INTRODUCTION
Integrated care is care planned with people that work together as a team to deliver
appropriate care and support to patient so that it can quickly overcome the health issue. In
another words, it is bringing together responsibilities for provision and commissioning for better
care and support to patient. This report has depicted information related to role of healthcare and
support services in person centred care, relationship with partnership working.

Role of health, care and support service in supporting person centred care
Role of healthcare profession and support services is to provide dignity, respect and
recognition to all patient so that it can be easily overcome from the health issue. Healthcare
professional and different individuals offer services to patient as per their personal needs and
health issue so that they can be treated in effective manner (Goodwin, Stein and Amelung, 2021).
Coordinate care, support and treatment is provided to the patient with personalised services for
its good health and well-being. The main role of healthcare is to understand what all key health
problem are face by patient and the alternative treatment, intervention that could be used to cure
it in best possible manner.
Process of different person-centred assessment in defining individual own care pathway
Healthcare firstly lead efforts to understand what all problem or symptom, health issue face
by patient, then determine the type or health problem it is suffering. Then finally take steps to
decide a particular course of action or intervention that could be used by them to treat and define
individual care pathway. Another method is physical examination, record of key issue face by
patient and finding intervention that could be used to provide appropriate care and support to
patient for its overall health and well-being.
Review challenges and benefits of planning person centred care in workplace
Likewise challenges in achieving personal centred care is agreeing the roles and
responsibilities that need to be performed by each healthcare professional so that the desired care
services can be render to patient (De Luc and et.al., 2018). Along with it providing personalised
care to individuals as per its health issue. Benefits of planning personal centred care is that
patient will be provided appropriate care and support for its for overall health and well-being.

Positive partnership working between multidisciplinary working team contribute in
delivering qualitative and personalised services, support and care to patient. Empowering each
individual to have authority and power to complete the task related to providing care and support
to patient is necessary. Theory of collaborative working can be used to develop strong
relationship and partnership with multidisciplinary working with different team member. In this
informed decision making and information sharing should be promoted so that all team members
feel important part of team (World Health Organization, 2017). Informed decision contributes in
developing a feeling of belongingness among the multidisciplinary team member that are
working together as team for delivering qualitative healthcare services to patient. Confidentiality
of each other personal information needs to be maintained, along with it each individual or
profession need to effectively perform its roles and responsibilities while working in
multidisciplinary team. It is responsibility of all team member to openly, clearly and accurately
share all the information with each other so that there are no chance of confusion and conflict in
future scenario (Archibald and Barnard, 2018). It is responsibility of multidisciplinary team
member to share information related to patient condition, treatment provide with each other so
that more proper treatment can be offered to it.

Communication is one of the important processes in meeting and reviewing care needs as
it helps in exchange of information from healthcare professional to patient. Likewise, verbal
communication is important as both written and oral communication through use of proper
language, words contribute patient in effective understanding of patient condition and type of
treatment need to be provided. It helps in clarifying the misunderstanding and providing missing
information so that right treatment can be offered to right patient in right manner. While written
communication is important as all the detailed related to review of health condition of patient is
note down on paper (Naylor, 2018). At the same time, oral communication contributes in
