1. Project Approach
The project approach that would be taken for the successful accomplishment of the
installation project for the custom application is discussed as follows:
1. Project Kickoff and Setup – In this project kickoff stage, the idea over the
development of the project would be done. After the ideas for the project have been set, the setup
strategies for the project would be done. The project manager would have be responsible for
setting up of new ideas based on developing the best form of strategies for establishing the best
outcomes for the project.
2. Performing screening over the present state – The existing state of the project
would be screened. The ways in which the present systems are running would be assessed and a
report plan would be prepared accordingly. This would help in understanding the areas in which
certain improvements could be made and thus be supported for improving the existing state of
the project.
3. Designing of Future State and Implementation – After the process of screening over
the existing project state have been done, the implementation of new developed strategies for the
future state would be done accordingly. The budgetary assumptions should be planned and a
proper form of strategy should be developed. The custom application should also be compliant
with the newly set requirements and hence it should be ensured that all assumptions made for the
project are being accomplished.
4. Implementation of Future State and Securing Sustainability – The future state of
the application would be screened and securing of the sustainability of the project would be

considered. The implementation of the standalone software would need to be performed by the
IT manager within the institution. Based on the successful implementation of the custom
application, it would help towards determining of the situations of risk population.
2. Project Processes
The different processes that could be applied for the project are discussed as follows:
1. Installation of application to master server – In the initiation phase the deliverables
would be set for determining the increase in troubled patient volumes. During this phase, the
admin would perform installation of the application during non-busy hours after consideration of
the downtime of the application.
2. Measurement of effectiveness after the implementation of application – This
deliverable would be focused on the determination of the increase in the number of patient
volumes affected by cancer. In the following stages, the extraction of data, pre and post
implementation would be measured.
3. Surveying over the reliability and licensing – In this stage, further reliability of the
product would be measured. In the further stages, the enhancement of the reliability of the
system would be involved. The receptionist of the system would be contacted based on ensuring
any kind of changes and effectiveness.
4. Increase in the healthcare quality – In this phase, the measurement would be done
based on the increased quality of healthcare. The effectiveness of the application would be done
measured on the satisfaction of the work of practitioner. Further, surveying over the health
practitioners would be done based on measuring the betterment of healthcare.

5. Increase of patient awareness strategies – Increased awareness would be sought
based on determining the satisfaction rate of patients. A survey based on community outreach
would be made for ensuring that the satisfaction of the customers is done. Under this stage, the
outreach to patients would be ensured after developing contact with them.
3. Change Control Plan
Change Proposal E-mail results should be generated after the detection of critical
ailments for the patients
Review After the implementation of change, the risks, schedule, reviews
over the allocated resources would be done.
Reasons for Change The implementation of the proposed change would help patients
and doctors to keep a track over the medical history of the
patient.
Implementation of Change The implementation of the change would be done based on the
ideas developed by the IT Director. Moreover, the change would
be approved by the project sponsor
Result After the implementation of the change processes, the results
would be assessed by the IT Director and Chief Medical Officer.
Based on their approval, the outcome for the project would be
sought.
