Nursing Care Report: Analysis of Ischemic Heart Disease at SMMH

This report presents a nursing care analysis for Mr. Ian Dury, a 58-year-old male admitted to Sisters of Mercy Memorial Hospital with chest pain and a history of Ischemic Heart Disease (IHD). Introduction
Nursing refers to an important profession of medical field in order to provide appropriate care
services to patients for their wellbeing. It includes their role of maintaining safe environment for
people along with delivering better quality of care. In context of this project, it is based on the
case study of the patient named Mr Ian Dury having problems of ischemic heart disease from a
long time. He is 58 years old man and has medical history of several health problems including
Type 2 diabetes mellitus, hypertension, chronic kidney disease etc. This assignment will focus on
primary diagnosis and nursing problems which are assessed earlier related to selected patient. It
will also include nursing management and discharge panning for an individual.
Primary Admission Diagnosis
In context of given case study, it has been analysed that patient has been admitted due to facing
the problem of chest pain. It includes the primary diagnosis of carrying out physical examination
to analyse the quality, intensity, duration and location which helps to assume the reason behind
the same. Meanwhile, it is observed that there are number of reason behind occurrence of chest
pain such as angina, pericarditis, cardiomyopathy, aortic dissection and heart attack. It consist
the criterion of evaluating symptoms such as chest pain, heart attack, breathing difficulty,
breathlessness, abnormal heart rhythm and so on. However, it is observed that several other
pathological procedures are carried out to examine actual condition of ischemic heart disease.
Electrocardiogram (ECG): This method includes the procedure recording electrical signals as
they are responsible for travelling via heart of patient. It is also helpful often in terms of
revealing evidence of a previous heart attack or one that’s in progress.
Echocardiogram: The echocardiogram utilises sound waves in respect of producing images of
heart of patient to determine actual cause of chest pain to make decision of treatment. It is
helpful to doctors for identifying that all the parts of heart wall are contributing to pumping
activity of heart in normal manner.
Exercise stress tests: This method is based on the fact that signs and symptoms are most often
occur at the time of exercise which is known as stress echo. In some of cases, it has been
analysed that medication is also used by physician at place of exercise in order to stimulate heart
of patient.

Nuclear stress test: This diagnosing method is similar to ECF recordings as it is based on the
criterion of measuring blood flow to heart muscle at rest ad during stress. It consist the way of
injected a tracer into blood stream of patient along with special cameras that are capable in
detecting areas in heart which are responsible for receiving less blood flow.

In context of given case of Mr Ian Dury, it has been analysed that nursing problems diagnosed
for selected patient are acute pain and reduced cardiac output. It is required to manage these
