Day 1
1.John’S blood test clearly shows elevated levels of lipids. His LDL is 5.6. LDL is also called
the bad cholesterol. This when found in excess deposits on the walls of arteries. The normal
range of LDL is till 3.34. Anything above 4.9 is dangerously high. In john’s case, we can say
that his bad cholesterol levels are dangerously high. In the case of HDL, the normal range is
till 1. In John’s case, the HDL is 0.4. HDL is the good cholesterol because it removes the bad
cholesterol (Toth, 2005). But it is too low in here. It means the good cholesterol which can
remove the bad cholesterol from depositing in the arteries is too low. John has trigs at 5.7.
Triglycerides or trigs are also a type of cholesterol which helps in increasing the bad
cholesterol count. The limit is till 1.7 . In his case, the triglycerides are also very high . Now
coming to total cholesterol John has his total cholesterol 9.4. The normal range is 5.1. The
total cholesterol as the name says is the total of all factors of cholesterol. In John’s case as
we have all the other harmful values of cholesterol is high, the total. In his case, the
cholesterol is on the alarming side as you can see that it is far ahead from the normal range.

wants. His weight gains clearly show that he does not have any dietary restriction. He is also
smoking since many years , this also contributes to atherosclerosis. Finally coming on to the
blood test LDL is very high. LDL deposits on the wall of arteries causing atherosclerosis
(Zwaka et al, 2001). His HDL is low which prevents the deposition of LDL. Rest all the values
of his lipid profile is high. Hence we can say that he has all the factors which can cause
atherosclerosis.
3.Atherosclerosis developed in John under various condition . Development of
atherosclerosis took place slowly in John. Atherosclerosis has also caused narrowing of
arteries which has lead to high Blood pressure. John is having overweight . The main reason
for the development of atherosclerosis is high levels of LDL. LDL is the main cholesterol
which deposits on the walls of arteries causing them to block. As he is already having high
blood pressure due to narrowing of arteries. LDL cholesterol deposits on it making the
artery narrow (Toth, 2005). Rest as HDL is very low there is nothing to prevent the
deposition of LDL. Along with LDL the other parameters of cholesterol is also high causing
the arteries to block gradually over time. As the amount of cholesterol is too much to be
utilised by the body it deposits on the arteries.
4. Atherosclerosis can affect any artery in the body (Carew et al, 1987). It affects the
coronary artery most commonly. It is very rich in oxygenated blood flow. It supplies oxygen-
rich blood to our heart. We should prioritise the coronary arteries first. Keeping in mind that

venerable for atherosclerosis.
DAY-2
1.As john did not take any medicine until morning. John was already thinking about the
doctor’s appointment constantly. During the night and main reason of his restless was the
increased blockage of the coronary artery (Heusch et al, 2014. In this, the blood supply to
heart decreases. The heart has to make more efforts for pumping oxygen. This was the main
reason of night panicking and struggle for oxygen and also clinical visit a day before has
made him stressed by morning. There was also a lack of sleep since last night and the heart
was also over working for oxygen causing stress.
2. The increase in anxiety in John condition was due to the fact that his coronary artery was
almost blocked. The heart was not getting the required amount of oxygen.Hence causing
chest pain, increased breath, blood pressure and other symptoms. Also due to blocked
artery the blood pressure has gone high. His blood reports later that evening showed
elevated cardiac troponin and increased CKMB which are increased in the case of
myocardial infarction Myocardial infarction occurs when blood flow to the heart stops and
finally heart attack takes place (Thygesen et al, 2012).
3.Glyceryl Trinitrate is given to relax the blood vessels (Loscalzo, 2013). In this situation, the
blood vessels are constricted due to atherosclerosis. The medicine will help the blood
vessels to relax. Widen the artery and start the blood flow again to the heart. It acts within a
minute or two. Aspirin prevents the stickiness of platelets . It prevents the platelets from
sticking to the site of atherosclerosis. Sticking of platelets can cause a complete block of
artery hence the administer of aspirin is important. It reduces the blood clotting.
4.Beta blockers are given to prevent further develop of any other atherosclerosis. Also, it
has been seen that the mortality has gone down considerably if a dosage of beta blocker is
taken on a daily basis. As John is at an advanced age, administering beta blocker will help in
increasing his life span (Swedberg et al, 2012). ACE inhibitors widen blood vessels hence
improving blood flow. Widening arteries cause blood pressure to go down and blood flow
increases to heart.
4.It is important to remain on a low lipid diet to stop further development of atherosclerosis
in any other artery of the body. This condition can develop anywhere or any other artery of
the heart. It is important to prevent further development. Also, it is important to prevent
obesity as it can cause a reoccurrence of the event and atherosclerosis can take place again.
DAY -3
1. As per the blood test result, there are increased levels of cardiac troponin, CKMB and
cardiac lactate dehydrogenase. All these factors are suggestive of cardiac tissue damage. As

