INTRODUCTION
Meeting the needs of service users is a concept in which an organisation work for social
work and nursing attempts to fulfil requirements of their patients or individuals which requires
help care. The main aim of this project report is to assess various care needs and formulate
desirable goals in order to provide a rationale for systems and structures. In this essay, a specific
case scenario is provided in which an assessment is required to be conducted of Mrs X in order
to identify patient's care requirements. Mrs X is suffering from a bio medical condition of
psychosocial due to her loneliness. In this essay, a reflection report is also prepared which has
personal assessment process experience. In order to identify care needs, several methods and
structures are analysed in this report which results in effectiveness during the process of
assessment. Views of manager of National health care services is also analysed in order to
produce an understanding about the care plans and goals (Büyüközkan and Çifçi, 2012).
MAIN BODY
Question 1. Structures and models that are available to support an assessment of patient
Health care assessment refers to the process of identifying specific needs of a person
which should be addressed or fulfilled in order to provide them health care services. According
to the given case scenario, Mrs X is a 72 years old widow lady who lives alone and her family
lives quite far away. She manages her house all alone with a considerable help of neighbours
with garden. Due to her old age and loneliness, she has feelings isolation which follows to her
stroke. There are various problems faced by Mrs X and some of them are she can not manage to
get bath safely as she does not have a well fitted shower. Another issue is that due to her old age,
she has mobility problems due to which shopping is tough task to be performed by her as she can
not drive. Mrs X also finds difficult to manage her property which is two steps far from her
house. In order to conduct an assessment for Mrs X, it is important to explore various structures
and models of assessment (Care Quality Commission, 2012).
Health care organisation which is assessing care needs for Mrs. X is require to follow a
specific health care structure that includes step by step plan to be referred. First section or step of
this structure must be related with Review prescription. Mrs. X is a 72 years old lady due to
which it is intensely important to fulfil her medical needs. Medical prescription must be
reviewed by care worker so that all her medical needs can be ascertained. Next stage of this

health care structure is Reviewing personal issues. Being a widow and away from family raises
issues of isolation and depression due to which it is important for care worker to identify and
review all her personal issues. This can help in analysing medical health of Mrs. X along with
her physical health. Third section of this structure is Data collection. In this section, care worker
gathers all the information about Mrs X. from her neighbours, previous doctors and from her
prescriptions in order to identify measures which can be taken to facilitate her and to meet all her
service needs. Another section of this structure is Planning management, in this section care
worker will prepare and manage a plan for all the considerable steps which are necessary to be
taken to fulfil Mrs X.'s service requirements. Next section in this healthcare structure is
Communicating information. Under this section, care worker will communicate all the data
which is collected to manager of healthcare organisation. Last section of this structure is related
with Data interpretation. In this section, manager and care worker will analyse and interpret all
the information gathered about Mrs. X and will evaluate necessary service needs. Medical as
well as personal aspects of Mrs X. will also be evaluated in order to serve reliable needs which
are necessary to her. The above sections of health care structure are required to be followed by
care worker of Mrs. X and by manager of health care organisation (Kavanaugh and et. al., 2013).
There are various models which helps in supporting an assessment of Mrs. X. These
models provide an overview which classifies assessment in to various classes. Models of health
care which can be considered to assess Mrs X are quality model, OPT model and Roper, Logan
and Tierney model. The first model which is considered is Quality model. This model was
introduced for theoretical reference in which care worker can analyse quality dimension. This
model consists of few quality indicators such as assessment questionnaires, inspection
procedures etc. Questionnaires and these procedures helps in simulation of data analyses.
Questions which are asked by neighbours and other relatives are usually based on her mental and
physical health. Inspection procedures which are followed by care worker are a kind of metrics
which are used for technical documentation. Under this model, holistic approach should be used
by which every part of Mrs X's body can be examined in order to identify any medical issue.
Another model of health care assessment is OPT model which is based on evidences of clinical
reasoning. In this method, care worker ensures that all the information about patient which is
collected should be analysed and evaluated according to the educational practices of nursing. In
this model, various aspects and issues related to patient are identified and presented using a

