Healthy Setting Development 2
Introduction
According to World Health Organization (WHO) "healthy setting is a place or social context in
which people engage in daily activities in which environmental, organizational, and personal
factors interact to affect health and wellbeing" (WHO, 2019).
The healthy Setting is a systematic approach to promote and enhance health, by including
holistic and multi-disciplinary approaches that integrate risk factors during applying the actions.
The Major aim of the healthy setting approach is maximizing health development by preventive
disease via a "whole system" approach. The healthy setting approach is a basically WHO
concept for the health maximization of people, which has its roots or introduced in the Ottawa
Charter' which is a health promotion strategy of the World health organization (WHO).
The key principles of the healthy setting are the following (WHO, 2019). According to definitions
and descriptions by the World Health Organisation, a health setting approach can we developed
everywhere including, family, workplaces, school, locality or society, hospital, prison, market,
villages, etc. In simple terms, a healthy setting includes development of health measures and
prevention of unhealthy issues and disease around the individual or group of people to enhance
health standards and to promote health conditions of people.
This study covers a healthy setting approach, principles and objectives of healthy settings with
various applied theories. Various policy formations regarding the health setting prospective also
has been discussed in the study. This study also covers opportunities and challenges before
WHO in applying the healthy setting approach in Hospitals.

Health Promoting Health Services
The concept of health-promoting in healthcare or hospitals and health services unites (HPH) is
older than 30 years in the United Kingdom with 548 private hospitals and around 600 private
and public healthcare clinics and health services that are present members of the World Health
Organization and the whole system covers by NHS. (The guardian, 2018).
The major aim of the hospitals and health services (HPH), reorient the health services solely
cantering on curative care with more holistic health care that covers the health promotion of the
people. Thus, the NHS and local medical unite and care service providers need to developed
prevention with recognizing and Woking out on main causes of low and poor health and provide
targeted services to people with the given priority to people who are most at risk. It is also
important that health promotion helps to people empowerment to obtain healthier choices and
social care needs.
The whole concept of a healthy setting and health promotion is followed by the principles
Ottawa Charter. The Ottawa Charter covers Health Promotion Action divided into five areas. For
the further development of the concept "The Budapest Declaration on Health Promoting
Hospitals 1991” and “the Vienna Recommendations” have a significant contribution. Apart from
these documentations, the World Health Organization's that are management Policy, Patient
Information and Intervention, Patient Assessment, and healthy workplaces promotion
Cooperation and Continuity developed an outline towards healthy setting development in
healthcare or HPH. The key features of healthy organizational restructuring and infrastructure
changes, the whole health system with corporate social/environmental responsibility and
sustainability.

Healthy settings approach and methods
This chapter includes setting approaches for health development and promotion and theories in
the healthcare sector or hospitals. This health setting approach is a WHO's attributes towards
wellbeing and healthy development of people throughout the world, through its network like
Health Promoting Hospitals, Healthy Schools networks and other health promotion unites that
are mentions in the Ottawa Charter. It can be defined as a place or a social context in which
people spend most of the time and engage in daily activities that may be nearby surrounding,
workplaces, school hospital or any organization. In the health setting environment and
organization has physical boundaries and a certain range to define roles (Dooris, Joining up
settings for health: a valuable investment for strategic partnerships? 2004).
Greening setting approach
In the twenty-first century, the era of technological fast lifestyle, covered with industrialization,
immense use of CO2 and other gases, industrial waste, use of fertilizers and chemicals and air
pollutants are effecting to environment adversely that is leading climate changes. In this
concern, the relationship with people and the environment is getting strong by the growing
realization towards environment changes, thus towards the environment protection greening
setting approach is a new emerging trend for the last two decades (Barnes & Phipatanakul,
2013).
In today's modern scenario more organizations are using greening settings by establishing
sustainability and health orienting settings including hospitals and health services. To provide
customers oriented quality services, the healthcare sector widely experiencing climate affecting
causes like emissions of hazardous gases, hospitals' hazardous wastes and chemicals, huge
energy consumption, water consumption, and transportation issues (Chen & Murthy, 2019).
Poland and Doories suggest some sustainable solutions towards greening setting in hospital
settings such as wind, water harvesting, and water treatment plants for reuse of water, water-
saving tools in toilets and bathroom in hospitals, solar and sustainable energy source, use of

