Introduction
Integrated care provided by health, care and service (HCS) providers allow the individuals
operating in the sector to perform their job role effectively in ensuring the care plan designed for
patients are effective for their treatment. Integrated approach to treating individuals is beneficial
in the health and care setting as it provides necessary care for the patients to recuperate better
physically and mentally.
The following study develops a brief understanding of the wide variety of multidisciplinary care
presented to patients that would gain the most benefit from these services. The study identifies a
required skills set and personal capacity that would provide the individuals with the opportunity
to provide better services. Personal communication and relationship with the team members and
users of service would be evaluated in the study.

Integrated care is the best available option for individuals requiring multidisciplinary care in the
health sector of any region. The study will identify the aspects of multidisciplinary services
available for individual users in the locality of Newham.
P1 Outline local resources and provision that supports integrated care working
Integrated care working refers to the communication and inter-disciplinary tactics of work
related to the health and service-providing actions that enable the local community members to
attain better services from the HCS operators of the region (McCormack and McCance, 2016).

Thus, it can be observed from the figure above and the reports generated by the CCG reports that
the region experiences a lack of proper integrated and multidisciplinary treatment opportunities
for the disabled and the senile citizens of the region. The growing need for integrated support
services from the healthcare services of Newham proves that there is a shortfall of effective
services within the locality (Brimblecombe et al., 2017). Integrated support in providing health,
service and care to citizens through the application of multidisciplinary activities in the region is

that needs to be practiced within the care centres is to segment the patients according to their
diseases. It is essential for the practitioners to know the disease that every individual within the
health care centre is having, which can help them in assessing the need of support for every
patient. In this context, while working for Shrewsbury Road Health Centre, it is understood that
the relationship among the employees or the health care practitioners was extremely professional
and this has resulted in limiting the patients in receiving the emergency support. Therefore, it is
necessary for the team members in collaborating with their activities, which can help in
determining the support that is already provided to the patients, which can help in delivering the
future support appropriately.
Multidisciplinary teams are allowed to treat their patients independently (Smith et al., 2018).
This is required to be changed in order to create better understanding of the care that is required
by the patient. On the other hand, proper collaboration within the teams of a health care centres
can help the professionals in monitoring the health condition of the patients in a keen manner.

patients, with different medical urgencies, which can be assessed with autonomous information
sharing.
Patients suffering from dementia are highly prone to self-harm because of their disability to

side of every professional within the centre, which can help the authority in assessing the change
in behavioural pattern that the patient is going through. It can further help in determining the
daily activities that the patient must follow to receive betterment in his or her health condition.

in information sharing, the families of the patients can receive the needed information related to
their patents. This can further guide them in dealing with the patients accordingly.
