Please read these instructions carefully as they explain how you need to complete and submit this 360° Assessment form at the
end of Semester 2. Failure to submit this form correctly may result in a loss of 20 marks. It is your responsibility to complete this
form and submit it to your workshop facilitator no later than 13th May
As a result of working within a team for this assignment you are required to reflect on this process and undertake peer evaluation
on the contribution your team members provided and self-evaluation on the contribution you provided in completing the team
project. Your workshop facilitator will also provide an assessment of your contribution. All of these individual assessments will be
taken into account to determine your final score for this element.
Please note during the assessment period, individual team members may be contacted by the module team in relation to this
element if any issues become apparent which have not previously been reported to the workshop facilitator. However, individual
confidentiality is assured.

I take responsibility for my choices, I always try to asked questions when I don’t understand especially during lecture sessions, I always try to do my best all times , I treat others
people with respect when we are working has a group as well I help other people in my group, I always try to my best to finish what I start ,I offer help before I am asked to and one
thing I will remember to do in future is to work hard in my weakness and remain focused so that I can achieve my carrier goals .

It’s good to point that you work by assessing what resources are within you and our greatest resource is people and this as no limit you can ask for help through the internet, phone or
even in person. And if she can put this into practise I am sure she can do better.

One thing I really want to say is that she need to prioritize and make schedule and stick to it with no exception until she get the right balance . This so because when you let something
sinks into your subconscious and gesture you are likely to come back with better ideas.
