Introduction
The adult service provider is defined as a service of care that is given to an adult person in a
specific community. It can be an individual or an organisation, which is associated with
providing proper care and support to the adult individuals. It focuses on identifying the problems,
complications, need of an individual, and provides respective care and support for preventing
their problems. Here, the need of an eighty-year-old black British man named Tertullian, will be
described along with the services that will provide to him.

The National Health Service (NHS) is one of the most important health care systems in the local
community of United Kingdom, and is the most available resources that provide essential health
care support to the individuals. It is associated with providing complete health care to the citizen
of UK, according to their needs. This organisation is funded by taxes. It deals with the day-to-
day care, in practice care, providing treatment and medications, which are required to control the
disease, illness and disability of the citizen. It also focuses on long-term health care (Social care
and support guide, 2018).
Community and social care service is an important local resource for providing social care to the
individuals. It is associated with providing service to that individual, who needs extra care. It
focuses on providing proper assistance to them for their daily life activity. Social care is
associated with providing service about feeding, dressing, washing, using toilet and mobility. It
focuses on providing a proper company to them to improve their mental and social status (Sallis
et al., 2016).
Describe local unmet need related to healthcare provision in own locale
There are several needs within the concerned individual, which have not met properly. Here, the
British man Tertullien, lived alone and felt isolated always. As he had no family members
nearby, he always wanted a companion. Leading such a life was responsible to provide a great
negative impact on his mind. It affected his psychological status. The main thing, which he
required the most, was communicating and sharing his problems, complications and needs with
someone.
His physical condition was not in good condition as he had been suffering from cardio vascular
disease and hypertension. He developed paralysis in his left body parts due to stroke. This
incident made him unable to do any work effectively; thereby, forcing him to walk with the
support a walking stick. His physical condition was one of the significant causes of his disability
(Westwood et al., 2015). Hence, he needed proper care, which would help him in developing his
physical condition. The care that he had needed includes, providing proper medications in the

required time, preparing foods for him, considering his diet, proper counselling, to take him to
his physician for his treatment, having proper care to him, which will help in improving his
physical condition.
Explain the difference between health and social care providers and types of interagency
care provision about meeting the needs of the individual requiring care
The differences between Health and social care providers are discussed briefly in below:
Health care is associated with the treatment, taking of care and aftercare of individual, who have
developed an illness or disease or injury or disability. Whereas, social care, is associated with the
assistance of daily living (Baldwin, 2016). It focuses on social interaction, maintenance of
independence and it associated with providing supportive accommodation. The health care needs
focus on those needs, which have a relation with treatment, prevention and control of disease or
illness or disability and injury. This need can be met by providing proper advice from experts,
proper treatment by the doctors and supported health services by nurse and health care provider
(Alley et al., 2016). The need for social care is associated with the needs for day-to-day
activities. This need can be met by a social care setting.
Interagency is the collaboration of housing, health and social care organisation, which help the
agencies to work together for addressing the needs effectively. The type of interagency used to
provide support to the individual is the cross-sector service provision. This is an independent and
interconnected sector, which is working together. It helps to meet the need of the individual in a
good manner and to improve the quality of provided service (Glasby, 2017).
Evaluate local resources/provision in terms of meeting the needs of an identified individual
requiring multidisciplinary care
Tertullian needed for proper support that could cover his feeding, dressing, mobility and toilet.
As he has been suffering from cardio vascular disease, he required proper routine checkups and
medication. He had developed paralysis in the left side of his body; hence, it made him unable to
walk properly. Hence he required a person for supporting him during his movement to reduce the
risk of his fallen down. He had no family members and friends nearby. Thus, he required a
person to whom he could communicate and enabled him to share his thoughts.

The service provider helped to meet his physical, mental and social need. Support met
completely. It helped him in his treatment and associated with providing him proper medications
in respective time. It focused on several other tasks, which include preparation of his meal, dress
and toilet. It provided proper support to him by providing him with a moral support by
communicating with him in a lovable and respectable way. It was associated with the
improvement of his mental condition. Communicating with him, on different topic helped him to
divert his mind, and hence helped him to minimise his stress. It had associated with providing
him with proper care and support that could meet his need and satisfaction level. The
unexpressed needs for Tertullien not met properly as he was not aware of this need. The service
provider could not able to prevent that complication that was not addressed properly due to
improper communication. It was not possible to communicate with him all the time. The care
provider should perform several other tasks and hence are not available for all the time.

Conclusion
From the above report, it can be concluded that health and social care service has a great
importance in proving proper care and support to the individual, who have an urgent need. Here,
the need of the eighty-year-old black British man, named Tertullian has discussed who had
urgent need to prevent his physical and mental stress. The adult service provider identified the
need of the old person. Provide according support shows significant role in preventing his
complication. The proper care and support of the service provider helped him to meet his need
and his satisfaction level.
