Introduction
The Armadale is situated in the South-east region of Perth and it is 28 kms away from
Perth. The region is surrounded by City of Gosnells and the City of Kalamunda and are also
surrounded by Serpentine Jarrahdale and Wandering, Shires of York and Beverley and City of
Cockburn (Thorpe, & Sweeney, 2019). The population of the Armadale region is increasing day
by day and as per the census data of 2016, the total population of the region was about 76,602.
The female ratio of the region is higher compared to that of male and there are preschool
children, school children, adults as well as elderly people who stayed in the Armadale.
The city of Armadale region was increasing its population and the health services
available in those regions were not able to meet the increasing demand of the population. The
people who are living in the Armadale region are mostly poor and were of labor force. The
others are full time workers and part time workers and there are also some unemployed people
who lived in the region.
The needs of providing the people with health care services because the people in that
region were mostly suffering from chronic diseases, diabetics, obesity and mental illness. The
people living over there needed to be encouraged with healthy habits of living and was to
improve the diabetic condition of the people (Littlefield, 2016). The government of Australia has
decided to provide health care services to them to improve their health care conditions. .
Service Delivery Gaps
As Armadale region is in the suburb region of Perth in Australia, the community of
people living there kept far away from all the modern health care services that were available to
the citizens of the main town. As with the rise in population in the region, the number of diseases

were also increasing with no health services provided to them (Callander, Larkins, &
Corscadden, 2017). The people over there were mostly suffering from chronic diseases,
diabetics, mental illness as well as obesity in the region. There were 21 general practice in the
whole region of Armadale and there were 3 psychology and 5 dietetics to provide them with
health service. The service provided to them was very low and they were suffering from disease
more frequently. The government was not able to provide until they had decided to provide them
health service by establishing health care service to the people of Armadale region. The people
lived there were mostly of labor and were also full time workers. So, the government thought of
mitigating the lack of health service in 10 PHN regions of Australia.
Health Care Home
The Health care home that is being developed in this project is to be developed in the in
the Armadale region of Australia. Most of the people of Armadale region suffers from
cardiovascular diseases, diabetes, mental illness as well as obesity and they do not have proper
health care treatments in those regions (Coombs, 2018). The people of Armadale region have
limited number of health practitioners and with the rise in population of the region there are 0.26
general physicians available for 1000 number of people. The number of psychologists as well as
dietetics are also very low in the area in Armadale region.
To improve this issues, the government of Australia has decided to organize Health care
program in the PHN regions of Australia where Armadale region is one of the region where
program of health care is to be developed (Finch et al., 2019). The people over the Armadale
region would able to take health care services from the health professionals who would be
working the centers. The main aim of the Health care homes is to encourage a proper and health

way of living the life and also would help to decrease the risks health that the people in those
regions suffers from. The health care homes decided to be built by the Australia governments
would help ti improve the health condition of the people who are suffering from Type 2 diabetics
and also will help them to deal with other chronic disease that are common with the people of
Armadale.
There are many adequate number of mental health patients who live in that region. The
government has also decided to help those mental ill health people by providing them service for
mental health so that they can improve their status of mental state in the community (Kheradyar,
& Couch, 2019). For taking service from those centers, people enrolment was to be done till 30
of June 2019 and they would get service from the health care program till 30 June 2021. More
than 10,000 patients in the PHN region were registered for the program and is to get health care
service from the health care homes to be developed in Armadale region.
Objectives
The objective of this report is to develop a Health Care home center in the Armadale
region of Australia (Sakai et al., 2018). This would help to get coordinated as well as include
innovative way to increase patient care center to improve the chronic as well as complex
condition of the patients. The project objective states to complete the project within time and
within the budget that is schedule for the project. The implementation of this project will also
help to educate the people of Armadale region and get them to know about the health care
centres that are being build up in the nearby regions to enhance their health care systems. The
project will also recruit employees for the centres of Armadale health care system so that the
front desk, the diagnosis, the treatment process and the personal health professionals are assigned

to help the people in that region (Lyle et al., 2018). The objective of this project is to make the
life of the people easier in terms of proper healthcare systems.
Specific 1. This project will help to improve the coordination as well as
communication between the health care professionals as well as the
patients.
2. This project also will organize health care programs to improve
healthy habits of the people.
3. Ways to improve to promote the eating habits are stated in the
report.
Measurable The project is measurable because after the implementation of the
project, the people of Armadale region will get medical care
increased by 50% compared to the present situation.
Achievable The goal of the project is achievable because the government has
already decided to build the health care centre and the receptionist at
the front desk of the centre will have the patientID saved in the
database of the system after their enrolment being completed.
Realistic This project is realistic because this project will develop a heal care
centre in the Armadale region and will include health care programs
about how to take service from the health care centres hiring the
health care employees for front desk recruitment, health workers to
share care plans and many more.

Time bound The objective of the project is time bound because this project will
be delivered within 158 days from the day of project initiation. The
project is expected to be delivered within the time frame that is
schedule for the project.
Activities
Activity Milestone
Getting project approval
This is the milestone in the project planning process of the project
schedule. This project activity will complete all the meeting
process of the process and after being completed this milestone
will be check listed.
Determine planning Roadmap
In the planning phase there is sub activity known as planning
phase that would include milestone of creating a roadmap about
how to proceed with the planning process and recruit the
developing team for carrying out the planning process.
Making project finalization
In the project charter developing phase, there project finalization
takes place. This is the last phase project planning phase and after
all the activities are done, this milestone is check listed.
Initiate the Health care home
development
In the initiation phase, the health care home center is initiated to
start the development process of the building the health care

system.
Install all Equipment
In the project execution phase, which includes the building
process of the health care centers. This milestone checks that all
the processes are carried out properly to build the center in
Armadale.
Completing the development
of health care
All the checks are done related to the development of the health
care center and it is seen that the color the building is being
complete with all the fitting done in the center.
Project hand over
In the extra work phase, the project is handed over after being
completion of the center development.
Sign off the project
After the training program and recruiting program being
completed the project sign off activity is carried out where the
sponsors and the project managers will sign off the project.
Closing project
Closing project takes place after the documentation of the project
being completed. Project closing is done after all the payments
are done.
Organize health care
programs
The project workers also has to organize the health care programs
so that the people would get to know about the center that is
being built up in that region.
Educate the people They also need to educate the people about how to take medical
care from those centers and how they can be connected with the

Hire employees for the health
care center
Many employee will be needs to carry out the activities in the
health care centers and for those hiring of employees are needed.
Timeline
The timeline shown below states the different activities that are to be carried out for
developing the health care system. There are different health care programs as well that are to be
carried out for educating the people in the Armadale region. There are five different phases that
are included in the development process and those are to be carried out in the schedule time.
There are also different milestones shown in the chart indicating different activities that must be
carried out. The resources and the cost of the project are also schedules in the project schedule.
