AI Summary
This report analyzes various healthcare delivery models, including nurse-managed clinics, accountable care institutions, and health home models. It compares and contrasts these models, highlighting their strengths and weaknesses, particularly in the context of serving vulnerable populations and older patients. The report also reflects on the nurse's role within these models and in public health programs, discussing how academic work supports evidence-based practice, research processes, and patient outcomes. The assignment fulfills the requirements of a reflection assignment in the WCU RN-BSN program, specifically addressing Essential III by demonstrating an understanding of research, evidence application, and collaboration in healthcare settings. The student reflects on their personal perceptions, academic work, and the impact of their proficiency on their future practice.

Introduction
In the current world today, the population in the world is high and that have given rise to demand
of healthcare services. The quality of healthcare that one is entitled to is directly related to the
cost. In a way to reduce the cost of care, there is the introduction of various healthcare models to
satisfy the clients. The goal of this assignment is to compare and contrast various healthcare
models and give out my perception on the same.
i. Health clinics managed by nurses
These are clinics organized and run by the nurses. These health clinics are always
available among the vulnerable communities. The nurses that lead these clinics are
skilled and experienced according to Cordes, Reiter & Manson, (2018). These nurses
define how the services should be delivered in the clinics the services offered in these
clinics include education of patients who suffer from chronic illnesses and fitness
education. My perception on this type of model is that its main aim is to reduce the
cost of treatment and improve the patient results in these vulnerable societies. The
model is an essential way of ensuring that the slow developing communities can also
get access to quality healthcare services.
ii. The accountable care institution model.
The accountable care model involves the union specialists, primary care clinicians,
healthcare professions and hospitals. These group of people and institutions come
together to ensure that patients get quality care at a low cost. The nurses collaborate
in this model to ensure that patient receive the quality services. For instance, one
hospital can take part in doing a screening to the patient while the same patient head

to another institution for surgery and medical services. My perception on this
collaboration is to provide quality services at an affordable cost.
iii. The health home model.
Medical homes aims at providing medical care to patients at home. These patients
may be older than sixty years of age and cannot access the physical healthcare
facilities. Some of the services provided at home include the emergency care and
educative lessons on how to manage various illnesses. My perception to this type of
model is that it aims at ensuring the satisfaction of older patients. The mobile clinics
are also affordable and quality according to Jagannathan, & Udaykumar, (2020).
Comparison of the models.
The home system mainly aims at serving older patients who cannot reach to the physical
buildings far away from their premises. Besides that, it provide nurses with an opportunity to pay
close attention to the older patients according to Scott & Scott, (2017). However, when it comes
to the collaboration model, it aims at ensuring that patients get the same services at an affordable
cost. Different institutions can share resources to ensure they meet the needs of the patients. The
nurse managed institutions are like private hospitals which try to reach out to the vulnerable
communities for healthcare services.
On the other hand, nurses play a bigger role in the programs. Mohammed, Savage &
Zimmermann, (2017) said that they educate the patients and advise them on the best healthcare
services they should go for incase of illness.
The program essentials.

In meeting the outlined outcomes, the course work elaborate the standard required by evidence
based practice. The best way in achieving these outcomes is to follow the standards set and any
other update from the registered nurses.
