1. Brief Introduction
It is approach to the nursing practice which uses the current best support during the clinical
decision making. It helps to care better for patient with the help of the nurses and changing their
behavior when evidence says change is need. The support comes from the evidence and the
research.
2. Presenting complaint, assessment data and diagnosis
 The data of the observation help to gain the picture about the real pain management
practice.
 Questioning the knowledge of nurse and how well they felt while managing the pain of
the children.
 The data about the how critical perceived the task of the pain management.
The above database will analyses and compare with the theory of the nurses.

3. Past medical and health history including medications
 Review of the documents which are related to past and to NLPC model
 Interviewing with the nurses who are practicing from the years.
 Charts audit is conducted to which guidelines were followed for patients with the chronic
condition and checking the past detail of the patient.
4. Discuss the clinical manifestations of the chronic health problems on the body systems
There are several clinical manifestations of the chronic health problem which are as follow.
 Nausea
 Vomiting
 Fatigue and weakness
 Sleep problem
 Decreased mental sharpness
 Persist itching
 High blood pressure
Sign and symptoms of the chronic health problems on the body systems sometime nonspecific
and it can happen due to several other illness. In some cases, the sign and symptom may appear
when the damage occur.
5. Identify two (2) actual and two (2) potential health issues of the patient
Actual health issues
1. Cardio vascular diseases- if a person is smoking then they can have the sign of Cardio
vascular diseases
2. Stroke- A person can face the stroke due to several reasons.
Potential health issues
1. Heart Disease
A patient has the heart rate and high blood pressure. Sometime sudden stress can trigger
and nurse always try that patient stay away from stress to avoid the chronic pain.
2. Diabetes
Patient has a diabetes that increases the bad behavior and increase the unhealthy eating
and high level of drinking. Even sometime the level of the glucose increased drastically

1. Prescription: the better nurse knows about the condition the better they understand
why it is happening and give the prescription according to that.

2. Follow the doctor’s care: The best treatment is care by the nurses. The better they
care the faster the patient recover.
1. Clinical nutrition: It helps to maintain the nutrition of the patient. The better food they eat
the better will feel.
2. Psychology: It provide the better assistance, treatment and the consultation services for
the patient.

a. Identify two (2) nursing interventions you performed based on the patient’s needs
according to continuum of chronic disease
Follow up visits: The more nurse visits the better they care
Duration: It helps to work in detail.
b. Discuss the patient’s (as appropriate, with family or care taker to confirm their)
understanding of the conditions and its impact.
The follow up for the patient who has the diabetes can drastically improve the
psychosocial and health outcome which can be describe as the families cost
saving, better satisfaction and better clinical outcome.

9. Explain how you actively involved the patient in the development of strategies to manage
their condition. Write what progress notes documentation you did for this patient.

 Clear explanation about what can happen and open about the treatment and the
condition.

Family supports in several ways and one of the best ways they contribute is checking the
patient’ health and caring for them. Even they can focuses on the improvement of the
patient by giving them good support and medication.