John has already diagnosed with myocardial infarction. It is suggested that his heart muscles
have been damaged to a great extent due to lack of oxygen supply causing myocardial
infarction. Damage to heart muscles has caused elevated levels of all the given factors. John
blood glucose is constantly high. It is possible that he is having diabetes type ii. As per his
age and blood glucose levels, it is possible to have diabetes.
2. Elevated levels of cardiac troponin, CKMB and cardiac lactate dehydrogenase are
suggestive of cardiac muscle damage (Kristjansson et al, 2016). The symptoms started as
breathlessness at night , there was acute chest pain . All these are suggestive of myocardium
damage.
3.ACE inhibitor has been very effective in John’s case. It was given initially when he came to
the hospital. This has caused dilation of the arteries hence oxygen reaches the heart and
pain have reduced. From this we can conclude that there was an obstruction of blood and
oxygen supply to the heart muscles, hence causing all the symptoms.
4.In this case, the type of beta blocker is changed , it was a Non-selective β-blocker it is

patient is not under any medication for it. John is not under any medication for lowering his
blood sugar. Hence there are chances of having Diabetic Coma.
DAY-4
1. The recent condition based on a blood test is Diabetes type II. He is 62yrs of age,
overweight and also having high blood sugar since long. His lipids are also at a very higher
side. His waist circumference is too high which is also a contributor to diabetes type II. He
has now started passing high volumes of urine. Polyuria is one of the chief symptoms of type
II Diabetes ,so it's definite that he is been suffering from diabetes type II.
2. Since many days the blood glucose levels of John has been constantly high. The
contributing factors can be overweight. No exercise since he retired and also living on his
own so there is no one to check on his diet.
3. Since many days the presenting complaint of John and the line of treatment has been
towards atherosclerosis. The reduced urine output and swelling of limbs can be due to the
development of COPD as he has been smoking since long. In COPD there is stress on the
right side of the heart. It causes swelling of lower limbs. John is constantly maintaining high
blood sugar. In this situation, there is a large amount of urine passed. When glucose

concentration is high. It is secreted out of the body. Water also follows glucose
concentration causing polyuria.
DAY-5
1. Smoking cessation will improve his chances of survival and better recovery. The
outcomes of chemotherapy will be better. There cannot be benefits if the cancer is in a
terminal stage. But quitting smoking at the stage in which John is , is beneficial for him. As
nicotine addiction causes serious side effects but its withdrawal can be worse. Quitting
smoking all of a sudden is not easy (Kim et al, 2013). But it can be done gradually. It will also
cause less strain on heart and circulation.
2. Chemotherapy has many side effects throughout the body. Loss of appetite is also one
the side effects due to chemotherapy. There is always a feeling of full though there is
nothing eaten. But still one has to eat properly. Due to loss of appetite, there is weight loss
and also the nutrition required is not received by the body. This can cause a delay in
recovery. The loss of appetite is due to elevated levels of transforming growth factor alpha.
This also causes further fatigue.
3. Red blood cells are produced in bone marrow . A hormone released from kidney called
erythropoietin sends a signal to the bone marrow when to make red blood cells. During
chemotherapy, the bone marrow may damage temporarily. But it recovers as chemotherapy
gets over. Further in few cases, there may be kidney damage. Then the hormone released
from it erythropoietin secretion will be inhibited. Hence causing reduced erythrocyte
production and causing anaemia.
Anaemia is very common in patients with cardiac failure. This is due to deficiency of heamtic
factors. This becomes worse in the case of poor renal function. The main reason of anaemia
can be a loss of erythropoiesis (Kautz et al, 2014).
Day-6
1. John has been diagnosed with deep vein thrombosis. There are many factors associated
with him which have contributed to his condition. Firstly having an injury to veins . Recently
John has gone under atherosclerotic surgery. There might be a possibility that unknowingly
some vein might have got hampered. Secondly, John is overweight. Being overweight puts a
lot of pressure on the vein. It causes slowness of blood in the vein. Another risk factor is
smoking. John smokes a lot. Hence his chances of getting diseases due to smoking are very
obvious. Also, he is not exercising enough post-retirement hence that is also contributing
towards his condition. Being seated at one place also contribute to causing deep vein
thrombosis.