energy-efficient appliances, resource conversation through paper recycling, &electronic filing,
improving public transit links for sustainable transportation and prioritization for organic food
production methods (Poland & Doori, 2010).
Theories Implementation
To overall health development in the world, health setting strategy had been introduced by the
WHO In 1980. The 'Ottawa Charter' had more clearly defined to the strategy and brought out
the strategy on the actual working platform by developing healthy polices, creating and
developing a health-supportive environment, providing Strengthening to community health
actions and developing personal skills towards health promotions.
The health setting enhancement and promotion is a systematic approach that allows people to
improve their health and increase control over health affecting factors in the surrounding or
place where people live like a family, home, locality, playgrounds, workplaces, market,
education institutions or any place where pale spend their most time. This healthy setting
approach enables people to make healthy choices to improve health and promote their
wellbeing. Healthy settings in the environment often cover rules and regulations, policies and
producers related to the particular environment to make mandatory for the people who live in
the environment (Lawrence & Rootman, 2000). The 'Ottawa Charter' was an important
document to improve and promote and establishing a holistic and multifaceted approach to
development and personalized to Healthy Settings programs, and integration of sustainable
development and health promotion in the world (Dooris, 2009).
Sustainable healthy setting development in hospital
Hospitals are the centre platform to provide health facilities to people to reduce illness and keep
them healthy. Hence, it is a key concern and crucial hospital administrators to apply healthy
settings in the hospitals but quite difficult to maintain a healthy setting in the hospital.

Sustainable theory helps to provide an outline to work towards healthy settings in the hospital.
Sustainability gives an outline to implement green practices by given on patient-centric facilities
in the hospitals.so it is important to apple sustainable healthy settings in the hospital (Dooris,
2009).

The sustainable healthy settings in the hospitals majorly includes eight major elements that are
promoting innovative model of care in the hospitals, increasing community resilience and
promoting local assets, creative incentives for the change, alternative energy generation and

adequate waste management to avoid chemical hazardous with energy efficiency and reducing
emissions of air pollutants and greenhouse gases, and water conservation. The sustainable
healthy setting majorly concentrates on the management and maximum involvement of these
elements (WHO, 2017).
Although modern healthcare systems and hospitals highly resource-intensive, and deliver high-
quality care at the end of patients which is affecting to environment adversely that ultimately
leading unhealthy environmental setting. Sustainable healthy setting improves high technology
with minimum energy consumption, reduce or minimize the use of water, waste management
and treatment for sewage that are very helpful towards healthy setting in the hospitable as well
in the community. Thus focus of the WHO on the Sustainable healthy setting in for community
development especially in the hospital setting is very crucial.
Resilience healthy setting development in hospitals
Resilience Hospital health setting is an emerging concept. The Resilience healthy setting
develops resist capacity, absorbing abilities and enhances response for any negative
occurrence. This setting in hospitals developed to the hospital in such a manner to be prepared
for any disasters, to face the shock of disasters with maintaining critical health care functions or
proving health services to patients and then recover original or more developed state. (Zhong,
Clark, Yu Hou, & Zang, 2014 ).The resilience healthy setting majorly focuses on the
infrastructure development and environment sectors with aiming human health protection and
development and economic vitality. The overall health of humans that includes physical, mental,
and social well-being can be obtained by the resilience of healthy setting development in the
community (Sull, Harland, & Moore, 2015 ). The resilience setting development can be seen
NHS healthcare environment which is which helped keep medical professionals healthy and
develop the abilities to adapt to change in more stresses and a more complex working
environment.

Thus, these approaches help to develop a healthy setting in the environment to the welling of all
humans and help to achieve WHOs targets on the healthy and sustainable development of the
people. Although such approaches are widely using in the world in many sectors including the
health care sector of the country. However, sometimes these approaches fail to achieve targets
and health promotion and health development of people.
Policies and Practices of healthy settings in hospitals or healthcare
The Health care sector in the country United Kingdome, that widely covered Under NHS, is a
key factor to maintain the health and wellbeing of people in the country. It provides a
comprehensive framework which works encourages various stakeholders of health. The
healthy setting in healthcare in the country enables to develops connections between people
and develop the interactions between various health issues and initiatives. This sustainable
healthy setting can be developed at local regional, national and global levels (Dooris, Healthy
settings: challenges to generating evidence of effectiveness, 2006).
The healthy setting development concept is majorly known with the Ottawa Charter 1986, which
covers the availability of fundamental needs or resources that peace, food, shelter, education,
income, sustainable resources, a stable eco-system, social justice, and equity for the people
around them in the city. The document mainly focuses on the geographical setting approach.
Sundsvall Statement of 1992, which had been declared in The Third International Conference
on Health Promotion held in Sundswall, Sweden in 1991. This document is known for the
creation of a supportive environment in the health settings in the global context. In the healthy
setting development Sundsvall Statement targeted on rapid population growth, arsing conflict
and wars in the world, inadequate and unhygienic food, absence of self-determination mean in
the people and unethical degradation of natural resources and some other factors that ware
affecting to the environment adversely (WHO, 2019). Similarly, the Jakarta Declaration focused

