Introduction
Multidisciplinary or integrated care provides the concept of collaboration of workers
when the professionals try to give service or treatment to the users of their hospitals. As
team of professionals gives their concentration to provide support to the user, it assists
to get effective outcome. In order to conduct the study, Brent borough of London will
be selected. The health practitioner in charge of Brent Council will examine local
resources and provision (brent.gov.uk, 2019). After that, support services in
multidisciplinary care will be examined in an effective manner. Assessment of individual
needs and development of care plan will also be included in this study. At the end of the
paper, importance of person-cantered communication will be discussed for competing
the study in a lucrative manner.

LO1
Introduction
The study will focus on the local provision and recourses that support the integrated
care planning. In addition to this, the healthcare practitioner in charge of Brent Council
will examine unmet needs of people of Brent borough in London. Various types of
interagency care provision for individual needs will be negotiated in this paper.
P1 Local provision and resources
Local community resources, which supports integrated care
As per the viewpoint of Minkman et al. (2013), integrated care indicates that
professionals can discuss among themselves in time of giving support to the users of
their care home. In order to provide support, various recourses in Brent borough such
as integrated care related videos, proper leadership are required so that care providers
give best service to the user in the contemporary era.
Effective leadership in healthcare and collaboration
As mentioned by Brown and Menec (2018), if leaders play the effective role in the
venture, they can teach members about the job role properly. In addition to this, in
healthcare, different departments such as dementia suffer people, autism suffer people,
are there, and the leaders are different. Thus, in community-based services, NHS
(National Health Service) instructs that organisations need to maintain proper
leadership style so that junior can learn from the senior care providers. For example, the
healthcare practitioner in charge of Brent Council notices that if junior providers assist
the senior one in time of giving service to user, they are able to increase their
knowledge on integrated care service. It indicates the nature of collaboration in the
venture. Junior support providers apply this knowledge on young people and children in

Strengthen the relationship with system
In order to outline the resources of community based local service, NHS tries to train the
care providers as they can give best service to the users. In this context, National
Lottery Community Fund and NHS jointly offer free workshop and online coaching to the
care providers so that they can bring improvement in their service quality
(england.nhs.uk, 2019). It is observed by the health practitioner in charge of Brent
Council that proper training related to integrated care brings improvement in the lives of
users. As it is found out that workers of Brent Council provides nursing service for
children, attending workshop and online coaching, nurses are able to increase their
knowledge in this sector.
The above-mentioned resources help the integrated care providers to develop their
quality of giving service in an effective manner.
Local community provision, which supports integrated care

In order to give proper service to the users, Adwa et al. (2016) suggested that care
providers need to follow few provision such as giving high quality care. It helps the
users to recover as early as possible.
High quality care
In the opinion of Adwa et al. (2016), support workers need to give attention to the
workers so that they can fulfil the needs of users. In addition to this, many a times, the
healthcare practitioner in charge of Brent Council that support users are not able to
communicate with the workers, as they do not give attention. Hence, following Care Act
2014, Clinical Commissioning Group (CCG) focuses on the care quality as people can

provide the best service to them. It helps to uplift the health standard of people in this
area. In addition to this, pre-school provision is included in this borough so that
childcare option is available.
P2 Current unmet needs based on the healthcare provision
In time of focusing on the needs, it is observed that in Brent borough, health community
focuses on the needs of the children. They mainly cover the health status of children
and their educational policy. As mentioned by O'hara et al. (2016), many a times it is
noticed that children are suffering learning disability and in this situation, they need
support so that they can recover from this situation in an appropriate manner.

As per the local provision of Brent borough, it is found out that care providers need to
follow the requirement of users so that they can give best service to care users. As per
the report of government, it is analysed that around 4% children are suffering from
learning disability and it is necessary for the health community to solve this issue. In
time of focusing on the adults, around 7.5% people are suffering from above-mentioned
need (brent.gov.uk, 2019). It clearly indicates that support in learning for children is not
given. Hence, it can be taken as the unmet need in the area of Brent borough.
For example, in order to give service to the child of 12 years old, who has learning
disability, which can be specifically told as dyslexia, it is necessary to identify her needs
for giving her proper services.

