The Social determinants of health is referred as various non-medical factors that influences
health outcomes. There are various factors are included in social determinants such as economic
policies, systems, social norms and political systems as well that affect daily life style and
conditions of local tribe. Delivery of culturally safe care is the branch of health care services that
are related to the maintenance as well as improvement of health condition through prevention,
diagnosis and treatment of people. Change leaders is defined as the growing community that
provide various opportunities as well as learning and development through various conferences
and experiences. In this report two parts are covered, in first part Gibbs reflective model in
discussed and in second part aspects of change of leaders and social determinants are covered in
this project report.
PART1
Gibbs reflective Model
Gibbs model is developed by the Graham Gibbs in the year of 1988 in order to gain
structured learning from various experiences. It provides systematic framework to examining the
experiences to gain proper knowledge about particular things. It is the most famous cyclic model
of reflection that leads everyone to explore their experience through six stages such as
description, feelings, evaluation, analysis, conclusion as well as action plan. In this reflection
part my experiences and opinions related to the delivery of culturally safe care as well as change
leaders worked to address various social determinants of health in order to improve health issues
and outcomes of Aboriginal and Torres Strait Islander people who are the indigenous Australian
people.
Description: For this assignment I have decided to divide this islander people in different
sections in order to research all information about each element clearly. As Aboriginal and
Torres Strait Islander people are come from backward area so, they do not get proper facilities
related to medical. So, I have researched all area as well as decide to provide various medical
facilities in order to improve their life style as well as enhance their medical conditions
effectively. Proper written plans are developed according to defined time line in which these
objectives to be fulfilled (Browne,2018). The present situation of these people is too bed. In

Feelings: During the analysation of Aboriginal and Torres Strait Islander people I felt
that the condition of these people related to health and it is required to provide better quality
services in order to improve their life style. I have faced various problems during research about

is faced by me to analyse and evaluate information. I felt that after providing health services the
condition of people who lived in remote areas of Australia is really improved. During whole
situation I think that these people react in positive manner that motivates me to work in same
direction with great zeal.
Evaluation: I have faced various challenges as well as opportunities during this
assignment such as lake of support and communication with the tribe that takes to much time to
complete the project. Apart from that this assignment enhance various skills in my personality
like team sprite, effective communication and time management that enable me to enhance
career opportunities as well. I think that effective implementation of policies and bring medical
facilities to the tribe is reason that affect the project. The Icelander people support the leaders to
bring medical facilities in their areas.

in negative manner as well. I have analysed that social determinants help to enhance the social

this project in efficient manner various primary as well as secondary research is done that
facilitates to gather reliable and accurate information in effective manner.