2. Deep vein thrombosis affects very large veins particularly in lower extremities of the
body. It causes the blockage in blood flow. Blood is accumulated in that area. It thus further
causes swelling , pain and due to the accumulation of blood, the skin is warm to touch.

embolism. There it can block a blood vessel.
DAY -7
1. Copd mostly occurs in people who have a history of heavy smoking. Almost 90 percent
of cases are due to heavy smoking. In the case of COPD, the cigarette smoke goes inside the
lungs. In the lungs, there are air sacs present in which exchange of gases takes place. When
cigarette smoke reaches the air sacs of lungs it damages them. The air sacs break open and
create a large air sac. Due to the slow merge of small air sacs into large air sac, the capacity
of exchange of oxygen and carbon dioxide decreases. It makes difficult for absorbing
required amount of oxygen in the body. Hence, breathing becomes very difficult.
2. The first thing John might develop is to sleep problems. Because the body is not getting
sufficient amount of oxygen. Hence there can be problems in sleep . In the case of
progressive COPD, the lung can collapse (Hubner and Herzog, 2016). In this air will escape to
chest cavity. It is required to stop smoking immediately. Also, there can be a condition called
Cor pulmonale. It is the collapse of the right side of the heart. The right side of the heart
pumps blood into lungs. This happens because it has to work vigorously to pump blood in
lungs because lungs are damaged due to COPD and not able to work as per their capacity.
3. John is constantly tired and he is not getting out of bed much. He is trying to
compensate his condition by struggling to breathe, increased heart rate and also he is
having severe dyspnoea.
4. COPD and left side heart failure are not directly related. But the low oxygen in blood
puts pressure on the heart. It can make the condition of the left ventricle worse as it has to
take an extra load to pump required amount of oxygen. The right side of the heart pumps
oxygen into lungs. In case lungs are no more efficient the heart will be strained to pump
blood. It increases the chances of right side heart failure.

Day-8
1. There is a specific factor which may cause malnutrition . John was under chemotherapy
for lung cancer. Due to chemo, there is severe loss of appetite. Hence it is easy to get
malnourished in this condition.
2. In john’s case, there are many aspects which will make him vulnerable to respiratory
diseases. John is a smoker. That is the main factor which has already caused him COPD. Also,
he is overweight. It makes difficult for his heart to pump blood to his entire body and
eventually to lungs. He is not exercising. He does not put himself under stressful condition.
His breathing becomes weak eventually. He has left side heart failure . This affects the
amount of oxygen reaching lungs. Finally, John is already having COPD. He has gone
through a lot of treatment in the current week. He is also under chemotherapy.
Chemotherapy makes immune system weak (Zheng et al, 2015). His daughter sitting next to
him is having a cold. He is susceptible to infections. Hence due to his weak immune system,
his chances of catching infection are very high.
3. When there is an infection in the body stress hormones are released. These causes rise
in blood glucose levels. Hence the sugar levels have to be monitored constantly in this case.
As the blood sugar levels rise the microbes living in the infection site also develop ways to
survive in raised blood sugar environment. Prevention can be done by curing the infection.
Hence the blood sugar level won’t rise further due to infections.
4. Cor Pulmonale develops as a complication of COPD.Due to this oedema in the lower
extremity of the body can occur . This can be a sign of cor pulmonale( failure of the right
side of the heart) . Moreover, John had swelling in the calf region of his left leg . This was
mainly due to deep vein thrombosis. In this, the blood is blocked causing swelling of that
particular region along with pain. The swelling in case of DVT has been cured. In the case of
COPD, the infection requires treatment so that the lungs can work as per their capacity and
heart need not take any extra effort to pump blood. Once COPD is treated the swelling in
the lower extremities will go away. He can be on bronchodilators.It will reduce COPD.
Further causing less stress on the heart and hence reducing swelling.
5. Since past one week, John has been diagnosed with two very severe lung diseases. First
is lung cancer. It has been diagnosed during his hospital stay. He is under chemotherapy
therapy for the same. Chemotherapy might help him get rid of cancer. He has to quit
smoking for best results of chemo.
Another thing he is suffering from is COPD. It has resulted mainly due to his smoking habits.
It has caused him difficulty in breathing and severe dyspnoea. He has chances of developing
right side heart failure. John has to quit smoking to improve his condition. If he does not
quit smoking then the condition might go very worse. He is on bronchodilators for COPD. He

will act as the anticoagulant and prevents the chances of formation of emboli.
