Adverse event related to open discourse was identified when caring heart patient. This
person as admitted in hospital due to the reason of heart attack. After all necessary check up
identified that two veins have issues where one is totally blocked and second one blocked more
than half. Blockage of the half vein is not provided to patient as professionals thinking of
removal from the medication only. Information is provided regarding one vein which was totally
blocked. This was operated by the doctors for the removal of blockage. After passing of few
time, patient again faced the issue of chest pain. Now, check ups declared that the vein which
was blocked half is near to the stage of full blockage. This seems as one of the adverse event of
open disclosure because in future this also can impacts the life of patient due to to the fault of
medical professionals. If all information is provided to patient then it could be possible that same
was operated too at that time and patients gets the total relief. Also, if not operated then the fault
of medical professionals would not be counted in future (Collin-Vézina and et. al., 2015).

As Enrolled Nurse the actions will be adopted in the above defined situation when
information is not provided by doctor includes;
 Providing proper information to patient about the issues faced and identified under check
ups.
 Consult with higher authorities to provide all necessary information.
 Also provide information about all alternative measures along with their impact.

 Providence of all accurate information to patient about the diagnosed issues.
 Information about all alternatives along with their influences over health.
Nurses
 Provide proper care to the patient and taking care of all medical needs.

The main purpose of open disclosure is to provide all necessary information to patients
and allows to take effective decision regarding their health. This will also having the aim that
person perfectly knows about conditions which can be arise in future due to presence of some
issue. The concept of open disclosure brings an obligation over medical professionals that
disclose all the information to patients about their health irrespective to its nature and impact
over mental health. The main role played by policy of open disclosure is to safeguard the
profession of health from any kind of fraud (Heimstädt and Dobusch, 2018).