The multidisciplinary team tries to focus on the needs of the users, based on which they
can design their plan. As mentioned by Housley (2017), in the team, as many
experienced practitioners are there, they are able to draw the effective plan related to
the service for the users. In addition to this, in time of applying the plan, support workers
need to analyse the history of the users so that effective service can be given. For
example, in case of a child of 12 years old, it is necessary to provide support so that she
can learn the basic education as early as possible. In the multidisciplinary team, gp
(general practitioner), nurses, psychologists are there. In this context, psychologist tries
to communicate with child in different manner so that the child feels safe in the care
home and improve quickly. Based on the issues finding by psychologist, GP can refer
proper medicine and activity for her welfare. Therefore, the main function of
multidisciplinary team is to give support to the users so that they can lead a healthy life.
Difference between social care and health providers and types of interagency
care provision
Types of interagency care
There are different types in interagency care such as partnership approach, merger
approach and many others. As commented by He (2015), when healthcare
organisations follow the partnership approach, they are able to fulfil the needs of people
in different aspect such as learning disability, autism and many others. Therefore, it can
be stated that in interagency care, many experts are there, who are able to give
appropriate suggestion to the users in the Brent borough.

In time of analysing the needs of the individual, it is necessary to provide the
interagency care as in this care provision, many experts are there such as general
practitioner, psychologist and many others (Reiss et al. 2017). In Brent borough, a 12
years old child is suffering from dyslexia and it is necessary to meet her needs. As Brent
borough, health council tries to meet the needs and for the girl, they can follow
interagency care. In this care system, schoolteacher is also included. If they follow the
interagency care provision, the child can recover as early as possible as teacher will be
able to provide her appropriate support. It clearly indicates that interagency care
provision is stronger and effective than health care providers.
Conclusion
At the end of the study, it can be deduced that multidisciplinary team helps the users to
recover quickly so that they can lead a healthy life. Health practitioner in charge of Brent
borough tries to analyse the provision of the community so that people are able to bring
improvement in their health status.

Care practitioner of Northwick Park Hospital finds out that Flora Smith, a 12 years old
girl, is suffering from dyslexia and she does not have allergy in any kind of medicine
(lnwh.nhs.uk 2019). As mentioned by Reiss et al. (2017), in time of giving support or
creating care plan, care providers need to follow the health status of the user. In case of
Flora, it is necessary to mix with her so that she wants to hear the words of care
provider and it helps her to recover as early as possible.
P4 Role of health and social care support providers
Health and social care service providers are having integral role in mitigating the
diseases that are suffered by the individual. Health Practitioners of Brent Council are
found to deal with the children learning disabilities. The number of learning disabilities
are found to increase in UK Hence, the roles of these personnel in Brent council are
provided below that would help in dealing with this disease effectively:
 Healthcare professionals need to understand the behaviours of the children to
deal with the clinical condition. Moreover, the staffs need to be calm as well
supportive in every situation in order to motivate them patients. The
communication among healthcare staffs and patients of learning disability need
to be clear and should be assisted with effective tool and resources for gaining
best outcome. In addition to this, primary care should be given to these patients
along with proper health services. This will help in improving the quality of life of
these people (Alfonso and Flanagan, 2018).

 Strengthening the individual rights of learning disable patients is the role of
health practitioner and should be reviewed properly to provide safeguard. The
primary care staffs of Brent Council should be able to treat these people in a
motivated manner. The service provider should understand the challenging
behaviour and carefully provide prescribed anti-psychotic medication. Moreover,
health practitioners and support users need to monitor the prescribed medicines
regularly in order to use them appropriately.
 As mentioned by Fletcher and Miciak (2017), learning disability providers need
to help these diseases oriented children to learn skills. Hence, they should be
guided to lead an independent and healthy life. Health practitioners should be
able to understand the needs and demands of the patients by communicating in
their styles. For example, providing the child support and encouragement to
learn new skills such as self-management and independent living skills can help
in overcoming with this health disease.
 Service provider should be able to provide the child with effective diet chart that
should be followed by the affected person daily. They should be able to provide
them essential services, which is the primary aim for every healthcare approach.
Own involvement in person-centered assessment
As a health service provider, I would help the person with learning disability in the
following ways:
 I would treat the patients with special care by maintaining the principles of
Disabilities Discrimination Act, 1995 (nidirect.gov.uk, 2019). Moreover, I will
use strategies for developing the skills of the children so that they do not feel
alone. Healthcare services such as identification of needs, understanding of
personal behaviour are important to handle the patient effectively.
 As the patient is suffering from Dyslexia, I would prefer to improve the memory
and catching power of the patient. This will help in making significant
improvement in her memory to learn new things.
 I would prefer in treating with the patient with physiological testing in order build
the academic skills. Based on the study of Esan et al. (2015), treatment of a
learning disable patient starts at an early age, hence, I would prefer to enhance
their intellectual skills so that the individual is able to understand the words and
letters in a proper way.
 Moreover, I will check the medical history of the patient and conduct neurological
exams for the affect patient in order to evaluate and assess the psychological
state. Hence, this would help in understanding the learner difficulties that is
important for making the schedule in an order manner.
 I would conduct playful teaching games and arrange physical activities so that
they can bring improvement in their health. Creative activities such as visual art
and sports should be arranged for the person so that they can increase the
memory power and nervous system.