chart. This model includes all types of issues including medical and non-medical. In the case of
Mrs. X, care worker has to focus on non-medical issues more as there is no information about
problems regarding her health that are identified in the case scenario (Lippincott, 2012).
Another method which is considered for assessment of Mrs. X is Roper, Logan and
Tierney model. This model is considered as most effective for patients which are in serious need
of personal attention. Under this method, isolation feeling of Mrs. X can be handled. This model
is practice-centred approach which combines educational nursing, practices and experience. This
model was introduced with the aim of facilitating those patients which are in the serious need of
personal attention in order to gain affection and comfort. In case of Mrs. X, she is widow and has
a sense of isolation due to which care worker should focus on fulfilling her basic requirements
like shopping and other stuff along with which she should given few hours of interaction with
care workers (Roper, Logan and Tierney model, 2018).
Roper, Logan and Tierney model is considered as most effective model by analysing the
condition of Mrs. X. According to this model, factors are identified which influences living of
Mrs. X. Along with these factors such as maintaining safe environment, communication and
others, activities of living and dependence continuum are also evaluated. This model can help
care worker to assess all these elements and then prepare a worksheet which has information
about patient. Manager and other staff of health care organisation can evaluate all the factors and
then can identify measures which should be taken (Great Britain, 2011).
Above structure and models of assessment are considered as suitable for the condition of
Mrs. X as all these models involves different dimensions by which both mental and physical
health of patient will be analysed. Using these dimensions various solutions for Mrs. X can be
determined.
Question 2. Reflection on own current skills which would be effective during assessment process
As a carer or care worker, there are several needs which are required to be possessed at
the time of assessment process. I am a well experienced carer and for the last two years, I am
working with a National Health Care service Organisation. Manager of my organisation asked
me to assess the case of Mrs X. In order to effectively assess her case as I possess various skills
which can help me and Mrs. X. The first and most important skill which I possess is Supportive
and encouraging nature. This skill of mine can help in gaining information from the patient.
Another skill which can be possessed by me in assessment procedure is Understanding. Mrs. X

is a 72 years old lady which lives alone in her house, it is important to understand her personal
issues as the main core of her problems is her sense of isolate feeling. Being in an old age, it is
important to communication and be surrounded by people who can listen and understand all the
issues. In order to effectively assess the condition of Mrs. X, it is important to communicate with
her and listen her all problems.
It was identified by me that Mrs. X was suffering from a Psychosocial condition of
loneliness as she was a widow and lives alone in her house. Solution to this issue is identified by
me that bio Psychosocial approach should be followed. In this approach, an understanding of
physical as well as mental health is gained.
Sensitiveness is one my skills which is important in this case, being sensitive makes the
process of understanding so easy. This skill can help me in gaining trust of Mrs X and her
surrounded people. This patient is a widow and is in desperate need to talk about her
experiences, being sensitive allows me the flexibility to listen. Another skill which I possess as a
carer is Dependable, Mrs. X is a vulnerable case due to which she can depend on me when it
comes to her personal issues. From the case brief of Mrs X, it has been seen that she can not
drive due to which shopping for general utilities is a tough task for her. My dependable skills can
help her for shopping. My another skill is Professionalism. As a carer, it is important to act as a
professional care taker otherwise patients can take you for granted. Keeping the approach of
professional carer is important as it helps in maintaining balance between work and relaxation.
My main aim at the time of assessment is to gather as much information as I can for which my
Good communication skills has helped me a lot. Interpersonal skills or communication skills is
an essential part for working in care. This skill enables me to work effectively. Good
communication skills also includes good listening skills due to which Mrs. X can convey her
problems to me and can even listen my views about her issues (Nash, 2013).
The above skills can help me to achieve the target of effective assessment of the case
study of Mrs. X. Besides this skills, as a carer, I play a major role in the implementation of care
to meet the needs of service. I believe that the most suitable model or theory which should be
used in the case of Mrs. X is Roper, Logan and Tierney model as it is the combination of
considering physical and personal issues. The major role which I can play in the assessment of
Mrs X is communication. No other person can know better about some individual then itself.
Using this phenomenon, I considered that it is important to meet from Mrs. X to determine her

issues rather than to communicate with anyone else. Besides communication, reviewing is
another role which I can play in the assessment of Mrs. X. This case is about a 72 years old lady
thus it is important to review medical condition of Mrs X as it consider that old age bringing

