1. Problem
NewAccess is the mental health intervention which targets the depression and anxiety
of people. In the current system, young people with the mental illness are seeing various
professionals before they are getting help they require. Each time they are visiting the
professionals, they require telling their story. Each time the patient clam up and they say
little than before story. Therefore, it is difficult for the professionals to help them in this
case. Following are the problems which are identified in the current business operations
of NewAccess such as:
I. When the patient re-tells their story, then it takes lot of times and the process
becomes long. Due to long duration, the start time of the treatment is also
delayed. The situation becomes worse.
II. As each time, the young adult tells the story to the professionals, and then the
details become little. The professionals get little details of their mental
problems.
III. If one of the patients provides little details of their problems, then wrong
treatment is started to them.
In this case, the system analyst proposed such a system which helps the organization to
capture the story first time it is being told by the patient to the professionals. The proposed
system is patient record system.

proposed system such as:
a) The system handles to record the clinical functions and patient’s story. It
records the medical information. The recording system can identify and
manage the patient’s records at the first time they visited the first
professionals.
b) The system can manage the problem lists and medication lists.
c) It can also manage the clinical documents and notes such that it helps the next
professionals to easily understand the case history of the patients (Lindblad et
al., 2017).
d) The system can record the story in audio form or written form as per the
organization’s functions. The audio will then transfer to the word so that it will
be helpful. All the story records should be stored in central repository with the
patient’s data.
e) Next time, when the patient will visit the next professionals, then the
physicians should update the new records which will review in this case.
Update of the new records will help next professionals to become familiar
with the changes if any in the treatment.
f) In this case, when the information or story of patient is shared with others,
then it is mandatory to take approval from the patients (Feo&Kitson,, 2016).
Without their approval, it is not possible to share the confidential and personal
data of the patients with other professionals. At the time of appointment, the
information is shared with the consultants so that they can understand the
