INTRODUCTION
In the health care settings, professional values and principles are defined as the guiding believes
which influence work behaviour of healthcare practitioners. The values involve distinct concepts
including privacy, dignity, respect, independence, choice and partnership (Watzak, and et. al.,
2015). There 5 core principles of healthcare including safety, privacy, Independence, dignity and
communication. This report is based on utilisation of practice themes as model for reflection.
This report covers explanation of methods utilises to gain evidence of effective reflective
practice and a portfolio of evidence of reflection is developed by using practice theme
framework. Apart from this, it includes evidence gathered in own portfolio and creation of
targets for own professional career pathway development.
TASK 2
P3 Explain how the Practice Themes can be used to underpin reflection in learning and practice
In the setting of practice based professional learning, reflective practice is an important tool
their individuals learn from own professional experiences instead of knowledge transfer or
formal learning. While reflecting, it is required by the professional to consider 6 theme
frameworks for healthcare quality including safety, timeliness, effectiveness, equity, patient
centeredness and efficiency. In the healthcare setting, all these six themes are important to be
considered while providing care to the patients. The six theme framework for assessment of care
quality have directed measure development step in public and private sector. The first theme that
is safety is concerned with avoiding harm to care uses from the care which is intended to help
them. The second aspect, i.e., effectiveness is related to administrative services on the basis of
scientific knowledge to all individuals who could refraining and benefit from offering services to
those who not possibly to benefit which means avoiding misuse and under use. The third aspect,
i.e., patient-centered is associated with giving care which is respectful and responsive to needs,
preferences and values of patient and ensure that all the clinical decisions are guided by the
values of patient.
In fourth aspect, timely, in this the health Care professional need to reduce waiting time and
harmful delays for those who give care and who receive care. Efficient aspect is associated with
avoiding waste involving waste of supplies, energy, ideas and equipment. The last aspect that is

equitable which is related to giving care which is which does not differ in quality due to personal
characteristics like ethnicity, social economic status, geographic location and gender. All these
themes can be utilized by healthcare professionals to underpin reflection in practice and learning.
Reflection is the significant source of personal as well as professional development and
improvement. It is the way to bring together practice and theory. By the help of reflection, an
individual is able to label and see forms of thought in the context of own work. Reflection
allowed the healthcare professional to manage the influence of caring for other individuals. The
practice theme helps the professionals in reflecting upon the learning and experience that they
get while implementing these themes in their working as professional in hospital. Reflection
provide the professionals with the opportunity to look upon each phase of process of caregiving
and take decision regarding appropriate action to be taken which ultimately helps in improving
quality of care.
P4 Illustrate in detail the links between the Practice Themes and other performance indicators
Performance indicators in healthcare are referred as the popular mechanism to measure the
healthcare quality in order to facilitate systems management and quality improvement. The key
performance indicators are well defined measurements performance which is utilised to monitor,
assess and optimise all the relevant processes of healthcare in order to increase the satisfaction of
patient. The most important performance indicators in healthcare includes hospital readmission
rates, costs by payer, patient Safety, patient satisfaction, average hospital stay, treatment cost,
patient wait time etc. All these performance indicators are linked with practice themes including
safety, effectiveness, equity, timeliness, efficiency and patient centeredness.
The key performance indicator of patient satisfaction is linked with the effectiveness theme
as if an effective quality services are provided by the professionals to patients then it will help in
satisfying them. Apart from this, treating all the patients equally regardless their gender, colour,
caste, religion etc. will also help in satisfying the patient and providing them safe environment.
Thus, the performance indicators and linked with the practice themes in healthcare settings. In
healthcare, reflection is referred as the active procedure of reviewing, analyzing as well as
evaluating the experiences, drawing upon previous learning of theoretical concepts to inform
future actions. It provides a way to healthcare professionals to deal with sensitive problems they
are possibly to experience.