Another significant role which I can play in this case is provide assistance in data
accumulation which is required to be collected to further analysed and interpreted by managers

depression. A part time carer must be appointed to her who can fulfil her requirements of a
companion. Roper, Logan and Tierney model is based on identifying factors affecting health of
Mrs X by considering both medical and non medical aspects. By following this model, I can help
Mrs X in handling her issues such as isolation, taking bath, maintaining her property and shop
for general utilities. Above skills and roles can help me and any other carer to effectively plan
and assessment process of health care (Smith and Firth, 2011).
It has ascertained by me that Mrs X could be suffering from psychosocial trauma due to
which she requires immediate psychosocial consulting.
Question 3. Views of manager and advise to care worker
Views
Mrs X is a 72 years old widow. She is leaving alone in her house and is feeling lonely.
Due to this loneliness, she is facing numerous physical and emotional difficulties on daily basis.
Due to poor shower facility she is not able to take bath safely. She owns an additional property
which is two steps far from her house which is difficult to be maintained by her. However, she is
able to cook and maintain her house but going for the shopping, it is difficult for her as she can
not drive and none of the family member live with her. Few of her neighbours help her in
maintaining her garden.
From the manager's point of view, to solve the problem of isolation, she needs a care
taker who comes to her on daily basis for 4 to 5 hour per day with whom she can talk, share her
thoughts an in order to make her feel bit relaxed. As she goes to lunch club every week to meet
up with her friends so to help her in isolation, she can call her friends at her home and she can
gossip with them. It will help her to be happy and will reduce the feelings of isolation also
(Subashini and Kavitha, 2011).

To solve her bathing problem as she does not have fitted shower in bathroom so a shower
will fit in her bathroom so she can take shower easily. And she will not find the difficulty in
taking bath. The another thing she can do is that, a bath tub will be set up in bathroom, where she
can take bath easily and she will feel the relax also.
To solve the problem of managing property that she has two steps away from her house,
she can let out that property with the help of property agents. So the property will be taken care
by those tenants and it will also generate the extra income for Mrs. X.
However, she is able to cook and maintain her house but she is not able to go for
shopping as nowadays everything is available at one's door step. So she can use the online
platform for the anything that she wants to buy. With the help of online, she does not need to go
anywhere, she can order from her home only.
As if she does not want to take help from her neighbours for the gardening then she can
hire the full time gardener. It will help her in the gardening as well as gardener to help her in the
cleaning of her house and maintaining the house properly. As a manager it has suggested that
Mrs X should be empowered and socially connected with her friends and family to resolve her
sense of loneliness (Weissman and Meier, 2011.).
Advise
Care workers or carers assess the position and condition of patient and gather information
about them. In this case scenario, a carer has assessed the situation of Mrs. X and collected data
about her personal and medical aspects. Information collected by the care worker is evaluated by
manager and there are few revisions and additions which are identified by manager. These
revisions and additions include steps of care plan as carer should also include measures which
can solve her issue of isolation in a deeper prospective. Carer has suggested that a part time care
worker must be appointed for her which can communicate with her and help her with everyday
issues. But as a manager, it has been identified that the problem of isolation can be effectively
tackled if someone from her family can manage some time and spend few hours with her at least
once in a week.
Another addition which has identified by the manager is approach of social media. In
order to handle the problem of isolation, care worker can introduce social media platforms to
Mrs. X. By the means of social media, patient can spend her free time communicating with
people. Goal which is re- formulated by the manager is appointing a full time care taker for Mrs.

X. Patient is capable to manage cleaning of her house and cooking due to which there is no need
of full time carer. A part time care taker can effectively help the patient from her all the issues of
isolation and house hold utilities.
Another revision which is discussed by manager is renting her additional property to
some tenant. As given in case scenario, Mrs. X is a 72 years old lady due to which it is hard for
her to manage her additional property which is two steps far from her house but renting that
property to some tenant is not a reliable decision. Leasing or renting a property can raise major
issues of formalities. In order to resolve this issue, part time care taker can also manage
maintenance of her additional property (Weissman, 2011).
CONCLUSION
From the above essay, it can be concluded that needs of service users can be meet by
various methods and procedures. In this essay, case of Mrs. X is analysed and evaluated by care
worker and manager of health care organisation. Situation of patient is assessed by interpreting
the information which is collected by patient and her surrounded individuals. Issues which are
identified such as isolation, bathing problem and maintaining the additional property are
allocated few solutions. Solutions which are refereed by care workers are further analysed by
managers in order to provide additions and revisions. By conducted above essay, it can be said
that issues of Mrs X can be resolved by appointing a part time carer, installing a shower in her
bathroom and delivering online home utilities to her.
