Introduction
For the well being of the patients' health care and social care, the sector plays a very important
role. This social and health care sector aims at providing services, which are self-centred and
better and very well coordinated. The development of integrated care has been establishing for
twenty years but seems to be at a very slow rate. The health care system in the UK seems to be
the largest and the government of the UK predicts that the integrated social and health care
services will be present nationally by 2020. This assignment focuses on the support provided to
people regarding health and social care services. It concerns the requirements and needs of an
individual along with it show the relationship that develops between the multidisciplinary team
members and an individual. The assignment also shows the needs and acts to be taken for a
person-centred plan. Analysis of areas and regions of people who require anonymous care and
attention and hence the role of social and health care service providing professionals to deal
with such people and support them in any major issue. Providing local sources and its
availability and provisions in order to support the people in want of multidisciplinary care along
with it recognizing the needs of the patients. Task 3 of the assignment describes the leadership
qualities required to support the individuals in need along with this is the teamwork and
coordination expected by the social and health care professional task 4 describes the hourly
need of person-centred communication while implementing the plans for person centre.

The care needs for an individual who is suffering from any particular disease or syndrome need
proper personal care and assistance in order to fulfil his needs and requirements of living. The
individual who needs should realise and understand that there are resources, which are
available to support him and his requirements (Lawn and Schoo., 2010). Once the needs are
identified in an appropriate manner it becomes easy to integrate it between the social and
health care services, which are required. The requirements might include pain management,
personal hygiene, continuous care, proper nutrition and confidentially. Now individuals are
more concerned and bothered about their care needs as earlier the health care professionals
had various assessments on recognising the needs of individuals and work on serving them but
now the needs and requirements are well identified by the individuals itself and explain the
professional better on what they need urgently (Wilkins., 2006). As mentioned in the provided
case history of Jonathan Tertullian who is an eighty-year-old man and is suffering from heart
disease and dementia along with high blood pressure who often forgets to take his medicines
and has a weak left side, he has isolated himself and there seems to be no personal hygiene
maintained, the need of this hour is that the social and health care professionals provide him
with full-time personal assistant who can take care of him his meals nutrition and medication
also with this his personal hygiene is to be maintained by regular bath cleanliness of the house
and other belongings. Also, he can be taken to local near by parks and made to attend group
exercises for his physical health and also this will help him socializing with people and kids in
the park this will give him new hopes and his recovery can be early (Nancarrow et al., 2013).

social status. As now individuals have started their own needs they demand resources and
requirements on their interest of care and supply. The supply mainly refers to the services,
which are been provided by the health care professionals to the individual in need. The whole
process of identification of needs of the individual by the health care professional or by the
individual itself depends on his experience (Posavac., 2015), his knowledge regarding the
disease or situation, and it also differs person to person and also on different health care
professional. Hence there should a proper and systemic recognition and assessment of the
demands and wants of the people and their unmet needs and it should within the ethical,
economic and clinical aspects.

The health and social care services which are used by the individuals they are provided to be
equally involved in the services provided to them by involving them into the planning process of
developing and managing their unmet needs and requirements. And this whole is a process of
systemic person-centred care. And in this process (Brooker and Latham., 2015), it is made sure
that along with the individual their family is also involved putting them as the central and most
important part so that they are also a part of health care planning and the requirements for
them are fulfilled. This approach by the social and health care sector helps the family and
individual to follow their own believes, social and economic circumstances while planning
solutions to the health care problems of the individual. As in the provided case history of
Jonathan Tertullian if we the person-centred care plan is adopted it becomes better for
Jonathan as he himself can make sure what he wants and his needs to be fulfilled by social and
health care professionals (Kindell, Sage and Cruice., 2015).
As in the UK, the demands for the services of health and social care are increasing; it shows the
growing priority of person-centred care. As the life span is predicted to increase by the recent
reports the health and social care services demands comes more prior in society. Hence the
relationship between the social and health care professional and the individual seem to be

values and beliefs well and keep in mind the basic concern and demands of the patient while

It is considered more reliable when it comes to person-centred care as it is seen that with the

better and also the pressure on the social and health care individual is less to meet the
demands of the patients as the decisions are made by mutual consultation. The most important
job of the health care professional is to maintain the care planning in within the ethics and
social and economical decorum of the social and health care services, which are provided by

satisfaction of the health care professional making them less exhausted and getting them the
feel of goal accomplishments.

