Topic 1: Communication, Collaborative practice and Teamworking
Social care community rehabilitation home is the place where the people with disability
are provided with the shelter and also the effective and functional independence for leading a
normal life. In this context, Mr. K is one of the residents of the social care community
rehabilitation home and the person has some incontinence issues as well. However, the person
also needs to be aware about the independence in the facility and the daily personal works. Here,
I can reflect that as a social care practice worker I should help Mr. K in the process of the
independence development. Empowerment and education providence to the patient with some
disability can improve the social interaction of the person as well as the effective knowledge
development about the self efficacy (McLoughlin et al. 2018). Thus, collaboration and
communication can effectively improve the social interaction and daily living of the patient
through providing him with positive information regarding the independent living.
The person should be provided with the education through the practical way through
improving his knowledge regarding daily independent living. Thus, I should communicate with
him through application of the communication strategies such as the active listening, positive and
emphasising tone in the communication process and also non-verbal communication. The rapport
development of the with the person will be very much important. The factor of the
communication would be focusing on the medication management education, physical activity,
proper diet following and also spiritual conversations. This way the patient can be empowered
regarding self management and the relationship with the patient can be improved. The teamwork
with the patient can improve the health outcome as this will improve the daily activity regarding
motivation along with skills of the patient. Thus, this should be stated that the interaction and the
collaboration with the person would help in checking the care plan and daily routine of the

person and based on that properly communicate and motivate the person to do all the works
properly (Reeves et al. 2018). Furthermore, the rehabilitation of the patient can also be
considered as this can improve the knowledge, skill and also critical thinking of the patient
regarding his health. The education or the communication with Mr K will always help him
remember his daily works such as washing is clothes, medication dosage and timings, watching
the beddings, clean his room on daily, make his beds and other factors as well. This
empowerment process is focused on providing him information regarding the positive impacts of
these works on his health and daily life. This way the patient can be empowered for his
independence development (Craig et al. 2020).
This, can be marked as the person centred care delivery to the patient and this process
should be followed. Thus, I can reflect that the process of effective and improved care deliver is
dependent on the collaboration, communication and teamwork of the care worker with the
patient, as the empowerment development for the patient is dependent on this factor solely. In
this case, Mr. K can be empowered with the person centred care approach with consideration of
the positive communication, collaboration and therapeutic relationship development and also
teamwork (Castro et al. 2016).
Topic 2: Professional Development
Social care practice work in the social care community rehabilitation home is a complex
job as each staff needs to know different processes and should have different skills regarding the
care process as well. Hence, it can be stated that the process of the care delivery should be
developed with proper planning and also focused on the proper professional development as it
will effectively help in the improvement of the skills of the care staff (Nickson, Carter and
Francis 2019). Thus, I can reflect that the knowledge and skills of the social care worker is very

much important as there are need of several procedures in social care community rehabilitation
home to care for the residents. First aid, managing challenging behaviour among the patients
with mental disorders, CPR and many other skills are needed to be developed. Hence, the
supervision and training from the senior co-workers or the educators would be effective in the
process.
I can reflect that the need of the supervision for the training and skill development is very
important and considered with priority. The development of the skills and the knowledge about
these care processes will be effective in the professional d3evelopment of the social care
workers. Hence, I can find that the process of the development in the professional field can be
achieved through proper education and skill development trainings (Karvinen-Niinikoski et al.
2017). Thus, the supervision of the seniors and the proper collaboration would be effective in
this process. I can also find that in the process of the professional development as a social care
worker I can improve the skills and the competence of the nursing care in consideration of the
person centred care delivery process. I should focus on the professional development for the
future career aspect and also improving the care skills as well. These processes will help me in
obtaining better health outcome for the residents of the social care community rehabilitation
home.
Thus, I can reflect that the importance of the professional development should not be
neglected and also the improvement should be developed under proper supervision and help in
the knowledge development (Jølstad et al. 2017). I would also state that the professional
improvement or development should be obtained regularly through learning and competence
development for delivering improved and proper care to the patients. Hence, the importance of
the supervision in the professional development process can be seen in the process of the

knowledge and skill development processes in the practical care setting (Røsnæs et al. 2017).
The process of development for the social care workers should be improved through proper
supervision implementation and also providing effective education to the workers as well.
Hence, the development in the professional practice should be considered with higher priorities
and as a social care worker I can reflect that the supervision of the trainers or the seniors can
effectively help in the improvement of my clinical and nursing care processes properly.
