Impact of IOM Report and State Coalitions on Nursing Practices

This report examines the Institute of Medicine (IOM) report, "The Future of Nursing: Leading Change, Advancing Health," and the role of state-based action coalitions. The Future of Nursing
Nursing is a significantly broad field with a variety of aspects which include but not
limited to treatment, disease prevention and management, patient care, and health campaign. As
a broad area, it is bound to have its fair share of challenges. In that regard, the Institute of
Medicine (IOM) and the Robert Wood Johnson (RWJF) formed a program to assess and make
recommendations for the future of nursing. This paper seeks to shed light on the role of RWJF
initiative and the IOM report, including a review of State-Based Action Coalitions, state action
initiatives, and challenges.
The RWJF initiative core mission was to generate a report with recommendations for a
more action-based design for the nursing profession. The committee’s role was examining the
capacity of the broad nursing workforce to meet the demands of a more reformed health care
approach. The committee looked into the capacity of nursing education, healthcare setting, and
the delivery of health care services (National Academies, 2018). By making changes in
institutions and public policies at different levels of the government, the committee
recommended specific actions. The recommendation stressed the aim of highlighting the key
nursing roles when it comes to the design and implementation of an efficient health care system.
Thereby, suggestions were made about conceptualizing the roles of nurses in the context of the
whole health sector, shortages, societal issues, and the advancement in technology (NCBI, 2015).
Additionally, the committee suggested expanding the nursing profession by restructuring the
whole nursing education and enlarging the capacity of the nursing school to produce adequately
groomed nurses (American Nurses Association, 2019). Finally, the committee stressed the need

to attract and sustain the appropriately groomed nurses in many health care settings and facilities
across the nation.
Basically, Action Coalition is an incentive that operates at both the local and state levels.
State-based action coalitions are made up of a team of collaborators who work to make sure
nurses practice at their full potential for the realization of the highest quality healthcare. The
coalitions link with individuals at state levels and national organizations to make positive
advancements to health care (Cherry & Susan, 2018). They are responsible for discussions on
structure and health care processes developed by various states. The state-based coalitions do so
to make collaborative move towards the implementation of the IOM recommendations while
paying attention to unique state requirements and needs. The step helps to establish and maintain
desirable change not only at the state levels but also local and regional levels (APIN, 2019). On
that note, the coalitions play significant roles in making sure the quality of health care is
considerably high. Precisely, their actions help in the implementation of the recommendations of
the IOM report thereby ensuring nurses commit to provision of quality care to the entire
American population.
The state of Indiana Action Coalition has been central for health care delivery in the state.
Coined in the year 2011 via joint move between the Center for Nursing and the Center for Area
Health Education, the coalition has aided in the implementation of the IOM recommendations
across the state. Significant concentration has been on nursing practices, nursing education,
interprofessional education, and patient safety concerns. Two of the state’s Action Coalition
initiatives are; access to care and implementation of Indiana state scholarship programs. In line
with access to care, from 2013 onwards, the state-based action coalition has been jointly

collaborating and communicating with more than 30 healthcare-related organizations across the
state to help generate ideas on improvement on the sound utilization of state resources to meet
healthcare needs. Secondly, the scholarship program initiative has been supportive of the future
of nursing. The program is continuously raising funds which are awarded as scholarships to
educate and train nurses at both undergraduate and graduate levels (Indiana Center for Nursing,
2019). The move continuously helps nurses to be in preparation for the complexities and
diversities of nursing profession.
To conclude, although the state has been profiting from the IOM program,
recommendations, and state-based action coalitions, there are barriers to advancement that
currently exist. Firstly, a section of the key state stakeholders fails to work collaboratively. A
significant number of health care advancements call for equal participation for the realization of
positive results thereby posing great challenges to state health care delivery (The National
Academic Press, 2016, p. 39). Luckily, the nursing advocates have stepped forth to help remedy
the situation by defining stakeholder roles and campaigning the need for harmonious
partnerships. Secondly, the budget is too tight to fully accommodate the scholarship programs
among many other state-based ventures. This has stressed the need for constant budgetary
adjustments to match the growing needs of the state-based initiative.
