INTRODUCTION
Nowadays, there are several changes are taking place in the field of science, health,
personal as well as social care. Person centred approach in health and social care refers to give
more importance to individuals and provide them personalized services. It ensures to provide
services as per the needs and requirements of different individuals that is crucial to satisfy them.
In this, emotions, feelings, opinions of individuals are consider while providing health services.
It stands to put people and their families in centred while taking necessary decisions. This
practice is evaluate by managers on continuous basis in order to make necessary
improvements(McNamara 2020). The present report is based on reflection which gives a detailed
overview regarding hardships faced by an individual in implementation of person centric
approach in the field of health and social care. It describes implementation of medical and social
models in person centre practice in health care. In addition, it also comprises different
legislations and provisions of person centred approach. Here, several suggestions for further
improvements are also discussed in current report. Lastly, it includes an action plan which need
to be develop to make improvements in skills and potential for further enhancement.

TASK 1
P1 Discuss association of medical and social models that are apply to person centred practice in
health, care or support services
Person-centred approach refers to provide personalized care to patients as per their
desires. It based on the concept that all the individuals are different thus have different, needs
and wants which need to consider while providing services. Here, patients are health
professionals who provide care to other individuals regardless their own health conditions. Here,
individuals are treated independently who have right to take their own decisions and receive
services as per their needs. There are several norms, guidelines that have determined by higher
authorities need to follow with an aim to provide more enhanced services to patients(Edvardsson
2020).
Here, health worker who render services to patients need to have higher skills and
potential so that they can serve patients in better manner. Physical, emotional well being of
patients is also important to make the whole process effective and deliver positive results to

health care professionals and patients. In this, social model asses different variables such as
social, culture, environment that also play a crucial role in overall well being of an individual.
Main focus is on promotion of health policies and education in order to spread awareness among
patients. On another side, medical model states that there should be a systematic process to
assess disease in order to provide appropriate treatment to individuals. There should be process
like testing, identification of disease with an aim to provide better treatment to patients. Under
this, cause of disease is identified in order to improve health condition of people.

A person-centred approach refers to the the nursing which is the individual need and want
of people which help them to become the centre of such process. There are different
consideration which should be made while planning and implementing person-centred approach

Physical support and personal care
While planning and implementing consideration is one of the main program which is for
the individuals require supports which is related with the health professionals provide physical
support as well as personal care to the needed person or patient. Such that patients who are able
to take such decision which are more informed and with confidence.

they need to to be provided support which is for their daily living needs. Such type of practice is
most successful which is whenever their family and friends of the patient also try to provide

This refers to the time when providing care one should keep in mind that every each and

The patient or individual who is receiving the person centred care that one should be
treated with respect along with dignity. There is need to to follow the all help along with safety
standards and also different regulations by the health-care professionals during delivering their
services. For example- NHS which is National Health Services follows the different type of
health and safety standards which are able to a sure the security of the patient.
Safeguarding
There are some of the law for safeguarding the patient which defines that local authority

There is a need to revise the policies from time to time to get ensure that presents

There is chance of miscommunication which is either due to not properly complicated in
between health-care professionals with their patients where the information is not passes to the
receiver clearly. This is the one through which it creates barrier.

should need to be followed by health-care professionals. The need to plan properly then

P5 Reflective account
During working in the NHS, I got an opportunity for working as a part of person-centred
practice where our team had to focus on providing special care and support to ensure that they
can trust us. I have employed and interacted with patient to learn more about him. Present was
the main focus of our care help me in realising that each and every patient is unique and and
requires different type off care and there is also clearly identification about the need. I was
successful in completing the expectations which I have from myself and also from the team
members. Working as a team I found that my strength and the witnesses. There are many cases
where I received criticism for not being able to perform some task in appropriate manner (Sawan
2020).

improving the the team performance and also the overall productivity which is related with the
team. Working together is able to help in building some confidence which is related with the
team members and along with their productivity of working as a team towards achieving the
objectives and the goal of the organisation (Fortin 2017).
Team leaders or managers which are the one who are able to act as a mentor or coach and
arrival to provide counseling as an advance to any of the team member whenever it is needed.
Time to time feedback can also be provided which will be able to help them in realising about
their resistance along with a particular area where they are needed to improvement as an
individual. Each and every team member need to have the complete knowledge which is related
to the patient or individual who are receiving person centred care so that a there is need along
with the Expectations of their family members which need to be met effectively. This can help
the organisation in building Goodwill which is not only in the market but also to their patient and
their relatives or family member to (Saviano 2017).

From the above report there can be concluded that it is important to have the person
centred care to the patient which can help in making the present better confident which is related
to the speaking along with compatibility of the services which an organisation are trying to
provide them. The patient should also need to trust on the other health professionals which from
the health-care department which is most appropriate for them. Then it to get the chance to
identify about their strength and different area which is needed to worked up for the
improvement. This can be done when there is an action plan which need to be used for attaining
various goals throughout the systematic steps in the organisation.