handle the welfare of the individual who is in need this. Leadership qualities and proper
implementations of various responsibilities to an appropriate member can only help in better
functioning of multidisciplinary care (Mitchell and Golden., 2012). While working in a
multidisciplinary team there are various responsibilities heading to leadership for a health and
social care professional worker. As the leadership is supposed to be taken by the health care
individual on the behalf of both service seekers and service providers this done to keep in mind
the needs of them and give them equal satisfaction. It is said that the most important role
played by a health care individual is to identify the needs of the service seekers as once the
requirements are known leading the offered service in favour of the service users by making a
care plan becomes easier (Mclnnes et al., 2015). These identified needs are further reported to
the care manager that decides the care planning n assures that all the services, which are to be
provided, are under national standards and legislation. It is necessary that the care manager
should monitor the needs and take proper care of mentally disabled patients. For example in
the mentioned case history of Jonathan, the social health and care manager needs to give
attention whether the provided services are set under the guidelines of the policies and
legislation made by the senior authorities like NHS or CQC. It required for the health care
manager provide concern and care to the individual seeking services keeping the record of his
proper physical and mental health (Cameron, Lart, Bostock and Coomber., 2012). As we see in
the case of Jonathan where he is suffering from dementia and he forgets things easily.
Another important role of a healthcare professional stating his leadership qualities is keeping
records on all the patients whether their demands are fulfilled or not along with the rate of
their recovery. It is important to maintain the sharing of information between the health and

should be explained to the family members of the patient so that they well informed about the

The term Communication is defined as the transfer of information from one individual to
another. In this case, communication is very important to be taken place between the team
members of the multidisciplinary care, or the communication between the health care
professional and the patient or the patients relative (Jonassen and Grabowski., 2012),. The
different methods of communication are to be worked on as well as various strategies due to
which communication becomes easier to understand and convey. In the social and health care
sector, communication is vital as there are different types of information, which are to be
conveyed to the patents and their family members. Various methods of communication are
discussed as:
Written communication: the contact between the patient, their relative and doctor can also be
in the written form, the doctor maintains a written record of the patients' health and problems
and provides them with prescription of medication (Palinkas et al., 2015). The written form of
communication can also be used as in case of Jonathan where he has dementia and he forgets
to take the medicines, written notes with timing can be provided by the health care assistant so
that it becomes helpful for him to memorize and take the medicines on time.
Verbal communication: the quick transfer method of information is by verbal communication.
The communication verbally should be maintained between the health care workers,
professionals, doctors and patients (American Psychiatric Association., 2013). It is the best
means in case of an emergency. Also, verbal communication makes it easier to convey the
thoughts and information with detail discussion is required.
Formal communication: the transmission of information between the superior authorities and

Questionnaire: in order to identify the needs and requirements of the patients, health care
professionals try to ask several questions to the patients and their family members. They try to
then do the care planning and working according to the needs of the patients; hence it
becomes easier to provide them with an expected better quality of services. As in the case of
Jonathan (Moorhead et al., 2013), various questions are required to be asked by the health care
professional to the patient and his family as Jonathan is a heart patient and he is also suffering
from forgetfulness.

towards the health care officials (Kreuter, Farrell, Olevitch and Brennan., 2013).
Solicit relevant psychosocial contexts: a friendly and healthy environment is expected to be
created by the health care professional while they have a conversation with the patient and
their relatives so that they feel easy and comfortable to share their problems may it be related

Offer personalized and compassionate reassurance and care: as the patients and his patients
are itself into a dilemma of improper health of the patient. The doctor should discuss each and
every process and treatment planning with the patient and his family, answering all the choirs
that patients have by the doctor will help to build up trust and confidence in the patient
towards the doctor (Bowling., 2014). This personal attention of the doctor towards the patients
helps in building a better professional bond between them. Seeing the passion of the doctor
towards his work and patients health, they keep a check on the regular recovery charts of the
patients.

This assignment concludes that all individual suffering for some or the other illness needs the
support and care provided by social and health care centre which is multidisciplinary care.
Through National Health Service that is NHS health and social services are provided to every
individual in the UK. The health care officials require various information about the patients
that, is taken by the patient and his family members in order to identify the needs and
requirements of the patients. The main aim of multidisciplinary care is to serve people when
they need it the most and satisfy their expectation. Various strategies should be worked on
