INTRODUCTION
The Social determinants of health is referred as various non-medical factors that influences
health outcomes. There are various factors are included in social determinants such as economic
policies, systems, social norms and political systems as well that affect daily life style and
conditions of local tribe. Delivery of culturally safe care is the branch of health care services that
are related to the maintenance as well as improvement of health condition through prevention,
diagnosis and treatment of people. Change leaders is defined as the growing community that
provide various opportunities as well as learning and development through various conferences
and experiences. In this report two parts are covered, in first part Gibbs reflective model in
discussed and in second part aspects of change of leaders and social determinants are covered in
this project report.
PART1
Gibbs reflective Model
Gibbs model is developed by the Graham Gibbs in the year of 1988 in order to gain
structured learning from various experiences. It provides systematic framework to examining the
experiences to gain proper knowledge about particular things. It is the most famous cyclic model
of reflection that leads everyone to explore their experience through six stages such as
description, feelings, evaluation, analysis, conclusion as well as action plan. In this reflection
part my experiences and opinions related to the delivery of culturally safe care as well as change
leaders worked to address various social determinants of health in order to improve health issues
and outcomes of Aboriginal and Torres Strait Islander people who are the indigenous Australian
people.
Description: For this assignment I have decided to divide this islander people in different
sections in order to research all information about each element clearly. As Aboriginal and
Torres Strait Islander people are come from backward area so, they do not get proper facilities
related to medical. So, I have researched all area as well as decide to provide various medical
facilities in order to improve their life style as well as enhance their medical conditions
effectively. Proper written plans are developed according to defined time line in which these
objectives to be fulfilled (Browne,2018). The present situation of these people is too bed. In

order to improve their heath related issue I have created proper plan to complete the research on
time.
Feelings: During the analysation of Aboriginal and Torres Strait Islander people I felt
that the condition of these people related to health and it is required to provide better quality
services in order to improve their life style. I have faced various problems during research about
that people and in order to provide medical services, communication is the biggest challenge that
is faced by me to analyse and evaluate information. I felt that after providing health services the
condition of people who lived in remote areas of Australia is really improved. During whole
situation I think that these people react in positive manner that motivates me to work in same
direction with great zeal.
Evaluation: I have faced various challenges as well as opportunities during this
assignment such as lake of support and communication with the tribe that takes to much time to
complete the project. Apart from that this assignment enhance various skills in my personality
like team sprite, effective communication and time management that enable me to enhance
career opportunities as well. I think that effective implementation of policies and bring medical
facilities to the tribe is reason that affect the project. The Icelander people support the leaders to
bring medical facilities in their areas.
Analysis: Through this project I think that the procedure of providing medical services to
the Aboriginal and Torres Strait Islander people plays important role as well as support of these
people is the make project more effective. Communication and reachability to these people affect
in negative manner as well. I have analysed that social determinants help to enhance the social
life and health conditions that facilitates them to earn effective livelihood. In order to complete
this project in efficient manner various primary as well as secondary research is done that
facilitates to gather reliable and accurate information in effective manner.
Conclusion: From this assignment I have learnt various skills and competencies such as
communication skills, effective decision making, leadership, team sprite as well as time
management skills that impacts positively on my personality (Carey, 2017). I have analysed that
social determinants of health improve health of people and maintain equality through enhancing
their educational, income as well as affordable quality health services that enhance their life and
working conditions as well.

Action plan: I have prepared proper action plan that also help me in implications of these
skills in future projects as well as obtain various career development opportunities. According to
my point of view there are some corrections are required that help to achieve desired goals in
effective manner. I think some other skills are required to attain future goals as well.
PART 2
Explain the social determinants (Racism) and how this is a barrier for Aboriginal and Torres
Strait Islander people
Social determinants: Social determinants of health is described as the condition in
respective environment in which for Aboriginal and Torres Strait Islander people born, learn,
live, work and play that affect the health, functioning as well as quality like outcomes and risks.
There are five various determinants are included such as economic stability, healthcare assess
and quality, social and community context, education and neighbourhood and built environment.
Racism discrimination is determined as important key of social determinants of health as
well as driver of racial health inequality within for Aboriginal and Torres Strait Islander people.
It is referred as the poorer outcomes that reduces the access of health care and patient
experiences (Hanley and Marchetti, 2020). It is found that prospective cohort provides strong
causal evidences regarding various impacts of racism on health and healthcare outcomes.
Racism as a barrier: Racism is act as the barrier for the for Aboriginal and Torres Strait
Islander people that affect their health care qualities and discriminate them with other people in
effective manner. perceived racism creates various difficulties in accessing health services of
people of Australia. Along with class-based discrimination happened through racism develop
different barriers that affect health care of woman, low socio-economic status individuals and
minorities as well. There are systematic differences in the procedure of treatment of patients of
indigenous group.
Explain the role of Adam Goodes played in addressing the social determinants and therefore
improving health outcome for Aboriginal and Torres Strait Islander people
Adam Roy Goods is a former professional Australian rules footballer played in Sydney Swans in
respective Australian Football league. He is born on 8 January 1980 in Australia and known for
his community work and anti-racism advocacy (Harnett and Featherstone, 2020). He changed his
field and begin to start his outspokenness on various racial issues in order to enhance lifestyle of

indigenous peoples of Australia. There are various effective roles are played by the Adam Goods
to addressing the social determinants for the purpose of improving the healthcare of for
Aboriginal and Torres Strait Islander people in effective manner. Various campaigns are run by
him on social media in order to attract attention of healthcare workers on these areas as well as
launch various creative and supportive videos (Lovett, 2017). He enhances awareness of people
to maintain their health in effective manner and provide them knowledge to access various
factors that improve their life style. Adam Goods carry different health policies and medical
facilities as well in order to improve their health outcomes.

Cultural safe care is referred as the outcome which is based on the engagement that is recognised
as well as strived in order to address imbalances in healthcare system (Rennie, Thomas and
Wilson, 2019). There are various medical plans are formulated to the for Aboriginal and Torres
Strait Islander patients in order to improve their health as well as their status. All these plans and
policies are discussed below:
 As a registered nurse, I will suggest for Aboriginal and Torres Strait Islander patent to
maintain proper hygiene as well as take health diet with having exercise on regular bases.
 It is advised to patients to avoid bad habit as well as take various safety precautions in
order to remain safe care of themselves.
 It is advised to patients to take medicine according doctor’s prescription on time with
having essential diet which is suggested.
By taking these measures effectively and properly for Aboriginal and Torres Strait Islander
patients can maintain their healthy (Moodie, Maxwell and Rudolph, 2019).
CONCLUSION
As per the report, it can be concluded that social determinants of health is important factor
that affect the health as well as life style of people of for Aboriginal and Torres Strait Islander
people. There are various programmes and policies are conducted in order to remove all
discrimination and other factors that impacts on the lives of people. There are various change
leaders who plays important role in the improvement of these people health and other factors as
well.
