INTRODUCTION
Integration of care at the micro level is concerned with the co-ordination of care for
individual patients and careers. This report highlights local resources which are available to
support the integrated care. After that the report highlights current need related to the health care
and support service. After that the report highlights individual skill in health and care in
identifying their own need in the market. After that the report describes the health, care or
support service practitioner in supporting person-centered care. After that the report goes on to
explain leadership within the remit of own role and multidisciplinary team working and the role
in transferring information in team. After that the report explains different communication
method which can be used to provide appropriate support to different individual. In the end the
report highlights communication strategies in identifying and responding to the needs of different
service users.

Multidisciplinary care used to take place at the time when professionals from a range of
disciplines work together to deliver comprehensive care which can help in addressing as many of
the patient's health and other needs as possible (Fleischman and et.al., 2016). There are many
resources which are available in the market to improve the level of the integrated health care in
the nation some of them are as follow:
Integrated voluntary approach in STP/ICS programme: It is one of the best resources
which is available to improve integrated care in the country. This resource will help the patient in
getting the integrated care at minimum cost as the funding of all this project will be done by
primary and community services to help build ‘primary care networks. These GP-led
partnerships mean people can see an expanded professional team in the local community, which
could include therapists, nurses and mental health staff plus non-clinical support for concerns
like social isolation or debt. This resource will also help the hospital industry and patient in
understanding the data about the intelligence health care and wellbeing of the nation public in the
organization. This will eventually tempt and force them to have a better day to day physical and
mental health and reduce health inequality between different group in the country. This resource
was developed by NHS. NHS has compiled this resource of integrated health with the number of
