1. Ways to improve the client of information.
It is very important to gather all the required information of the client in an appropriate manner
so that patient can get help with best possible treatment without facing any such issues at the real
time. In order to improve the performance at the time of collecting the client information’s, Lane
requires focusing on below points, such as-
It requires focusing on the process of preparing the document or software where all the required
information will be mentioned to ask from the client. Lane also requires to be making sure for
asking the entire question at one time and document them at the same time as well. It will
generally include the name, age, the gender of the patient. It will also include the earlier history
of medical, her/his diagnosis etc. Lane is suggested to prepare the file of all the documents of
each & every client specifically which will be time-saving process while looking for the
information's in future references (Kaakinen, et.al, 2018).
2. Technology for collecting, recording or maintaining the information’s of client
In order to collect, record or maintaining the client information in an appropriate manner some of
the software that is related to the nursing or healthcare services should get maintained. Maintain
the records in the word file will be the most appropriate way in which the personal details of the
client will get motioned. The Adobe Pdf files will also be useful to maintain the records properly.
The use of e-mail, database, client management system, excel sheet will also be very useful for
the same. It can be said that as observed computers are the high management system & efficient
information storage for keeping the data or information of the client safe & secure.
3. Guideline to write accurate progress notes
It is highly important to maintain the progress notes of the client time to time. The main purpose
of it is to provide the daily account of each patient & their illness or improvement in their care as

well. Lane is suggested to be concise while preparing the progress notes. It requires including the
details about the symptoms of the patient or the signs of the disease as well. It also requires being
very careful while describing the patient's treatment. At the time of writing the progress notes, it
also requires to take care of the privacy of the patient. The progress notes also include the
information about the stages of improvements of the client while taking the treatment (McCoy,
et.al, 2018).
The progress notes should be in the SOAP format which includes both presenting patients on
rounds &writing notes too. This format contributes in facilitating with a standard method for
client information. In the progress notes Lane needs creating the comprehensive records, keeping
only necessary documents, organizing notes appropriately, documenting the information
properly so that it can easily get retrieved when required. The information also needs to be
attested by senior management that will help in ensuring that information is valid &
authenticated.
4. Things that require to happen before Lane could start with these agencies or people
Colleagues- In order to share the information with a colleague first it is required to confirm that
if he or she is the right person to talk with. The information of that colleague needs to be taken as
well.
Nursing home- In order to share the information with nursing agencies or people it requires to be
confirmed that it is not hampering the privacy of the client’s information in any negative manner
& they will be the best people to provide accurate care to the client (Lindquist, et.al, 2018).
Schools- In terms of sharing the information with schools, it requires making sure that it is not
going to affect the status or mind of the client in any which ways. Lane requires confirming with
the client that whether he or she agrees to do this or not. It is importantly required to obtain all

the reliable & accurate information from the client with current status before sharing the
information with any other people or agencies (Davies, et.al, 2018).
5. Different methods of communicating the relevant information to people or agencies
In order to talk about the three different types of methods to communicate the relevant
information with agencies or people, it includes below points-
Verbal communication- In this process Lane can call the agencies or people to provide relevant
information related to the client. She will have to make sure that it is not getting recorded so that
no one can get the private information of the client. It will be less cost effective method.
Written communication- It will include e-mails, letter, fax etc. and with making sure that all the
personal information is well sealed, password protected and cannot get accessed from anybody
without permission. It will be helping in saving money or to documents each & very relevant
information in a proper manner (Smith, et.al, 2018).
Face to Face communication- This method will include conducting the meeting with other
person or agencies to provide the information of the client. As same it will also require to avoid
any other person who does not relate to the matter. Taking care of customer’s confidentially is
importantly required. This method will help in feeling comfortable to share the information by
being familiar to the other person. For verbal communication like via phone or face to face like
meeting, it is importantly required to be making sure that no any other person is hearing the
information of the client or patient. While talking on the phone it needs to be sure that the call is
not getting recorded, so that the information will not get disclosed to any other person. Likewise,
while talking face to face it requires to be sure that no any other person is presented there &
hearing that conversation (Silver, 2018).

6. What needs to do if a client has refused for providing the required information
If in case any of the clients have refused to share their information, Lane requires behaving very
calmly or patiently at that point in time. As same, she is required to make them understand that
why collecting the information is important and to make them sure that it will be confidential or
not going to share with any other person without their permission. The most important thing is to
make them feel comfortable so that they can get agree to share their information by themselves.
In order to gain their trust in this process, she is also required to make them knowledgeable about
the services or facilities they will be getting while their treatment (Leon, et.al, 2018).
If the client continues to refuse to share his or her information, then Lane requires reporting to
the senior management or senior professional of allied health services. She importantly requires
documenting the complete scenario or incident so that the health professionals may understand
the same & help her in the best possible manner. It will contribute Lane to get the best solution to
get rid of this issue or problem.