on healthy setting and infrastructure development with implanting comprehensive strategy
public health throughout the globe (Kickbusch & Buckett, 2010).
Policies and practices in the context of the healthcare system or hospitals in the United
Kingdom have set out some principles to do good practice and protect and promote patients'
health interests in the health management system and to set legal requirements and other
standards with describing ethical conducts (NHS, 2019). These principles cover safety and well-
being of the people, Competence, Scientific and Ethical Conduct for the healthy development
and research, highly involvement of all stakeholder like Patient, Service User and Public,
Integrity, Quality, and Transparency in the services and research development and legislation
like Care Act 2014, The Health and Social Care Act 2012, Health Act (2006), etc. (Thorlby &
Arora, 2019).
Part 2
The above Section gives an idea of greening a healthy setting approach in health care or
hospitals and insight of theory application of various theories in the healthy setting and policy
framework of healthy setting in with focusing on healthcare and hospitals setting especially in
the United Kingdom. The major concern greening a healthy setting in the hospitals or healthcare
system is developing a balance between health promotion, disease prevention, and treatment of
illness, which is the greatest opportunity for the individuals as well as the community (Institute of
Medicine (US), 2003). This section of the study includes opportunities that include re-
orientation, patents supportive environment, increments in health support equality, development
of personal skills in the healthcare workforce, reduction and prevention from disease,

Opportunities for healthy settings in healthcare or hospitals
Re-orientation
The healthy setting approach in hospitals improves and enhances the primary care services,
through maximum participation of the community, strengthened community profiling and regular
assessments help to find out the gaps in the healthcare setting. Greening setting approach also
improves mobilization and inclusion of social services. The hospitals or health services that are
promoted by the Word Health organization also improve and expanded the integration in
evidence-based health promotion in acute health care and social care services. Apart from
these, the staffs of health care services and hospitals also gets enable meaningful engagement
and partnership to encourage considering policies and practices to shrink health inequalities
that ensure maximum health attainment (Aveling, Martin, Herbert, & Armstrong, 2017).
Create supportive environments for health
The greening health setting approach in healthcare settings promotes and improves health in
terms of lifestyle, medical, behavioural and social-environmental improvements. The setting
approach also works on a needs-based strategy that incorporates in finding coordination
between needier and health care providers. The sustainability in the healthcare setting gives
adequately resources strategy and monitor to structure, function and effectiveness of the health
services (SDU, 2019).
Development of personal skills in the healthcare workforce
Under greening healthy setting approach, skill development towards patients cares, community
services and welfare, leads an opportunity for the personal skills development of staff or
workforce which are working with healthcare or community care services (Kabene, Orchard,
Howard, oriano, & Leduc, 2006).

Reduce health inequalities
The main aim of greening a healthy approach is health promotion and development throughout
the globe. Thus, the setting approach provides an opportunity to enhance inter-agency
cooperation to improve health equality and identified the social determinants of health
enhancement. The healthy setting in hospitals develops quality services and quality from the
grass root level to provide services for the wellbeing of the community by providing health care
facility to all and reduce health inequalities. As above discusses healthy setting approach
increase participation of the community which allows identifying the needs which are a step
towards health equalities (DPHP, 2019).
Improve health
The greening healthy setting approach majorly works on the health improvement principle. The
approach creates awareness with increasing communication and community participation in
health care services. The sustainable healthy setting provides an eco-friendly hospital
environment and the concept of waste management in sustainable health settings also an
opportunity towards the healthy improvement of individuals and the community (Dhillon & Kaur,
2015). The resilience healthy setting also provides an opportunity to be prepared for accidents
or calamities to reduces health risks and provide maximum wellbeing. Besides, the healthy
setting mechanism also set the services and practices to targeting individual population like
children, adults, and older people and group of people with special interest that is also an
opportunity for the community to get services for particular interest (World Health Organization,
