This essay is going to discuss about the care planning in the healthcare of the patients.
Care planning in health is a form of communication between the patient and the health
personnel that acts as a means of communication. It also promotes high standards of care and
helps in reflecting the nursing process. A care plan is informed via the assessment tools,
clients or patients that are the core integral part of the entire process. The care planning is a
collaborative process that needs involvement of every health personnel (Coulter et al. 2015).
There are certain components of the healthcare planning. It includes the 7 C’s of care
planning that consists of clarity in the thoughts of patients and nurses, better communication
among the nurses and the patients, change in the way of managing the patients, criteria for the
analysis and diagnosis of the patient, preparation of checklist for the patients’ needs and
requirements, call on help and choosing the right provider (Care Management Software for
Care Homes 2019). The care plan must include problems of the clients and should be
engaging. It should be shared with that of the client which will help the patients to advocate
about their own health issues. This way care planning ensures client engagement (Barello et
al. 2014). The goals of the care plan must be linked to that of the client’s problem. It should
be achievable and the care plan must also ensure that the patient’s views are included in it
even if the client does not agree with the care plan. The care plan must include the proper
evaluation plans that should be assessed and reviewed from time to time (Koh et al. 2013).
Therefore, from the above essay it is concluded that the care plan can be summarized
that it should be as creative as possible and should be simple. The patient must be able to
relate to the care plan and thus, it should be personal to every patient. The care plans should
be assessed at certain time period, should address the risk and must be made along with the
patient so as to include their viewpoint as well.
