Please read these instructions carefully as they explain how you need to complete and submit this 360° Assessment form at the
end of Semester 2. Failure to submit this form correctly may result in a loss of 20 marks. It is your responsibility to complete this
form and submit it to your workshop facilitator no later than 13th May
As a result of working within a team for this assignment you are required to reflect on this process and undertake peer evaluation
on the contribution your team members provided and self-evaluation on the contribution you provided in completing the team
project. Your workshop facilitator will also provide an assessment of your contribution. All of these individual assessments will be
taken into account to determine your final score for this element.
Please note during the assessment period, individual team members may be contacted by the module team in relation to this
element if any issues become apparent which have not previously been reported to the workshop facilitator. However, individual
confidentiality is assured.
Fill in your name and the names of each member, then assign points for each category and total the points. You should add comments and further
