Introduction
Integrated care provided by health, care and service (HCS) providers allow the individuals
operating in the sector to perform their job role effectively in ensuring the care plan designed for
patients are effective for their treatment. Integrated approach to treating individuals is beneficial
in the health and care setting as it provides necessary care for the patients to recuperate better
physically and mentally.
The following study develops a brief understanding of the wide variety of multidisciplinary care
presented to patients that would gain the most benefit from these services. The study identifies a
required skills set and personal capacity that would provide the individuals with the opportunity
to provide better services. Personal communication and relationship with the team members and
users of service would be evaluated in the study.

Introduction
Integrated care is the best available option for individuals requiring multidisciplinary care in the
health sector of any region. The study will identify the aspects of multidisciplinary services
available for individual users in the locality of Newham.
P1 Outline local resources and provision that supports integrated care working
Integrated care working refers to the communication and inter-disciplinary tactics of work
related to the health and service-providing actions that enable the local community members to
attain better services from the HCS operators of the region (McCormack and McCance, 2016).
The local resources available to Newham locals in receiving integrated care are acknowledged as
follows:
 Community involvement through regular checkups for senile and vulnerable individuals
in the society ensures that the governing bodies are aware of possible patients and
develop local medical sources appropriately (Novitzky, 2016).
 Interaction between multidisciplinary care providing bodies in the Newham locality
allow the development of local healthcare bodies to identify the possible methods that
would prove beneficial for the residents of Newham (Feo and Kitson, 2016).
 The integrated performance of Clinical Commissioning Groups (CCG) functioning with
the regular health service providing bodies in the locality allow the residents to gain
support form a wide variety of HCS providers related to their health plans.
P2 Describe current local unmet need related to health-care and support service provision
in own locale
It is recognised through the report generated by newham.gov.uk (2019), that the locality
experiences efficient healthcare facilities relevant to the youth. There is a lack of integrated
practice schemes for individuals with disability and dementia to receive an integrated form of
treatment from the available facilities within the Newham locality. The region experiences a
growing surge in demographics as well as the increase in number of aged individuals. There have

been reports of growing disparity in the quality of services provided by the HCS professionals
towards disabled and dementia patients.

Thus, it can be observed from the figure above and the reports generated by the CCG reports that
the region experiences a lack of proper integrated and multidisciplinary treatment opportunities
for the disabled and the senile citizens of the region. The growing need for integrated support
services from the healthcare services of Newham proves that there is a shortfall of effective
services within the locality (Brimblecombe et al., 2017). Integrated support in providing health,
service and care to citizens through the application of multidisciplinary activities in the region is
beneficial for improving the situation within the Newham locality.
M1 Explain the difference between healthcare and social care providers and types of
interagency care provision in relation to meeting the needs of the individual requiring care
The variation in duties performed by the ‘healthcare providers’ and the ‘social care providers’
can be established through the differing purposes that these schemes provide to the population of
the country according to plans designed by the NHS. As per the guidelines framed by the NHS
health and social care facilities, required by individuals are different:
Healthcare needs of individuals consist of providing treatment, preventing the spread or
continuation of an illness as well as the control of medical issues that render a person sick,
disabled or injured in the general context. Social care parameters, in contrast, refer to the

activities and support that individuals receive in order to be deemed as productive members of
the society (continuing-healthcare.co.uk, 2019).
The type of job role fulfilled by individuals operating in the healthcare sector is different from
the social care sector due to the difference in core functional abilities performed by them.

The NHS in the UK HCS sector provides a variety of interagency care provisions for the service
users. In the health and social care operations provided by the individuals to Newham locality,
the interagency care provisions include collaboration, coordination, integration, mergers and
partnerships (Winters et al., 2016). These variations of care provisions enhance the service
quality availed by the users within the society.
Conclusion
It is observed through the study that there are multiple local resources available within Newham

P4 Describe the role of the health, care or support service practitioner in supporting
person-centred care
The role of the professionals functioning in the HCS sector need to identify the specific needs of
the patients in order to provide them with effective treatment is identified through the type of
treatment the patients receive from the institute. It is the duty of the health, support and care
professionals to develop a friendly relationship with the users in order to generate better
treatment plans for the patient. Understanding the behavioural traits of patients enable, the
professionals to develop an appropriate treatment plan for the patient through multidisciplinary
facets of service.
M2 Explain own involvement in different person-centred assessments used to define an
individual’s care pathway
Ability of service providers to generate proper treatment plans for the patients according to the
care pathway designated for the user. It is essential for me to maintain proper relations with the
patient in order to gain their trust and develop an accurate understanding ability of the patients’
demands. In the process of creating a person-centred assessment for an individual’s care
pathway, I would have to recognise their thought procedures and issues that they experience. I
would require developing a better understanding of the ailments that the patient experiences in
order to prescribe an effective care pathway relevant to their scenario.

