1. Through internet search, Extendicare Leduc was selected for gaining further insight into
the specifications of selection and recruitment. In the process of selecting this care home,
information was gathered about the quality care and treatment, which is offered to the
customers. Caring for the people is the main motto behind the operations. The staffs are skills
and qualified, which produced good practices towards catering to the needs, demands and
requirements of the customers (Extendicare.com, 2020). Collaboration with government
agencies, communities, researchers and care partners reflects the extended supply chain
network of the care home. The mission and vision statement revolves around enhancing the
standards of living of the people by emerging as one of the top class care services in the
threshold of Canada (Extendicare.com, 2020).

 Hello sir, myself Bismark Kofi Oppong, aspiring to be a nurse. I intend to pursue the
position of personal support worker for serving the mankind by ensuring their
wellbeing. Hard work and commitment are the agents, which helps me to accomplish
the identified and specified goals. Previously I worked as a line lead in a packaging
company; shuttle driver; receiver/shipper, which reflects my competencies. I have
achieved diploma degree in nursing and have completed bachelors’ degree in social
work. I am also passionate towards sports and have bagged awards like best
goalkeeper of the year; valuable player, certificates, medals and others. In addition to
this, I have enrolled myself in the course of CPD, the duration of which is from Dec
2019 to Dec 2022.
 Well, I think I am responsible, which is reflected from the responsibility towards
undertaking challenging and enduring tasks. I have experience in leadership, which
enhances the scope and arena of my performance. I like to interact with people, which
I feel is important in terms of increasing the preconceived knowledge, skills and
expertise. Dedication and ambitious are also the qualities, which adds to my

personality. To tell about my weakness, I am weak in fluent communication. I also
have stage frights, which compels me to encounter unproductive results towards
giving presentations.
 At one time, one of patients needed high quality care. I was allotted his responsibility.
The head of the department entrusted me with the responsibility of preparing the excel
sheet for the medicine supplies. Once the patient was in need of something for which
he called me, but could not find the required help. The family of the patient
complained to the head of the department about my careless approach. I was called in
a meeting for providing justification. It was a challenge for me.
 I dream of becoming a successful nurse. For this, I have played a proactive part in
volunteering the team projects, team work. Hard work and commitment are the
agents, which contributes towards accomplishing the identified and specified goal.
Ambitious nature would be efficient for me towards expanding the scope and arena of
my skills, abilities and competencies. I think self-evaluation would be effective for
ensuring their feasibility towards achieving the goals. Consistency in this evaluation
would be effective in terms of upgrading the standards and quality of my
performance.
 Coping into a new environment would be a challenge, if I do not get support,
motivation and encouragement from the seniors. I specifically need training in the
area of problem solving and creativity for expanding the scope and arena of thinking
skills and judgmental capabilities. Along with this, conflicts, misunderstanding
between the team members would act as a challenge in terms of getting the crux of the
business and working according to the needs, demands and requirements. Apart from
this, if the managers are not supportive, I would not be able to approach them for

clarifying the doubts. This issue would be a constraint in terms of enhancing the
stability in the professional relationship.