Reflective practice is referred as the method of analyzing own actions and thoughts for the
aim of personal development and learning. Towards the continuous professional development,
reflective practice is very helpful. it is also known as personal reflection, self-awareness, self-
appraisal, reflective dialogue, critical evaluation, self-criticism, self-assessment, intrapersonal
awareness etc. There are some reflective frameworks which are utilized by healthcare
professionals to reflect on their own experience in systematic manner. Reflective practices
include an active, dynamic action based as well as ethical set of skills which are placed in real
time in dealing with difficult, complex and real situations (Levin, 2015). Reflective practice
provides a healthcare professional with huge benefits in terms of increasing self-awareness
which is the main component of emotional intelligence and help them in developing good
understanding of others. It also helps professionals in development of creative thinking skills and
promotes active participation in work processes. There are some methods which are utilized to
gain evidence of reflective practice effectively in own area of work practice (Ellis, 2016). Some
of the methods are mentioned below:
Reflective journals:
Reflective journal is one among the methods which can be utilized by an individual.
Keeping a reflective journal is the way to reflect by documenting observations, visions, ideas and
feelings. It can be done on computer or a paper and it helps an individual to emphasize on own
thoughts and develop own ideas. It also helps in organizing own thinking by exploring and
mapping critical issues. Apart from this, it also assists healthcare professionals like nurses and
other medical staff in expressing feelings and emotional responses, developing conceptual and
analytical skills, conversations with others, writing skills, and becoming aware of own actions
and strategies (Tennant, Garmory and Winsch, 2015).
Peer groups and cooperative enquiry:
A peer’s groups who mitigate on continuous basis to learn and reflect collectively can be a
strongest supporting element of personal reflective practice. The group that decides collectively
the way to utilize and organize its time may discuss issues associated with the work, share

learning journal excerpts or try to develop collective reflective practice. It includes group
working by a structured four phase cycle of reflection and action, by which the members of
group move towards developing noble ways of acting.
P6 Produce a portfolio of evidence of reflection using the Practice Themes as a framework
Portfolio of evidence is referred as a form of assessment that administers samples of work of
nursing professionals that reflect their development over time. Reflecting on own learning
provide healthcare professionals with the opportunity to determine strengths and weaknesses
related to the work which further becomes their improvement goals (Brenda Scott, and Mindy

Ineffective communication and team working skills was the biggest challenge that I faced while
achieving the practice criteria.
Are there any specific examples from your placement that evidence your

Yes, from the placement, I learnt many things and it improves my communication and
interpersonal skills. While working in the healthcare setting, I am able to deal with the issues
associated with care of patient very effectively.

this element of the unit?
As an outcome of completing this element of unit, the skills or qualities that I need to develop
includes communication and team working skills. By developing these skills, I will be able to

In order to develop communication and team working skills to further help in improving

The evidence gathered in own portfolio highly contribute in my own personal and
professional development. By creating the portfolio, I learnt many things including the
significance of reflective practice in improving the quality of healthcare in health and social care
settings (Minoshima and et. al., 2016). This in determining my strong as well as weak areas
along with the areas on which I need to work to improve them so that I will be able to provide
high quality care services to the patients along with providing them with safe and secure
environment. It also helps me in maintaining the dignity of patient and treating each patient
equally with equal respect and dignity. Creation of reflective portfolio help me in knowing about
the six practice themes as well as their significance in healthcare settings. Apart from this, I am

able to know the significance of reflective model in providing self-reflection in systematic way.
Gibbs model of reflection and Kolb's reflective cycle are the two main reflective models that
help in performing the reflection systematically and assessing the experiences from a particular
situation in an effective way. In context of healthcare setting, reflection help in effectively
reviewing and evaluating experiences regarding previous learning to inform future actions. The
significance of 6 themes in providing care to patients is determined with the help of this unit.
P8 Produce targets for the development of own professional career pathway
In order to develop my own professional career pathway in health and social care settings,
the targets which set includes:
Improve skills and competencies needed to provide effective and high quality care to
patients.
Providing care responsive to the needs and requirements of patients or service users
(Hughes, Colbert, and Baugh, 2020).
Learnt to treat each individual equally regardless their religion, culture, gender, colour,
caste, creed, nationality, geographic location, socio economic status etc.
Improve knowledge of regulations, practices and policies associated with health and
social care.
All these are the targets fulfilment of which helps in development of own professional career
pathway.
CONCLUSION
As per the above mentioned report, it has been concluded that reflecting on own experience
is necessary for health care professionals in order to improve the quality of provided care by
taking crucial decisions in an effective manner. considering six practice themes will help in
growth and development of career pathway of heath care professional.
