INTRODUCTION
Law, Ethics and Policies are three different terms where they have a similar meaning i.e.,
rules and regulation which should be followed by each and every individual. Legislative Body do
constitute law for number of department and it is important for all those sectors to work as per
the guidelines which are given under the law. In the field of healthcare and social care, it do have
huge importance because single mistake can take the life of innocent person. In context of the
file, there are number of department which need to be considered accomplishing the goals which
have been given. Three different tables will be prepared and they are observation log, activity log
and reflective statement which will help to understand the file in detail. Also, their will be the
discussion about legislation, policies and standards and why they are required to be considered in
any of the health care centre.
PART A
Produce an observation log that includes noted on the practitioner's application of relevant
legislation, policies and professional standards.
It is necessary for NHS to know what are the primary roles and responsibility towards
patient because that have huge importance for the betterment of healthcare centre and patient too.
It is said that delegation of work should be in systematic manner where right candidates should
be selected for delivering the services to the patient (Ventola, 2014). Whenever working within
the healthcare it it is necessarily to be focus that all the information sharing system are well
experienced and have the knowledge that what should be be shared when because it helps to
bring better outcome. Also, it is to noted that every work should be done by following the laws
and regulation. Below, observation log is prepared which is based on healthcare practitioner.
Observation Logs
Rationale – The main objective behind preparing observation log is to know the role and
responsibilities of practitioner in healthcare centre.
Explain the individual’s needs: For me, it is necessary that I should be able to have
conversation and communication with practitioner through which I can easily know that what
can be the additional requirement in any of the NHS.
Describe briefly the details of the setting: It is necessary for me to know about the location

Describe the context of the setting: Everything should be planned so that output of the that
particular things can be more accurate and for that I must try that how I can distribute my work
load into number of weeks.
Scenario: I need to prepare an observation log where I am connected to the field of
medical. I need to observe that what are the essential things which need to be focused by
practitioner whenever they are in working hour. The practitioner under whom I have been
assigned to observe is dealing with one of the patient who is dealing with the problem of lungs
and because of that reason he had been admitted in the hospital since last 3 weeks. So, I will
observe that how practitioner is dealing with the situation and what can be the challenges which
are being faced by the partitioner while handling the case.

Week 1: In starting week, I had observed that how practitioner need to treat the patient
and what are the essential guidelines which they are required to consider and in practical life do
they follow or not. In reality, it was one of the harder worker which need to be observed because
decision taken by most of the practitioner should be fast. While observing I came to know that
policies upon NHS are well defined and because of that practitioner was treating each and every
patient equally but I came to know that it was not easy task for them because there are some of
the patient who need special care and some of them don't but still practitioner was forced to do
so.
Week 2: When I entered into second week, I though to cross check the legislation,
policies and standards that are they being followed or not and if not why. And I came to know
that practitioner have to face lots of problem because most of the decision which are being taken
by practician do no match with laws. In case of the patient who is suffering from the problem og
lungs, I found that law says that while dealing with this type of case approval must be obtained
from the family member but there was no one his family who can take the responsibility so
doctors had to take decision of own.
In addition, I found that practitioner need to follow the instruction of NHS and that is one
of the reason that practitioner are unable to give better treatment to patient. There are number of
facilities which are lacking within the premisses of hospital and the facilities which are available

provide best services to every patient so that they can become healthy and the main thing which
need to be focused between all this is that healthcare centre must take the reasonable amount of
money for the treatment (Martinez and et. al., 2015). The another one is state obligation which
ensures that approach of healthcare is well framed were they should only think that how they can
provide the better health treatment to the patient. The law also ensures that whether any of the
NHS discriminate the patient or not and if find any of the discrimination then they do take legal
action against them. Some of the additional law which are required to be considered in NHS is
