training sessions has been organized by the leading teams in healthcare and the leader provide
the knowledge to the medical professionals in order to increase their knowledge so, they can use
this while treating the clients.
Analysing the values for using theoretical models in order to build the successful health and
social care teams
The theoretical models are the base that ensures the effective health and social care team. The
leading team in health. Effective implementation of the team ensures the effective
communication in the organization and this results in the better outcomes of the patients. The
health care manager ensures that there is effective communication has been done between the
service providers so, there no barrier has been faced while treating the patient. In order to build
the successful team in health care the theoretical modals has been adopted. (Doekhie and et.al,
2017). This theoretical modal helps in implementing the effective team in the healthcare and
ensures the effectiveness in the healthcare. In order to understand this with more description here
are the theoretical modal of team in healthcare:
Multidisciplinary- This refers to the collaboration among the different teams within the
healthcare. The collaboration among the different team has been done in order to ensure the
effective care plans to the patients. The team based strategy ensures the quality care to the
hospitalized patients. The coordination among the different teams results in low cost, high

output. Effective interaction among the members of the different team results in creating the
highly effective communication among the different teams within healthcare. This model of
teamwork has been proven highly effective when the healthcare is addressing to the complex
cases and attending the complex patients. (Ivers and et.al., 2019). In such situation, the
coordination among the different team ensures the quality treatment to the patients and along
with this is also ensures the better communication process between the patients and the service
providers.
Interdisciplinary- This refers to the group of the professionals in healthcare within determining
the various areas of the expertise. And all professionals and experts works together in order to
achieve the goal and provide satisfaction to the clients. This approach consists the different
health care professional from different disciplines. These experts come in a team and works in a
collaborative form in order to ensure the high services to the patients. These experts or group of
experts handles the high cases. And ensures the effective treatment by coordinating with each
other. All the members in this are highly skilled and knowledgeable and respects each other's
autonomy. (Challis and et.al., 2018). This results in increasing the efficiency while building the
team. Such experts with calm nature always results in the effective implementation of the team.
This team focuses on the psychological and the spiritual needs of the patients. The group of
healthcare providers from the different background works with the major motive of full-filling
the needs of patients in order to ensure the effective health.
Trans-disciplinary- This model refers to that team approach that is concerned with the family-
centred process. It works with the main motive of supporting the families of the young children
who are facing the disability or delays. In this team approach, the one member of the
multidisciplinary team has been appointed and receives the guidance and the coaching from the
other team members. (Mabel, Altinay and Ferrando, 2019). This is that approach of team under
which the different members come together and communicate with each other and comes up with
the solution that is related to the particular healthcare services. This model enables the different
team members of the healthcare to come together and also enables them to work with the
cooperation in order to ensure the effective services to the patients.
This models helps in implementing the effective team in the healthcare. These models determine
the skills and knowledge of the experts and also helps in creating the different formation in the
team in order to ensure which case has to be handled by which team. The roles and

responsibilities has been cleared with the help of these models. Thus, this ensures the effective
team building in health and social care.
Understanding the stages of team development in order to make plans and motivate the team
members
Team development refers to the development of each individual of the team in order to ensure
the effective team. The team development focuses on creating the effectiveness in the working of
the team members in order to ensure the achievement of the goal. In a team there are large
number of members and all of these members are concerned with the different personality and
this leads to create the conflict in them. Therefore, the stages of team development has been
performed in order to ensure the effectiveness of the team. (Liaw and et.al., 2018). The health
and social care services are concerned with providing the services to the patients and increasing
the effectiveness of the health. The team development in the health care is important as it ensures
the effectiveness in over all working. In order to understand this with clearance here are the five
stages of team development that has been given under the Tuckman,s team model:
Forming- This stage of team development involves the interaction between the team members. It
has been considered as the period of orientation where the ream members has been come
together and introduction among them has been take place. The forming stage involves
determining the team goals, individual roles, the strategies and the ground roles. In health and
social care while forming team the leader and team members meets with each other and the
introduction has been done and afterwards, the leader describes the goal of the team. The basic
goal of the team is to provide the effective healthcare services along with maintaining the fair
practices. The goal has been introduces with all the team members and then, the individual roles
has been determined to members. (Lee and Doran, 2017). The clarity in the roles of the team
members result in performing the task by each individual in effective manner. Then the strategy
has been prepared in order to achieve the goal. The health care experts determines the different
strategies with the healthcare workers and then on the basis of that. The task has been started.
The general rules have been formed by the leader. And all the members has to follow those rules.
This stage of building the team results in the better interaction among the members and within
this, it defines the goals, roles, strategy and rules to the members. A better understanding of all

