Project Assumptions
There is a need to bring in change in the hospital discharge
planning so that the chances of readmission of the patients are
eliminated through complete recovery. The process for
deciding the best care for the patients after discharge from the
setting would be smooth if the basics of discharge planning
are maintained. These relate to evaluation, discussion,
