INTRODUCTION
Psychosocial assessment is considered to be the important aspects and vital level of step in
order to have the creation of heath care plans by especially making the patient in palliative cares.
There are the proper evaluations of the patient mental, physical and emotional health’s in which
major level of importance is given to the patient level of perceptions in ability t gave the
functioning in the community (Reader and et.al., 2017). A psychosocial assessment can be
relatively simple or extremely complex. Whether simple or complex, a good assessment should
cover all the aspects of a person's life in order to get a picture of his or her mental state. In this
report there will be clear level of discussion on the psychosocial assessment of the City of
Horizon in terms of the mental health issues. In addition to the report will be structuring the
different parts which will have the discussion on the Psychosocial Assessment, Health-related
issue and the role of the Registered Nurse in health promotion. hence the aim of the report is to
have their determination of the present and future behaviour while preforming the health
programmes which can be termed to be more level of plan of management and actions in the
respective medical team in perfect manner.
Part A Psychosocial Assessment

research along the knowledge of the local populations profile in respective manner with their

control over there heath sector.
Education Level- the education has the major level of chance in creating there more level of
opportunities for the better health. As poor health can be the result of the educations attainment
at rise. 12.2% of students in the Limestone coast who attained a Year 12 qualification in 2015 As
the education have the providence of skill in order to anglaise there information and tackle the
issues to provide the navigation the modern health system. Almost one fifth (19.1%) of the 16
year old population of the Horizon were not participating in full-time secondary education. As
the result in the city about one fifth of community are not attaining the secondary education
Employment- the more level of employment opportunities ca be resulted in the increasing level
of the improvement in the health status and vice versa decrease their level of health care cost in

Income - the income and the wealth have the direct level of support of the better health as
wealthier people have the affording ability to have improvement in the conditions as compare to
low income. It has been analysed that most of the infants and children are been more vulnerable
as they have been raised in the low-income family. The larger number of adults has been faced

affected 10.7% of males, and 10.6% of females in Horizon (Scannapieco, Connell-Carrick and
Casolaro, 2019). The rural area like Horizon has been facing the problem that so many people

It has been analysed that in order to enhance the health outcome of the people living in
Horizon community, the role and responsibilities which has been done by the registered nurse is
really important. It has been analysed that the nurse’s needs to have full knowledge about the
problems which has been faced by the people living in Horizon community related to mental
health. They need to undertake the various strategies for the development of community, this will
include the following:
Enablement: Enablement is an intervention by which the health care provider recognizes,
promotes and enhances patients' ability to control their health and life. In this health service
community will be engaged in making use of evidence based intervention so that the problem
related to mental health which has been faced by the Horizon community can be overcome. They
also will be engaged in analysing the main reason behind the problem related to mental health
issues. In this nurses will also be engaged in checking upon the past medical history of patients
and also the different social determinants which has affected the life style of people living in
Horizon community. Health service community will also be involved in doing effective
interaction with the patients suffering from the mental health disorders. This will assist them in
enhancing the health outcome of people suffering from the problems like depression, anxiety and
various others. In the enablement intervention the nurses will be engaged in making use of the
patient centred model, so that they will be able to build up the therapeutic relationship with the
people suffering from mental health issues in the Horizon community (Oram, Khalifeh and
Howard, 2017). It will also assist them in building better relationship with patient and also nurses

been analysed that the nurses also needs to make sure that the cultural belief and thought process
of the patients is not been hampered by them. This will assist them in enhancing the quality of
life and well-being of the patients suffering from the mental health disorder. It will enhance their
health outcome.
