The family includes a single child named Richard who lives with his parents. They live in a two storied

he used to see deaths of his parents, in some he used to see blood all over and in some he used to see himself
falling from the top of a building. This condition was affecting his sleep and constantly deteriorating his

also. All these conditions began to engage him a year back after his grandmother passed. Richard’s parents

thoughts where they could not decide for whom to consult for the treatment of their child.

How does the service adapt its environment
and routines to provide inclusion and
support of children?
The child will be recovered from his current health condition by being
under the supervision of speech therapist and medical counsellor. These two
professionals will deal with the speech problems of the child and will help
him in getting sound sleep.
Suggest how these procedures and/or
practices can be made more specific to
ensure the inclusion of the child.
These practices and procedures can be enhanced by including the liked
activities of the child such as playing computer games in the treatment
tenure. The more the child can feel at home while being in the care sector,
faster will be the recovery of the child.
Additional resources or sources of
information to assist educators in
developing and adapting the service’s
curriculum to meet the child’s needs
No additional resources are required.

MONITORING AND REVIEW
Describe the progress of the child
Were the strategies in place able to meet the child’s
needs and address the learning barriers?
What is the impact of the strategies to the child?
Richard has been found to speak properly. Still sometimes, he has
been observed to stammer. However, it can be stated that he will
recover from his current condition eventually being in the care tenure.
Meditation has led to a great improvement in sleeping because of the
fact that this has made the child to relax more.
Issues of concern
What are the issues and/or difficulties you have
encountered during the implementation of the plan?
No specific difficulties were found to be associated while
implementation of the care plan because the child was cooperative and
well aware of the situation.
Barriers to implementation
What are the barriers that you have or you may
encounter while implanting the plan?
How will you respond to these barriers?
The barriers to implementation was communication because the
child had problems with speech, which made the therapists work hard
to understand his concerns apart from those reported by his parents.
Overall effectiveness of the plan
Do you think the plan and the strategies in place are
effective? Explain your answer.
Are there adjustments that should be made to the plan
and to the strategies already in place? Explain your
answer.
The plans and strategies are effective because of the fact that they
are directly associated with the cognitive and psychological wellbeing
of Richard.
Need for consultation with professionals and/or
specialists
Based on the child’s progress, is there an urgent need
to contact a professional and/or a specialist for
consultation and referral? Explain your answer.
No. Since this plan has successfully made Richard recover from
his condition.
