The rampant growth of chronic diseases has led to a mass malfunctioning of the
traditional model of care setting due to the rising numbers of patients that need sensitive care and
sophisticated machines. Population health approach is a very efficient method that would reduce
the situations that the traditional system has been able to overcome and it’s making its states care
of patients such John become difficult to handle.
According to Egger (2013), he argues that a population health approach is a form of
integrating the health sector where it can be easy to analyze population health concerns and the
predicaments that the population needs to be addressed. Public approach to the health sector will
help in solving the community problems where the community can have participation to
determine the agendas that need solutions; this thus helps to serve and care about the health of
the whole community and individual. Through the public or population health approach, the
community is guaranteed equitable service to health every one thus this kind of health set can be
very good to John being able to access health services without problems.
The devolved health sector will contribute a lot of good changes in the community health
development. Macro level of devolution will impact changes in better buildings of hospitals that
can be able to serve the community effectively, better machines and ambulances that can be able
to curb any major problems that may arise.Also, funding from government and non-
governmental organizations improved policies and programs that give the patients such John
privileges to good health services and affordable. At this stage, the health system will be able to
have GP’s that can serve the community and benefit patients such as John. With the availability
of specialized PR actioners such diabetes educators who could help John in managing his

Keleher, 2012).
Population health approach impaction to the organizational coordination and operation
(meso) of the health service can be very effective since all the management can be well
monitored at the community level. The PR actioners in the community or local areas can be able
to have good coordination together with the community and thus this also enhances relationship
to patients such as John and his GP; He can be able to get much better answers about his illness
through good understanding from his GP (Young, 2005). The population health approach at the
micro level it’s an advantageous platform to both the health workers where they have exclusive
state of health of the community, common illnesses and most chronic illnesses that do occur in
the areas. Through this, the doctors can be able to have public forums that be able to help people
in the community like John on ways to live healthier lifestyles and also warn them on upcoming
emergencies.

In summary, through population health approach Johns managing problem will
efficiently be eliminated through the availability of good medical care to his poor health.
Through good access to better and health and civic education to the public about their health will
reflect a much healthier society and good medical states for John (Murray, 2002).