these elements results in the less number of disputes in the group. And the effective leading team
in health care results in providing the effective services to the clients.
Storming- This stage of building team has been considered as one of the critical stage. The
health care team is consist of large number of members and all of them have their own
knowledge and skills. When all these experts interacts with each other. Then, it is hard to create
the peaceful environment as all of them has a sense that they have more capability. In such
situation, the large number of clashes takes place and this affects the services of the healthcare.
Maintaining a peaceful environment is mandatory in the healthcare as this ensures the effective
services to the patients. Therefore, the healthcare crates the sub-groups under the groups in order
to ensure the effectiveness of whole group. (Peng, Wang and Chen, 2019). This stage refers to
sub-grouping in the main group and then the task has been assigned to each sub-group. This
leads to the completion of task in effective manner and this also ensures the less number of
disputes in the team working. The leaders make the sub-group after analysing the service
providers understanding level. And this also leads to create the development of the healthcare
workers in the group. The sub-group helps in the better development of the service providers.
For examples: For treating the particular diseases the particular sub-group has been created. This
adds value in the team working of the healthcare. The effective services has been provided to the
patients.
Norming- After passing the storming stage, the focus on this stage has to be made. This stage
enables the team member to work together and also enables to share the common interest. The
roles of each member is clear in this stage. This stage enables the team members to understand
each other. The healthcare leaders and managers consistently monitors the sub-groups that has
been made by them and along with this they also ensure their positive working. As the positive
behaviour by each member plays an important role in health and social care. The healthcare
services are based on a theory and that is related to implementation of positive behaviour in the
patients so, the patients can improve. (Grossman and Feitosa, 2018). The same theory is applied
on the service providers while developing the team in the healthcare. The positivity among all
the members ensures the positivity in providing the services. This stage focuses on creating the
positive behaviour in each member.
Performing- This stage focuses on the working of the member in the group. As stated earlier,
the main motive of the healthcare team is to ensure the effective services to the patients. All the
Document Page
policies and procedures has been taken for this only. The leaders monitor the performance of the
service providers and on the basis of that the guidance has been done. If the leaders found that,
service provider needs guidance in order to treat the patients. Then, the healthcare leaders
provides them the guidance. And boost them to perform in more effective way. This stage
ensures the effectiveness in the performance of all team members so, the over all effectiveness of
the team can be increased. (Super, 2020). Besides this, in this stage the purpose and the roles of
the members has been re-call to them so their major focus on their goal has been in a consistent
form.
Adjourning- This stage refers to the implementation of the new goal along with the existing
goal. For example: in healthcare services the leader has determined the main goal and that is to
provide safe and effective services to the patients. Along with this, the new goal has been added
in the team. Such as, implementation of the healthcare theories in effective manner in order to
provide the effective services to the patients. All the steps in building the team in health care is
related to providing the effective services to the patients. (Stup, 2019). Thus, this proves that the
leading team in health and social care are concerned their patients. The theoretical modal
increases the values of building the healthcare team by providing the effective vision to the
leaders in order to create the unity in the team. This creates the immense benefits for the patients.
All of these stages of the team development leads to motivating the team members in
effective manner the first stage results in making the interactions between the team members and
defines the clear goals and responsibilities. The second stage refers to creating the sub-groups in
order to provide solve the complex situation. (Lacerenza and et.al., 2018). The third stage
enables the team members to share the common interest with each other and also enables them to
learn new things from their teammate. The fourth stage helps in the evaluation of the
performance of the members and on the basis of that, the healthcare experts provides training to
them. And the last stage ensures the completion of the goal. And also the new goal has been
added in order to increase the productivity of the team members.

