The Script:

The first half has just started. Nurse 1 went for her regular runabouts. She came out and approach
to other nurses.
Nurse 1: Have you with checked all the patients?
Nurse 2: Not yet. Why?
Nurse 1: The patient in the left corner has a reddened skin area.
Nurse 2: Really? Are you sure it’s a new development?
Nurse 1: I did not see it yesterday. I checked with everyone including him before leaving.
Nurse 2: Let me check the updated report on him.
Pause for two minutes…
Nurse 2: Yes, you are right it is a recent development. Let me see.
Nurse 1 and nurse 2 went inside…
Both came out after two minutes.
Nurse 1: What do you think it is?
Nurse 3: give me a brief idea.
Nurse 2 explained the situation.
Nurse 3: It is a definitely pressure ulcer.

Nurse 1: Go and inform the dietitian, at once. Also, inform the doctor.
Nurse 2: On that.
She: Check the condition of the patients, and note other significant changes if there are any.
Dietitian came in…
Dietitian: What is the condition?
Nurse 1: It is pressure ulcer stage 1. We have to change the diet.
Dietitian: Sure. Give me his details and let me check his previous records.
Nurse 1 handed the file.
Dietitian: The chart will be made within 20 minutes and I will inform my kitchen team.
Nurse 1: Thank you.
Nurse 1 made a care plan.
Nurse 1 to Nurse 2,

Nurse 1: You are in charge of his medicines and you will talk to the doctor. Update the result and
report each day, and take care of every words of the doctor and always check with his medicines.
Nurse 2: Okay.
Nurse 1 to Nurse 3,
Nurse 1: Turn the position after every two hour, and change the pad along with it. Also, put a
pillow underneath the ankle, and make sure after every two hours change shave to be made.
Nurse 3: Okay.
Nurse 1 to Nurse 4,
Nurse 1: Check his diet every day, and maintain the records on him. The entire plan should be
continued for a month and then we will assess him again.
Nurse 4: Okay.
After a month patient was perfectly fine. The goal was achieved.
The Report:
Introduction:
The continuity of care refers to the quality of care that is provided to the students. It is the
process of taking care of patients over the time. It is observed that in the care giving section an
entire team is involved to provide the proper attention and care to all the patients, especially to
those who come with an extreme heath conditions and spend a good amount of time at the
hospital (Shin et al. 2014). It is a continuous process and the aim of the process is to provide
quality care and to the patients.

Importance:
The practice of continuity of care is very important in ore to ensure a complete healing of
the patients. Especially for the patients with disease like Cancer, such an approach is very
important. The studies have shown that those, who spend a more number of time at the hospital
are tended to get less attention over the time or the appointed patients or the care giving team is
changed for him or her (Jeffers, and Baker. 2016). However, this is not a right approach, a
patient must be given the proper care as long as he or she need sit. Also, the researchers have
shown that continuity of care through the same team gives the patients a mental satisfaction,
which helps in the healing of the same (Gray et al. 2015).
Conclusion:
The importance of staying with one doctor and one health care team is more satisfying
and it is more helpful for the patients to recover their situation. A trust relation is built which
then helps in the betterment and the development of the health of the patients.
