After Amanda was diagnosed with hypertension. As her caregiver, the first thing I did
was inquire about her current condition to which she reported feeling better than when she was
being admitted to the emergency room 24 hours ago. Then I asked her to narrate what she was
doing before she fainted. Amanda reported she was selling vegetables at her grocery. She had
spent the whole morning working without a break. The patient reported feeling out of breath for
almost three weeks and dismissed the feeling as exhaustion.
I asked her what she understood from her diagnosis (hypertension). She said she had no
idea. Her sister, who had accompanied said she was familiar with the term but not aware of what
constituted the condition. Therefore, I explained the condition to the both of them covering facts
on diagnosis, causative/risk factors, and treatment which included lifestyle modifications.
I then asked the patient to narrate her understanding of hypertension. She was very
attentive and had understood most of the information. The both of them understood the need for
lifestyle modifications and adherence to treatment. She understood the need for exercise, healthy
eating, and sleep. She promised to avoid fatty and salty diets and focus more on fruits,
vegetables, and whole grains.

This week I handled a 65-year old patient presenting with hypertension. I was supposed
to check for the recent information on drugs for hypertension. My clinical setting has a resource
center for all conditions treated in the hospital. Upon identification of the disease, a provider logs
in and confirms the medication being used. However, every clinician has to assess which drug is
most appropriate for the patient. The hospital protocol mandates the verification to be determined
by sex, age, race/ethnicity other comorbidities, allergies, side effects, and patient preferences.
After identification, the medications are sent to the pharmacist for review before administration.
The available support for the patient is a family member; a daughter. The daughter
brought the patient to the hospital and is responsible for her overall healthcare. Also, the patient
is listed as a critical care patient; indicating she can access the critical care unit of the hospital
any day and time. As for the resources, the patient seemed well informed about her health. On
every visit, she is given a brochure on hypertension as an education platform. The patient and
caregiver report that the pamphlet has been a useful resource in the management of the
hypertensive condition.

I have never treated a patient directly, but I have been involved in the treatment of a 70-
year old patient with a neck fracture, hypertension, and osteoporosis. The patient had received
surgery to restore the neck fracture four days before the meetup. Her medical records showed
elevated blood pressure, and she was in pain. The patient needed both restorative and
rehabilitation care.

was given an anesthetic to manage the pain. Then she was made aware that her wound was
healing slower than expected and told that a nurse would be allocated to attend to wound
cleaning in the rehabilitative clinic. The nurse engaged the patient throughout the process of
creating the rehabilitation plan including the most suitable medication and exercise for her
condition. I helped her fill the how to help me hear from and that of the hearing aid and attached
both forms to the recommended rehabilitative clinic plan of care.
In the rehabilitation center, she wound undergo strength and gait training to enhance
recovery from the fracture and osteoporosis. For hypertension, she will be attending education
classes on dieting and exercise with other patients in the clinic. She would also receive assistance
on self-care and keeping her hearing aids clean.
