INTRODUCTION
Health and social care refers to a term which relates to a services that are provided by
health and social care providers to their patients. It is a term which refers the whole of the health
care provisions and provides the treatments for ill health and medical conditions within the
hospitals, community and health care centres (Read and et. al., 2017). This report is based upon
the individual's assessment in a health and social care settings which includes the range and
purpose of different forms of assessment and how partnerships supports the assessment process.
Present report also discusses about the early assessment, active participation of an individual in
shaping the assessment process also developing a care or support plan in collaboration with
individual that meets their needs. This report also includes developing other's understanding of
functions of rage of assessment tools and understanding how assessment may have positive and
negative impact on other's and their families. This report also includes the reviewing, evaluating
the outcomes and developing an action plan in order to address the findings.

1.1 Comparing and contrasting different forms of assessment
There are different forms of assessment and all these assessment serve with different
purpose during and after instructions. Range of assessment and their purpose can be compared

1.2 How partnership work can support assessment process
Partnership work can positively support the assessment process as partnering with

understanding of what type of a care an individual needs and also to ensure that these needs are
effectively met. Working in partnership can provide a lot of information about the individual and
thus accordingly organisation would be able to offer better service and build better picture of the
service users (Schulz and Eden, 2016). Partnership work will highlight the principles which
determine good practices such as Rights, Privacy, Emotional needs, confidentiality, valuing
people, etc. Working in partnerships with others can ensure that service users who are using the
health and social care service should be benefited. For example- information gathered from

friends, family or the people who may know the service users and this gathered information can
help in carrying out the assessment and developing a care plan in such a way that there is a better

assessment process but advocacy and support arrangements that are available can facilitate the
assessment process. So it can be said the effective assessment of an individual is considered as
crucial when individual's friends, family members, carers and various care professionals are
involved.

2.1 Initiating the early assessment for an individual
Initiating individual's early assessment can be done when there is a pre-admission of an
individual in healthcare organisation. Number of assessment of services users abilities and needs
are taken into consideration as a person centred. As a manager of health and social care
organisation one should look at- Hearing/speech/sight- if any individual has any kind of deformity with hearing, speech
and sight. Severe or complete loss comprehension- clearly understanding the needs of the
individual. Ability to make choices- Identifying how the service users want to end their day and agree
on daily life plan. Mobility- identifying the modes of mobility and handling the assessment sheets at the
time of admission immediately. Well being- checking whether a service user is suffering from anxiety or feels
restlessness. Oral care- checking the conditions of teeth and how he or she deals with dentures. Skin condition- making the proper and complete score risk assessment within four hours
of admission and if any risk is identified care plans should be formulated (Cesari and
et.al., 2016). Sleeping- one should ask the patient that if it has any difficulties with sleeping or is on
any sedations. Medication- Observe what are the effects of medication on individual and maintain the
record outcomes.

Pain- Ensure if service user is suffering from pain or not.
2.2 Supporting the active participation of individual in shaping the assessment process
Active participation and support of the individuals can help in shaping the process of
assessment and individuals should be supported and encouraged to maintain individuality and
self identity. Individuals should be encouraged to get engaged in the Care Plan Assessment
meetings, get integrated into the wider community and formulation of personal centred plan. All
individual should be encouraged to express their views, needs, preferences, etc. related to the
aspects of daily living which also includes the type of care which they receive within the
healthcare organisation (Baldwin, 2016). Career plans of the individual are person centred and
are typically formulated from the inputs of the individuals. Meeting of CPA are set for the
individuals and are invited to get engaged and also can discuss their own care package. Active
support can be provided to individuals within the care package for all the aspects of living.
Individuals should be supported and encouraged to become independent in order assess others
health professionals like dentists, general practitioner, nurses, opticians etc by taking the help
from the necessary liaise and staff advocate on the behalf of individuals. Also, individuals should
be provided with proper information and support in relation to care and treatments provided to
them which can help in assessment process.
2.3 Undertaking the assessment within the boundaries of own role
As a manager of social care organisation, i should be qualified and meet the requirements
of Care and Social Service Inspector Wales (CSIW) in order to work accordingly and to carry
out the service user's assessment accordingly. I should have the main focus of work on
undertaking the care assessment with the service users within the organisation. I should work
with several other service users in order to develop their care packages that is able to meet their
required outcome. I should accountable for all the responsibilities and should make contributions
towards the development of service users with a purpose to provide empowerment for vulnerable
individuals and promote other services to promote access to them. I should undertake timely
assessment at an appropriate level with the in depth information of service users which will help
me to determine individuals level of needs that line with the skills framework (Woodhead,
Northrop and Edelstein, 2016). After making the full assessment of each service users, I should
discuss the access to advice and specialist professional support and access should be provided to

