An individualized support plan has been designed in order to support both the two clients with
disabilities.
For client 1 who is suffering from lower limb disability, the equipments that I will require
include canes, crutches, leg and thigh lifters, lift chairs and standing aids.
For client 2 who is suffering from eye sight issue, the equipments that I will require includes
large button telephones, speakers, canes, Braille and screen reading software.

While developing the individual plan I made it compulsory to incorporate the family members
and friends of the clients with disabilities. I planned a discussion session with the friends and

1st client: Mr. X possesses visual disability. First of all, I assessed his disability and then
provided him with Braille for conducting non verbal communication. I monitored the same on a
periodic basis.
2nd client: Mr. Y has got lower limb disability. For this, I will provide Mr. Y with a wheelchair. I
will also monitor his improvement with time in a weekly basis.
3rd client: Mr. Z has got mental disability. For this, I have introduced a councilor for him. I will
also monitor his improvement with time in a weekly basis.

I have undergone an in depth assessment to find out the need of the three individuals with
autism, dementia and post traumatic stress disorder. For evaluautaing their needs I spend 12
hours with them and communicated with their family members. After understanding their
requirements, I prioritized the same on the basis of their immediate nessesities.

In order to assess whether the issues of the three individuals has got addressed, I conducted a
weekly feedback system for monitoring their needs. On the basis of the feedback their needs are
evaluated and reviewed.