Recommendations for open communication
Open communication is considered as the key ingredient for leading successful and effective
teams. It is not only vital for leaders but also important for team members so that they can focus
on productivity rather than indulging in politics or conflicts. In order to motivate individuals to
work together for accomplishing a common goal it is required that there must be fair and
productive communication. For this purpose, variety of approaches can be used by the leaders
and management. It is recommended that leaders may adopt different leadership styles
depending upon the service requirement but they must value and listen to their team members.
Nursing and health care environment are very complex and often most of the time leaders are
occupied with critical situations (Buljac-Samardzic, Doekhie and van Wijngaarden, 2020).
However, in such situations leaders must no neglect the concerns and needs of their team
members. There must be regular interaction among team members and leaders so that key issues
related to team working and development can be highlighted. Leaders must give equal
importance to the concerns of their team members. The effective communication practices such
as eye contact, active listening, acceptance to diversity, clarity in tone and language used as well
as gestures must be chosen with due care. The negligence to these aspects can often lead to
miscommunication and conflict among team members (Rosen and et.al., 2018). For encouraging
the quick and open interaction leaders must frequently ask about the suggestions from team
members. The feedback review process will make individuals comfortable in sharing their
concerns and express. Such expression is essential in terms of identifying the team performance
areas which needs significant improvements.
Another strategy which is recommended for effective communication is that leaders must be
fair and ethical. It has been observed that when leaders are not open to motivate or appraise their
team members for the excellence performance then it creates a barrier between team members
and leader. It also creates an unhealthy competition among team members to compete with each
other so that they can get the credit of work. Such approaches make the team to shift form shared
goals and efforts to follow the individual focus and rivalry among team members. In order to
accomplish the desired team goals, it is important that leaders must appreciate others as well.
This will develop a tendency among team members to share their perspective with each other
and to improve performance (Ervin and et.al., 2018). With evolving technology and
communication methods there is vast range of choices available for the professionals to use. It is

recommended that social care service providers must use effective communication and both
formals as well as informal communication mediums.
The use of informal communication during breaks or training period can create a strong
bond among team members. It is beneficial for improving the team performance (Morley and
Cashell, 2017). It has been also observed that lack of clarity in team role and responsibility is one
of the reasons that many individuals are not able to communicate with other team members. It
may also result in conflicts or gaps in performance. Thus is it recommended for the leaders that
they must ensure that team members have well defined understanding and clarity in terms of
their role and contribution in team goals. Another vital step which can be recommended to
leaders for encouraging communication is that they must make individual feel confident and
comfortable. It is particularly observed that in initial stages of team development when team
members as well as leaders are not known to each other or when the professionals are in their
training or transition phase then they do not have confidence or courage to speak up about their
concerns or thoughts. It makes them hesitant towards communication and this tendency is grown
throughout the team development phase.
Thus it is recommended that leaders must try to build an environment which is professional
but which gives courage and confidence to every team member to express them within team.
Leaders can also use informal gatherings and conversations with team members to make them
feel comfortable. For this they can use information communication tools like social media as
well. However, it is suggested that in social care such informal means must be used with all
necessary cares and in any way they must not violate the ethical standards of care practices,
confidentiality and duty (Smith and et.al., 2018). Open communication can also be engaged with
one to one communication so that team members can share their thoughts and concerns without
any fear. On certain aspect open communication also demands for confidentiality assurance from
leaders or management side. Thus leaders must also ascertain to their trustworthy and ethical
image and code of conduct.

CONCLUSION
From the above report it can be concluded that in order to deliver quality services it is
important that leadership and management skills must be developed and used in the practices.
Individuals must have ability to work in team because health and social care practices demand
for collaborative and diverse team working. It has been analysed from the discussions in the
report that team development takes place in various phases. If each of the team development
stage is analysed and understood, then it can lead to build and retain several team development
and growth issues.
While working in teams with not only other professionals but also with service users and
communities care providers may face conflicts on various issues and agreement. These issues
can be resolved by means of adopting suitable leadership styles which has crucial impact on
project outcomes. It can also be concluded from the study that in order to encourage the open
communication among team members and to enhance the effectiveness of functional team
regular efforts must be taken by the team members as well as leaders. It is also suggested that for
meeting the needs of individual’s professionals must choose variety of leadership style which
best suit care setting and needs of service user.
