also needs to be aware about the independence in the facility and the daily personal works. Here,
I can reflect that as a social care practice worker I should help Mr. K in the process of the
independence development. Empowerment and education providence to the patient with some
disability can improve the social interaction of the person as well as the effective knowledge
development about the self efficacy (McLoughlin et al. 2018). Thus, collaboration and
communication can effectively improve the social interaction and daily living of the patient
through providing him with positive information regarding the independent living.
The person should be provided with the education through the practical way through
improving his knowledge regarding daily independent living. Thus, I should communicate with
him through application of the communication strategies such as the active listening, positive and
emphasising tone in the communication process and also non-verbal communication. The rapport
development of the with the person will be very much important. The factor of the
communication would be focusing on the medication management education, physical activity,
proper diet following and also spiritual conversations. This way the patient can be empowered
regarding self management and the relationship with the patient can be improved. The teamwork
with the patient can improve the health outcome as this will improve the daily activity regarding
motivation along with skills of the patient. Thus, this should be stated that the interaction and the
collaboration with the person would help in checking the care plan and daily routine of the

important and considered with priority. The development of the skills and the knowledge about
these care processes will be effective in the professional d3evelopment of the social care
workers. Hence, I can find that the process of the development in the professional field can be
achieved through proper education and skill development trainings (Karvinen-Niinikoski et al.
2017). Thus, the supervision of the seniors and the proper collaboration would be effective in
this process. I can also find that in the process of the professional development as a social care
worker I can improve the skills and the competence of the nursing care in consideration of the