Conclusion
The need for identifying an individual’s ability in taking decisions relevant to their care plan
provides the operators with the opportunity to design effective treatment methods. Development
of person-centred care and relevant care pathway is proved beneficial in the HCS sector as it
provides the users with the opportunity to received customised treatment plans.

P5 Provide appropriate leadership within the remit of own role in a health care or support
service to promote effective inter-professional and multidisciplinary team working
In order to provide the required support to patients suffering from dementia, the foremost thing
that needs to be practiced within the care centres is to segment the patients according to their
diseases. It is essential for the practitioners to know the disease that every individual within the
health care centre is having, which can help them in assessing the need of support for every
patient. In this context, while working for Shrewsbury Road Health Centre, it is understood that
the relationship among the employees or the health care practitioners was extremely professional
and this has resulted in limiting the patients in receiving the emergency support. Therefore, it is
necessary for the team members in collaborating with their activities, which can help in
determining the support that is already provided to the patients, which can help in delivering the
future support appropriately.
Multidisciplinary teams are allowed to treat their patients independently (Smith et al., 2018).
This is required to be changed in order to create better understanding of the care that is required
by the patient. On the other hand, proper collaboration within the teams of a health care centres
can help the professionals in monitoring the health condition of the patients in a keen manner.
With the help of this, the practitioners can understand the impact of support, which can help the
professional in developing better strategies in care provide to the patients. Following
transactional and democratic leadership within the care centres can help the professional to
work effectively for the development of health condition of the patients. The democratic
leadership will allow in receiving information and suggestions from all the available teams
(Walshe et al., 2016). On the other hand, the transactional leadership will allow the professionals
to perform in a better manner, in order to receive the rewards that are promised by the authority.
P6 Describe the responsibilities of information sharing between multidisciplinary teams
Information that is related to patients is the main element that is required to be considered in
order to provide the needed support to their health conditions (Kerstenetzky et al., 2018). It is
duty of the professional within a healthcare centre to pass the information to every department of

the setting in order to ensure that the medical practitioners are aware of the health conditions of
the patients. The other reason for information share within the teams is to ensure that the needed
information is passed to the patients and their family in the needed manner. Another reason for
information sharing within the multidisciplinary teams of health care is to create achievement of
the set goals and objectives of the care setting.
While working for the Shrewsbury Road Health Centre, I understood that the patient during
night was affected by fever. This can crate severe effect on the health condition of the patient.
Additionally, because of such incident, there can even be a change in the medicine patterns,
which can only be done if this information is passed on to the senior doctor teams. Furthermore,
this information will even allow the nurses to provide the needed medical support when the
patient suffers further from this feverish condition.
M3 Provide competent and autonomous leadership in information sharing within a
multidisciplinary team in own setting towards meeting different individual’s care needs
Flow of authority plays the most essential role in the development of health condition of every
individual. It is even relevant for the practitioners in Shrewsbury Road Health Centre to follow
autonomous leadership while sharing the information and this can help the care setting in
developing proper information base of their patients. The Health Centre with the help of
autonomous can understand a patient’s condition from various angles and this can help them in
determining the needed medical support to its patients. In a healthcare setting, there are several
patients, with different medical urgencies, which can be assessed with autonomous information
sharing.
Patients suffering from dementia are highly prone to self-harm because of their disability to
identify their actions (Intellectualdisability.info, 2019). The patient needs monitoring from the
side of every professional within the centre, which can help the authority in assessing the change
in behavioural pattern that the patient is going through. It can further help in determining the
daily activities that the patient must follow to receive betterment in his or her health condition.
Within a healthcare centre, there are various teams, and with the help of autonomous leadership
in information sharing, the families of the patients can receive the needed information related to
their patents. This can further guide them in dealing with the patients accordingly.