specific services on the basis of service user needs including healthcare, physiotherapy and
nutrition.
2.4 Making recommendations to support the referral processes
Process of referrals is a systematic approach in order to help the service users with a
purpose to promote the well-being as well as enhancing the quality of care and self care. By
gaining the information about the individuals life stories during the process of planning, one can
ascertain that what may prove to be a positive stimulus for the individual and can help in the
referral process. Career plan requires an active participation as well as active engagement in
order to promote independence and well-being of the service users. While making the referrals,
one should identify following:
Resources or agencies that an individual may need.
What kind of a help that an individual might need.
Getting the individuals ready for referrals by provide the help in expressing their feelings
about asking help.
Individuals should also be referred to service of other professionals like speech therapy,
occupational therapy, sensory impairment, swallowing disorders etc.
It is important for a manager who is working with individuals needs referral to other
professionals for promoting their health and well-being.

3.1 Developing a support plan in collaboration with individuals
The proper way of developing a care or support plan in collaboration with individuals is
making a complete risk assessment and every care service organisation must involve in their
organisation (Career and Support Planning, 2019). Risk assessment can be considered as an
important tool for a manager in order to understand how to deal with the provisions of care
whether reviewing the care planning, manual handling, medicine distribution and general health
and safety requirements. While doing risk assessment, manager should look at the balance
between the possible benefits and harmful outcomes and also likelihood of various occurrences
within in a given timeline. In risk assessment there is an involvement of an activity of
information collection through observation, investigation and communication which is ongoing
process that requires skills and persistence to manage the relevant information which become
meaningful for service users as well as practitioner which are involved in delivering the support

One of the crucial element of care planning is to consider the individual within the
process and in order to achieve the best outcome a manager needs to make sure that there is the
full involvement of the individuals within the assessment. It is important for a manger to identify
the desired outcome for the individual and the process of development of individual care plan
should start with gathering the information from the key sources like family friends, current
service providers, social workers, general practitioners and other health professionals (Valentine,
Nembhard and Edmondson, 2015). Once the goals and objectives have been agreed, intervention
and support is considered and manager need to think how to implement the plan. Also, manager
need to be aware of on going collaboration with the service users on continuous basis so that
outcome of intervention are effectively achieved. Manager needs to make sure that health
packages are effective through out the lives of individuals and also services users are able to get
most out from the health packages which is designed for them. This will surely contribute to the
positive outcome for the individuals.

