INTRODUCTION
Law, Ethics and Policies are three different terms where they have a similar meaning i.e.,
rules and regulation which should be followed by each and every individual. Legislative Body do
constitute law for number of department and it is important for all those sectors to work as per
the guidelines which are given under the law. In the field of healthcare and social care, it do have
huge importance because single mistake can take the life of innocent person. In context of the
file, there are number of department which need to be considered accomplishing the goals which
have been given. Three different tables will be prepared and they are observation log, activity log
and reflective statement which will help to understand the file in detail. Also, their will be the
discussion about legislation, policies and standards and why they are required to be considered in
any of the health care centre.
PART A
Produce an observation log that includes noted on the practitioner's application of relevant
legislation, policies and professional standards.
It is necessary for NHS to know what are the primary roles and responsibility towards
patient because that have huge importance for the betterment of healthcare centre and patient too.
It is said that delegation of work should be in systematic manner where right candidates should
be selected for delivering the services to the patient (Ventola, 2014). Whenever working within
the healthcare it it is necessarily to be focus that all the information sharing system are well
experienced and have the knowledge that what should be be shared when because it helps to
bring better outcome. Also, it is to noted that every work should be done by following the laws
and regulation. Below, observation log is prepared which is based on healthcare practitioner.
Observation Logs
Rationale – The main objective behind preparing observation log is to know the role and
responsibilities of practitioner in healthcare centre.
Explain the individual’s needs: For me, it is necessary that I should be able to have
conversation and communication with practitioner through which I can easily know that what
can be the additional requirement in any of the NHS.
Describe briefly the details of the setting: It is necessary for me to know about the location

healthcare setup and how it is being helpful for them.
Describe the context of the setting: Everything should be planned so that output of the that
particular things can be more accurate and for that I must try that how I can distribute my work
load into number of weeks.
Scenario: I need to prepare an observation log where I am connected to the field of
medical. I need to observe that what are the essential things which need to be focused by
practitioner whenever they are in working hour. The practitioner under whom I have been
assigned to observe is dealing with one of the patient who is dealing with the problem of lungs
and because of that reason he had been admitted in the hospital since last 3 weeks. So, I will
observe that how practitioner is dealing with the situation and what can be the challenges which
are being faced by the partitioner while handling the case.
Observation log write up
Week 1: In starting week, I had observed that how practitioner need to treat the patient
and what are the essential guidelines which they are required to consider and in practical life do
they follow or not. In reality, it was one of the harder worker which need to be observed because
decision taken by most of the practitioner should be fast. While observing I came to know that
policies upon NHS are well defined and because of that practitioner was treating each and every
patient equally but I came to know that it was not easy task for them because there are some of
the patient who need special care and some of them don't but still practitioner was forced to do
so.
Week 2: When I entered into second week, I though to cross check the legislation,
policies and standards that are they being followed or not and if not why. And I came to know
that practitioner have to face lots of problem because most of the decision which are being taken
by practician do no match with laws. In case of the patient who is suffering from the problem og
lungs, I found that law says that while dealing with this type of case approval must be obtained
from the family member but there was no one his family who can take the responsibility so
doctors had to take decision of own.
In addition, I found that practitioner need to follow the instruction of NHS and that is one
of the reason that practitioner are unable to give better treatment to patient. There are number of
facilities which are lacking within the premisses of hospital and the facilities which are available

within the hospitals are expensive which cannot be afforded by the patient (Martine and et.al.,
2015).
PART B
Prepare the activity log which reflects the necessary requirement within the NHS and also
include different legislation with regards to the health and safe practice.
It is said that everyone have the right to be healthy and for that human rights act has been
enforced. There are number of laws and policies which has been enforced to ensure that health
safety is being provided to every patient. There is WHO Constitution 1946, it is necessary to
provide best services to every patient so that they can become healthy and the main thing which
need to be focused between all this is that healthcare centre must take the reasonable amount of
money for the treatment (Martinez and et. al., 2015). The another one is state obligation which
ensures that approach of healthcare is well framed were they should only think that how they can
provide the better health treatment to the patient. The law also ensures that whether any of the
NHS discriminate the patient or not and if find any of the discrimination then they do take legal
action against them. Some of the additional law which are required to be considered in NHS is
Health and Social Care Act 2012, Equality Act 2010 ((Key legislation, 2019)) To know all about
this in detail activity log was required to be framed so that better outcomes can be obtained.

