2. Identify the client’s current situation in relation to health and well-being
indicators.
3. Identify the client’s needs.
4. Make decisions regarding the services to be provided taking into account the
information you have gathered.

You will then be required to carry out an analysis of your client using the questions
provided to structure your answer.
New Entrants
You will be required to obtain access to 3 individual client case notes or case studies
detailing work with groups/communities at your work placement, and use these as

seen a gang of mobs beating up a man on a street and since the, he has been hearing
