fill the client information intake form.
Fill up of this form would have helped her to understand Tony's case in a better manner and she could
have helped Tony in a better way. She could have asked for emergency details from Tony so that she
could have contacted him in case of emergency. She could have also provided him with toll free service
who could have helped him at night.

1. One of the safety issue for Tony is related to his anger i.e. in anger he can harm himself or
committee suicide.
2. Another safety issue related to Tony is is medical condition i.e. without pain killers his pain
can get worsen.

1. For the first issue I will contact a counsellor so that he can talk to him in a better way.

All the information required were not recorded in a proper manner and most of the information were
missing. He didn't talk about this wife and his kinds in a detailed manner and did not provide with the
reason due to which she left him.

I will ask the counselor to talk to him and talk about these issues in a more clear manner. Ask about the

reason due to which she left him with their kids, reason because of which he is facing financial issues
and ways to resolve those issues.

Dear Rachel,
I am writing this email to you in order to brief you about Tony's case and the issues
he is facing currently. He has nowhere to stay and is sleeping in his car. He is an alcoholic
and drinks a lot when he is in anger and because of this he cannot stay in men's shelter. Three
years age he meet with an accident at building site and after that he had constant back pain
complain and for this he used to take Endone but now he is out of Endone and as a result he is