Week 1: In first week, my target was to
enhance the knowledge in the field of law and
policies which are related to healthcare centre.
Even I am required to know what are the major
roles and responsibility of HSC sector of the
NHS. Also, I have gone through some of the
practical exposure related to the rights of
human in the field of HSC for example: patient
most be treated first if they are required the
help of any practitioner.
For all this task, I decided that I must perform
my duties and task under the circumstances of
laws and policies for example: fair treatment
act. Under this act I was ready to perform my
task i.e., trying to treat the people in effective
manner. The main motive which was behind
this law is just to reduce the number of death
which had happened during the time period of
2005-2008.
Week 2: In second week, when I was working The main focus in this week was that human

which are related with keeping the data of
patient who visits the NHS trust. Even though I
was doing my work, I had focused on my work
plan that it must be done as per the human
rights act where superior priority must be
given to the work.

spend my most of the practitioner who are
working within the health care centre. They
were able to define roles and responsibility that
has to be consider while maintaining the log
book of those patient who were being treated
in NHS trust.
Week 3: When it comes to the last week, I was
required to so something new related to the
protection of health of a patient. In addition.
Managing the campaign has been continued so
that treatment of patient can be done in
effective manner. Also, promotion of health
and social care sector helps to improve the
level of standard of healthcare sector. It even
helps to improve the treatment facility within
NHS trust.
While working in this week, my main focus
was to treat patient by understanding my role
and responsibilities so that trust and belief can
be build among the patient as that is required
the most in any of the patient. All of roles and
responsibilities were defined under equality act
through which working condition becomes
more easy. Even this act tough me that every
patient must be treated equally (Tomkins and
et. al., 2015).
Number of laws and policies were undertake in each week while preparing activity log in
defined manner. The laws has been applied in different situation which occurs in front of the
practitioner at the time of training period. This has helped me to enhance my knowledge in HSC
field which can be beneficial ion future.

Develop a series of reflection on your practice and that of other by taking the help of logs which
have been prepared in part A and part B.
Reflection Note
How well did you achieve the practice criteria for this unit: While talking about the
understand I can say that above logs had been prepared with the helps of appropriate planning ,
decision making ability so that targets can be accomplished. In context of healthcare, law and
acts have clearly indicated me that I must know what should be the roles and responsibilities

and even it have clearly indicated me that treatment should be fair and equal. This laws have
helped me lot that how I need to use different tools so that better treatment can be done.
What challenges did you face: When I joined NHS I found that management system do
create lots of problem sometime so it is necessary me to know that what can be major problem
that can be faced. I found that management system doesn't allow to work freely because of that
it was never easy task to deal with any of the patient. Even delegation of work was also lacking
due to which I had no idea that what are the roles and responsibilities that need to be done by
me.
Specific examples from your placement that evidence your learning/development: I do
have detail knowledge about health and social care sector due to which I thinks it can be easy
task for me to deal in this particular field. Even I have the knowledge about of laws and
policies which are required to be considered within healthcare which can be helpful to sustain
for longer period of time in NHS sector. My learning skill need to be at it best in practitioner
field and I think that I can be able to do this through which I can easily treat the patient who
requires the help of healthcare partitioner.
What changes or improvements do you think could be implemented into practice: It is one
of the most important field so I think it is necessary that practitioner should be allowed to take
some of the decision of own which can help to bring more effective result (Rock and Degeling,
2015). Even some of the improvement can be done in the field of management and policies for
providing better services.
How can you action plan to develop these skills or qualities: If this skills and qualities are
required to changed then it is necessary that there should not be any kind of communication
gap. Better training and development process need to be conducted.

CONCLUSION
This file can be concluded by saying that law, policies and ethics are the three terms
which must be consider in the field of healthcare for the purpose of effective result. There are
different laws and regulation that need to be considered while dealing with any of the patient.
Different types of laws and policies have been enforced so that death ratio can be reduced while
treating the patient. Also, there are lots of changes which need to be done in this field so that
better outcomes can be achieved in future.
