1. Introduction
Wellness is an intricate term and is much wider in its approach or implications.
However, wellness is not just a mere state of physical well being but covers much broader
aspects of health such as – social wellness, emotional, physical, spiritual, environmental,
intellectual, personal health and disease prevention. In this paper we have conducted a
comprehensive health assessment based upon lifestyle approaches and health habits of an
individual. The assessment helps in finding out individual areas of weaknesses or loopholes
associated with overall maintenance of health. Thus, undertaking the assessment may help in
deciding upon the long term and short term goals, which may contribute towards the
improvement in health of an individual. Further an action plan has been provided where
SMART goals for coping up with the health shortcomings have been provided. The motto for
undertaking a personal health assessment is to assess current health status and find out future
precautions needed to fulfill all dimensions of wellness
2. Discussion of the results of the assessment
The assessment tool divides wellness into seven dimensions of health which are –
physical, social, emotional, environmental, spiritual, intellectual, personal health promotion.
The summary of the test results has been provided below. The scores have been arrived at
out of a total score of 20.

Analysis:
With respect to the social, emotional and environmental health a score of 17 out of 20
have been achieved, which is quite a high and outstanding score. The high scores in a
personal health assessment tool depicts that one is aware of the importance of these
parameters on the overall wellness of an individual. It also shows that one is concerned about
implying positive health behaviour in daily life, which reduces overall health risks. Though
the scores received for physical health, intellectual and personal health promotion is high
there is scope for improvement. For personal health promotion engaging in strenuous
physical exercises once in a while can do more bad to my health than good by resulting in
injuries.
On the contrary, engaging in light sports or activities on a day –to- day can help in
becoming more active and healthy and avoid any chances of personal injury. With respect to

the intellectual health parameter, I think I am really poor in time management which can
affect my growth career wise as well as develop a negative social image about myself. I think
I am often careless in doing the necessary adjustments as well as take time out to consult a
physician in case I am suffering from poor health.
I think I am low on the spiritual dimensions of wellness. I think I need to spend more
time focussing upon my personal strengths and weaknesses. On identification I should make
a smart goal to overcome the weaknesses I have found in my personal retrospections.
Additionally, possessing less faith in the almighty along with a lack of zeal in living my life
to the fullest can considerably lower the positive energy of mind and develop additional
amount of stress within an individual.
Influence of certain events on present health status
I think the culture and values with which an individual is brought up has a profound
impact on the overall wellness of an individual. Additionally, the lifestyle patterns we pick up
in the due course of growing up also influences upon our health considerably. In the personal
assessment tool I have achieved the lowest score with respect to spiritual health.
The lack of enthusiasm in living life to the fullest can hamper the positive energy of
the mind and affect my day-to-day performance. This is because I often inhibit myself from
living life in a wishful manner and rather focus upon the consequences. Committing mistakes
open more avenues for learning and spending longer time between decision makings and
performing a certain action often destroys the willingness to participate in a certain action by
an individual. This is because most of the times our decision making as well as the spiritual
aspects of an individual are guided by peer opinions. For example, the child of catholic
parents is supposed to grow up imbibing the spirits or the values taught by the bible.
Similarly the inevitable events through life often shape up the aspects of spiritual and

emotional connectedness within people. I think growing up in a family with less spiritual
inclination; I base my judgements more upon reality than spiritual faiths or beliefs.
3. Conclusion
Therefore, based upon my self assessment I think that I have much scope for
improvement with respect to a number of wellness dimensions. This is with respect to
physical, intellectual, spiritual and personal health. I think in order to cope up with my
personal weaknesses I can start writing a habit journal where I can chart my progress towards
reaching my personal goal or behaviour. For the purpose of which a SMART goal could be
designed by me, which will help in achieving positive changes in health behaviour
accordingly.

Action plan
From the assessment tool I have found out that I need to work upon my spiritual
health. For the purpose of which a SMART goal could be planned by me alternately. Here,
the SMART refers to Specific, Measurable, Attainable, Realistic and Time-bound, which has
been further presented in a table below.