P5 Appropriate leadership in healthcare to promote effective team working
Leadership plays an important role in promoting the effectiveness of the healthcare
service. In context of learning disability patient, it is important, analyse the team working
activities so that the patient can overcome the disease. For example, in treating with this
disease, group of professionals such as health service providers, support providers,
educationist and general assistance need to work collaboratively where leadership
plays an important role. Hence, it can be said that autocratic leadership skills is highly
required. Communication is one of the major skills that are required for communication
with all the personnel for bringing improvement in health of the patient.

The above figure has shown that selfless service, critical thinking, teamwork, integrity,
and emotional intelligence are highly required to deal with patient of learning disability. It
has been found that in Brent Council healthcare administrator, managers discuss with
the special educationalist, teachers, and other associates so that proper coordination in
treatment care is implemented. It has been evident that leadership helps the healthcare
managers and support providers in understanding the priorities works, which are
necessary for making, changes in the operations. For example, educational activities
and prescribed medicines should be given consecutively, hence mutual relationship and
teamwork management is largely required in this context.
P6 Responsibilities related to information sharing between multidisciplinary team
Developing multidisciplinary team in Brent Borough Council is required for service
different services in an order manner. This helps in improving the health outcomes and
increases the satisfaction level of the client. In case of treating with the patient of
learning disability, it is important for understand their level of decision. In most case, it
has been found that multidisciplinary teams for treating with this learning disease
consists of advocate, parents, general educationalist, special education staff, and
medical provider. All these team members are having unique role. As mentioned by
Pullen et al. (2017), the key function of developing multidisciplinary team is to collect
strength type and affect area of disease effectively. Hence, collaboration skills are
highly required in order to make effective decisions in treatment process.

P8 Application of effective communication strategies
To meet the need of the different segments of service users in health, care and support
services, the major strategy would be the visual communication.
Visual communication is defined as the use of graphical presentation in communicating
with the required information with one another. Most of the communication of the
communication is based out of graphs, charts and images that are directly related to the
statements being shared.

The above figure represents some of the symbols used in visual communication. Some
of the tools that are used for the same include PowerPoint presentation, emails, news
articles, posters and pamphlets.
Another communication strategy that could be used for providing care to the children
with learning difficulty would be open source meetings and teach (Ray, 2017). This
would be characterized by the conduction of group discussions for the children. The
advantages associated with this strategy would be development of confidence of the
children with learning difficulties. This would reduce the quantum of difficulties
associated with the learning, as poor confidence in the children is one of the major
factors of learning difficulties. Additionally, grouping of similar children would enable
poor performing children to learn at their own pace. However, the limitations of the
same would be the following:
 The children with learning difficulty might not open up during the initial phases of
the meetings. This would reduce the overall efficiency of the care program.
 Conduction of such a meeting requires high-trained trainers that are difficult to
find in the long end.
Demonstration of safe and effective practice
Some of the effective practice includes person centred care support for the children.
This practice would be effective within the professional boundaries due to two reasons:
 Person centred care would result in effective communication between the service
provider and the service acceptor. In long end, this would result in the efficient
mitigation of the learning issues faced by an individual (Moore et al. 2017).
 The mentioned care would enable the integration of the support services
associated with the module. This would include the department of health and
child support for their input in the development of the most efficient learning
module based on pre tested method.
Some of the safe practice includes the Children and Families Act. However, major
subsections of the act would be taken for safe practice with the professional boundaries.