4.1 Developing other's understanding of the functions of various assessment tools
A manager should provide a good framework for the practices in order to ensure the
consistency for residents, one of the most important methods is to provide training and generate
awareness of the staff team. Providing the initial training is crucial for staff to understand not
only the process but the reason too. Also, the national minimum standards emphasises the needs
for authorisation and also emphasises that content of training should the ways of medication is
used, problems which may arise and various principles which have been informed on the policy
of management of medication (Kogan, Wilber and Mosqueda, 2016). There are various functions
of range of assessment tools which can improve the performance of other within healthcare
services. Range of assessment tools can increase the learning ability and skills of professionals,
careers, colleagues etc. which can help setting a proper bonding with the service users family
members and friends. It should be noted that one of the best way to provide support to service
use is through training and development. It is important to identify the difference between team
member's abilities and it developmental abilities so for this purpose manager have to audit the
skills and knowledge and supervise them in order to ensure that they they remain up to date with
their current roles.
4.2 Developing other's understanding which an assessment may have negative and positive
impact on the individual and on their families
Carer's families and service users have to play a critical role for the success for care
planning as well as the care management. Service users and carer's families and their
involvement has an integral part in each and every activity that are performed within the health
and social care sector. By involving them in sharing of information with respect to illness and
reasons of being there. Involving families may also have negative impact on the service users
health and well-being like family member of service user insist to shower over the service user
with hands as there are no rails in place which may increase high chance of getting infected. Also
family members may think that service user is not provided with proper hygiene care without
considering the health and safety factor (Orthwein, 2017). Another negative impact of getting
family members involved in the assessment is that it may make them feel bad or may develop
anxiety among the family members which are not strong hearted or are too emotional to handle

the situation. So, these are the positive and negative impact that assessment can have on
individual and their family members.
4.3 Developing other's understanding of their contribution within the assessment process
Entire staff have to attend the detailed care induction programmes and also their ongoing
processes should be evaluated through the process of regular monitoring and appraisals. It is also
compulsory for all staff to be provided with the annual training updates. Each and every care
worker should spend some time in gaining the experience from colleagues and also should be
supported by the management. Manager should encourage all the carers to undertake further
qualifications and whenever possible provide them opportunities for extending their studies. All
staff members should be continuously assessed during supervisions. Also carers play an
important role in providing the healthcare services to the people at care home. Manager of the
organisation should encourage that staff should carry out the health and assessment programmes
regularly on the service users and to report to the manager for any concern. Even staff should be
made aware about the legislations related to health and social care and what is their importance
for the staff as they have to work according to these regulations in order to avoid any mishap or
problems.

5.1 Reviewing the assessment process which are based on feedback from others and individual
Any health care organisation want to aspire practices which are set by CSSIW in
developing, implementing monitoring as well as reviewing the service delivery or care plans.
Care plans are formulated on the basis of individual and are implemented on daily basis through
out the staff that too in accordance with the guidelines which are interlinked with care plans
along the risk assessment (World Health Organization, 2015.). An organisation should monitor
the care plans daily in to ensure that care plans are maintained and sustainable. Some of the
legislations which are relevant to healthcare organisation in delivering the service or care plans
are Human Rights Act 1998, Data Protection Act 1998, Mental Health Act 1983, Care Standard
Act 2000, Mental Capacity Act 1983 and NHS and Community Care act 1990. So, these are the
acts through which assessment process can be reviewed. For reviewing outside agencies and
number of key professionals are invited to complete the service questionnaires which can enable
the organisation in maintaining high standards of care service.

5.2 Evaluation of the out comes of the assessment based on the feedback
On the basis of feedback from others and individuals there are some statements which
can help in improving the performance of the staff and service of the organisation. It has been
stated by General Practitioner that they are satisfied with services to service users and care is
appropriate (Thornicroft and et.al, 2016). Some of the family member of service users were not
satisfied with the services as felt think that staff were not able to communicate with the patients
who could not speak properly. District nurse stated that they are also satisfied with the services
and ways of providing treatment to patients.
5.3 Action Plan
Action plan provides a written summary of the issues and the goals, strategy for each
support, responsible person providing the support and the target date for completion. Based on
the findings from above feedback from some of the family member is as follows:

It can be concluded from the report that individual assessment plays an important role in

It can also be concluded that partnership work have a positive impact on the assessment
processes which can help in improving the performance of the health and social care
organisation. Present report also concludes that collaboration with individuals can help in
developing action plan and it is important for the organisation to take feedback from individual
and others in order to improve the performance of the organisation.
